Qualifications & Experience:
Required:
Education: Minimum of 48 college units or an associate degree (AA) in a related field.
Experience with Children: At least 2 years of experience working with children ages 3-6 in an educational or childcare setting.
Leadership Experience: Minimum of 2 years of experience managing staff and motivating teams in a youth development or educational environment.

The Refugee Case Management Coordinator is an exempt position responsible for coordinating psychosocial, educational, social, and housing services. This position is also responsible for supervising, processing referrals, and follow-ups to ensure timely and appropriate service delivery through a network of providers.
Supervisor Responsibilities:
* Provide case management and social service oversight to all program sites
* Provide weekly clinical supervision to social work interns and provide case consultations at case staffing and FL-CASIC periodic staff meetings.
Duties and Responsibilities:
* Provide social work services for the program, which include individual assessments, individual counseling, and group counseling.
* Oversee the individual, family service, and wellness plans for each new client
* Conducts outreach to a network of social service providers to build a trauma-informed network
* Ensures client referrals to social service services through a network of providers to include housing, education, childcare, employment, and psychosocial services
* Ensure concise, timely entry, and completion of all documentation in the Avatar system within 48 hours of the event occurrence.
* Participate in monthly supervision, monthly staff meetings, and peer audits with the program director and staff
* Assist in the data collection to ensure program outcomes.
* Perform all other duties as assigned.

ESSENTIAL FUNCTIONS:

• Record all client payments or co-pays for services rendered and ensure accurate monetary transactions, daily balancing, and reconciliation sheets.
• Maintain all daily and monthly schedules for medical, mental health, and social services personnel.
• Check client eligibility with insurance carriers including completing Medi-Cal reservations for care (TARs).
• Assist in maintaining medical records filing system, including routing charts to providers, accurate record-tracking, medical record filing, loose-sheet sorting.
• Process record requests for outside entities in a timely manner while adhering to confidentiality practices and procedures.
• Provide clinic phone coverage including scheduling and canceling appointments, placing appointment reminder calls, and providing general information about services offered.

The Young Adult Court Program (YAC) works in collaboration with University of California-Irvine, Orangewood Foundation, Probation, the District Attorney's Office, Defense Attorney's Office, and The Orange County Superior Court. The YAC program provides engagement, assessment, clinical case management and resource coordination support to Transitional Age Young Men (ages 18-25) as part of a collaborative research study to reduce recidivism.
Position Summary:
The Care Coordinator III (CADC I) plays a vital role in supporting participants through a trauma-informed, person-centered approach. As part of a multidisciplinary team, the Care Coordinator III provides intensive case management services to young men with a history of nonviolent felonies and history of substance abuse. The primary focus of this role is to support participants in their recovery journey, helping them navigate legal, social, and substance-related challenges to achieve long-term stability and success. The Care Coordinator III delivers direct services, including assessments, case planning, workshops and referrals, while fostering a safe and supportive environment that promotes personal growth and accountability. The program offers a structured support system through four key stages: Engagement, Accountability, Stability & Sustainability, and Attainment.

These positions are not intended to be a part of continuing operations. A temporary employee will not change from temporary status to regular full-time or part-time status because the employee's assignment is extended. An employee's status only will change if the employee is advised of such a change, in writing, by USHS's HRD. POSITION OVERVIEW
We are seeking compassionate and organized Care Coordinators to join our team in providing comprehensive case management and support services to individuals experiencing or at risk of homelessness. The Care Coordinator plays a key role in helping clients access and navigate resources related to housing, physical and mental health, employment, and other social services. This position supports various interim and permanent supportive housing programs across the agency.
KEY RESPONSIBILITIES
Essential duties include, but are not limited to:
Conduct timely assessments for new clients to identify housing, financial, medical, and psychosocial needs.
Collaborate with clients and partner agencies to create and implement individualized service plans.
Provide referrals and support to connect clients with appropriate internal and external services.
Maintain clear, accurate, and timely case notes and client records in accordance with agency standards.
Support site operations by coordinating intake flow, resident needs, and on-site services and events.
Utilize motivational interviewing and strength-based approaches to build trust and encourage client progress.
Administer and track standardized assessment tools such as the VISPDAT, coordinating appropriate referrals.
Support a safe and welcoming environment for all residents, guests, and staff.
Identify and address mental health and/or substance use concerns by facilitating referrals to appropriate services.
Collaborate with program partners and participate in case conferencing and coordinated entry system (CES) meetings.
Assess client readiness for employment and provide referrals to workforce development programs as appropriate.
Assist with arranging transportation and other logistics to support client access to community resources.
Complete required documentation and reporting in a timely and professional manner.
Perform other duties as assigned to support program goals and client needs.
QUALIFICATIONS & REQUIREMENTS
Familiarity with housing resources, public benefits, and service systems within Los Angeles County.
Strong knowledge of case management practices, including trauma-informed care, harm reduction, and housing-first principles.
Experience working with individuals experiencing homelessness and/or co-occurring mental health and substance use challenges.
Proficiency in Microsoft Office and client data systems (e.g., HMIS).
Excellent verbal and written communication skills.
Detail-oriented, organized, and able to manage multiple priorities.
Effective problem-solving and crisis intervention skills.
Ability to work independently as well as collaboratively with a multidisciplinary team.
Bilingual (English/Spanish) preferred.
Must possess a valid California driver's license and have reliable access to a registered vehicle.
WORK ENVIRONMENT & PHYSICAL REQUIREMENTS
Work may take place indoors and outdoors in varied conditions.
May require standing, walking, bending, lifting up to 25 lbs., and extended computer use.
Must be able to travel to off-site locations and community meetings.
Potential exposure to individuals in crisis and to sensitive or confidential situations.
