The Clinical Account Pharmacist will be responsible for supporting sales efforts through goals set by Reliance Rx management, supporting non-clinical sales staff, fostering supportive relationships with large practice specialist groups, attending community events, and other duties as needed to promote Reliance Rx's sales efforts from the clinical perspective. The Clinical Account Pharmacist must understand Reliance Rx products, services, distribution channels, the Reliance Rx distribution business, and payer networks. They will build relationships and be comfortable and confident in conducting meetings with physicians, consultants, and pharmaceutical medical affairs personnel. In addition, they will analyze potential client issues and needs and provide a solution to meet or exceed their expectations. Qualifications
Bachelor's degree required. Licensed to practice pharmacy in the state of New York required.

Summary of Position

POD Pharmacist
The POD Pharmacist helps develop and implement Monogram's medication therapy management (MTM) program which will ensure appropriate use of medication management resources. The POD Pharmacist will perform timely written member medication reconciliations and drug therapy action plans for all Monogram members and communicate them to Monogram clinicalstaff and treating nephrologists, primary care physicians and other providers. He or she conducts telephonic consultations with medical providers when medication changes appear necessary and speaks with members about medication use concerns. Review member care reports, clinical notes and other medical records as needed for medication action plan development, targeted clinical outreach programs and quality assurance purposes.
Serve as a pharmacist resource for Monogram field clinicians regarding medication use and Monogram MTM resources.
Assist with the delivery of MTM training sessions aimed at building a standardized and effective MTM program for Monogram clinicalstaff.
Monitor important medication availability and use trends for populations under Monogram's care management. Stay current with evidence-based prescribing for individuals with kidney disease.

Position Requirements
Bachelor's degree in pharmacy or PharmD
Active pharmacist license and reside in the state of position

Desired qualifications include:
