Description of the Role: As a Physical Therapist on our Home Health Team at Interim HealthCare you will have the opportunity to make a significant impact on the lives of our patients. Whether you choose to work full time or part-time, you will be responsible for providing high-quality care and personalized treatment plans to our patients in the comfort of their own homes. This role is currently for Per-Diem with potential to transition to part-time or full-time. Responsibilities: Conduct comprehensive evaluations of patients' physical conditions and develop individualized treatment plans. Implement treatment plans and monitor patients' progress closely, making necessary adjustments as needed. Provide and instruct on therapeutic exercises, therapy techniques, and other interventions to improve patients' mobility, strength, and overall function. Educate patients and their families on the importance of compliance with treatment plans and home exercise programs. Maintain accurate and up-to-date documentation of patients' progress, treatment plans, and any changes in condition. Requirements: Valid Physical Therapy License in the state of Pennsylvania. Excellent clinical skills and knowledge of evidence-based practices in physical therapy. Strong communication and interpersonal skills to effectively collaborate with patients, their families, and other healthcare professionals. Ability to work independently and prioritize tasks in a fast-paced environment. Experience in home health or a similar setting is preferred.
