This is a full-time opportunity working in adult health working five-8 hours shifts. Expected hours are 8AM to 4:30PM or 8:30AM to 5:00PM. There are weekend and holiday requirements for this role. POSITION SUMMARY This position provides comprehensive care coordination for patients as assigned. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for clinical quality of Care Coordination services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. The goal is to empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care of the population that it serves which includes planning for a safe discharge, continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient. CORE FUNCTIONS 1. Manages individual patients across the health care continuum to achieve the optimal clinical care, financial, operational, and satisfaction outcomes. 2. Acts in a leadership function with process improvement activities for populations of patients to achieve the optimal clinical care, financial, operational, and satisfaction outcomes. 3. Acts in a leadership function to collaboratively develop and manage the interdisciplinary patient discharge plan. Effectively communicates the plan across the continuum of care. 4. Maintains knowledge of Medicare, Medicaid and other program benefits to assist patients with discharge planning and choices. Knowledge of community resources relevant to health care, end of life dynamics, substance abuse, abuse, neglect, and domestic violence. 5. Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team. Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements. 6. Educates internal members of the health care team on case management and managed care concepts. Facilitates integration of concepts into daily practice. 7. May supervise other staff. 8. MINIMUM QUALIFICATIONS Requires a Master's Degree in Social Work, Counseling or related field (requirement is based on business need and regulatory compliance, all positions may not have this requirement). Requires a Licensed Master Social Worker (LMSW) (equivalent*) or Licensed Clinical Social Worker (LCSW) or have a MSW with the requirement to become licensed within 6 months of hire date. An equivalent license applies to states that do not recognize an LMSW; therefore, the employee must possess a Master's Degree and be a Licensed Social Worker. For assignments in an acute care setting, Basic Life Support (BLS) certification is also required. Requires a proficiency level typically achieved with 2-3 years clinical experience. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills. Must demonstrate ability to work effectively in an interdisciplinary team format. May have to take rotating call based on the Acute facility need. Banner Registry and Travel positions require a minimum of one year Case Manager experience in an acute care hospital. POSITION SUMMARY This position provides clinical assessment and psychotherapy services for defined patient population. Services are provided to patients, families and/or groups and may be in both inpatient, residential, and outpatient settings. Position provides treatment in one or more of the following areas: substance abuse, emotional and behavioral issues, eating disorders and other mental disorders as outlined in the "Diagnostic and Statistical Manual of Mental Disorders" (DSM). CORE FUNCTIONS 1. Provides assessment and direct psychotherapy to individuals, families, and groups. Coordinates all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care. Must be independently licensed in Arizona to bill out appropriate CPT codes for independently license clinicians. 2. Provides education and orientation for patients, families and others to assigned treatment program to aid in therapeutic process. Collaborates with the patient in the development of a written treatment plan. Includes patient and family involvement in assessment, treatment, education and discharge planning. 3. Monitors patients on an ongoing basis to continually assess needs and facilitate treatment and interventions. Documents clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality patient care. 4. Actively participates as member of the multidisciplinary treatment team and maintains effective working relationships with physicians and peers. 5. May provide clinical updates to representatives of managed care to assure coordination and reimbursement of treatments services. May conduct cognitive testing and document results. 6. Coordinates with community-based providers to assure optimal levels of care post discharge and to maximize treatment outcomes. 7. Provides critical assessments and appropriate treatment plan; Manages and provides direct care of patients in behavioral health treatment program (may be specialty area or across disciplines). Monitors, reassesses and documents treatment programs. Interacts with patients, families, groups, all levels of clinical staff in assigned program, physicians. External contacts include managed care providers, community resources, and referral agencies. 8. May provide indirect supervision to associate level therapists to support the process of becoming independently licensed. This may include weekly supervision and appropriate documentation. 9. May be required to review and sign off on clinical documentation, such as treatment plans and assessments from non-licensed staff within 72 business hours of completion. MINIMUM QUALIFICATIONS Requires a Master's degree in counseling, behavioral health, or a related human services field, or educational requirement as mandated by state board in order to maintain licensure in field of specialty. Behavioral Health Licensure is required in field of specialty as mandated by state board or regulatory agency. Depending on area of focus, may require a License in Clinical Social Work (LCSW) as mandated by state board. Requires a proficiency level typically achieved with 2 years experience in a behavioral health setting. Requires ability to work with a diverse population along with interpersonal skills and knowledge required in treating patients in area of assigned specialty.
