The Healthcare Transportation Associate (HTA) will be responsible for timely delivery of the Mobile Clinic to its assigned destination(s). Once the Mobile Clinic has been driven and delivered to destination, associate will operate as part of the care team performing a variety of clerical functions. HTA positions require competency in electronic health records. HTAs will focus on customer service and teamwork. The Certified Nurse Midwife (CNM) provides specialized medical services in the area that includes women's health care including prenatal, family planning, and preventive care. As part of a multi-disciplinary team of healthcare providers under the leadership of the Chief Medical Officer (CMO) or designee, the CNM works in a collaborative effort across all departments to remove barriers for accessing healthcare. The clinical care delivered is under the general supervision of the Ob Gyn MD, Clinical Director and Medical Director who will review the patient care standard and encounter note documentation routinely. Our providers must incorporate standard medical practices and health center policies into their clinical practice and deliver culturally sensitive care appropriate for an outpatient, primary care health center with a multicultural, multilingual patient population.

Medical Assistants III provide patient care under the orders and guidance of clinicians in accordance with Bay Area Community Health (BACH) clinic protocols and policies. Medical Assistant III, under Clinic Operations Management supervision, will perform various assignment within their scope of practice to support comprehensive quality medical care. Medical Assistants will be providing support in various clinic departments within BACH that include, but not limited to, Primary Care, Pediatrics, OB/GYN, Optometry, and Podiatry. The Healthcare Transportation Associate (HTA) will be responsible for timely delivery of the Mobile Clinic to its assigned destination(s). Once the Mobile Clinic has been driven and delivered to destination, associate will operate as part of the care team performing a variety of clerical functions. HTA positions require competency in electronic health records. HTAs will focus on customer service and teamwork. HTAs often interact with patients and provide services, such as but not limited to, making or changing appointments, checking in and checking out patients, handling payment transactions, verifying insurance and transporting patients. The Community Health Worker II (CHW II) at Bay Area Community Health will primarily work at serving and assisting our patients while located at different sites throughout the organization and community events. (CHW II) will work at different capacities based on their assigned area of training and specialty. The CHW II will be a trusted member of the community, and actively participate as a liaison between primary care, specialties and resources available to BACH patients. Activities may include, but are not limited to, outreach, engagement, care coordination, case management, linkage of the patient with resources at BACH and in the community such as food, housing, financial and transportation assistance and also linking the patient to care within the clinic.
