This position will be responsible for gathering resources, collaborating, and communicating with enterprise support to assist the Regional Director, Division Directors, and Office Directors in retaining, supporting, developing, and managing talent. The individual is responsible to align the division with BAYADA's People and Culture programs, both strategically and "hands-on", to achieve practice goals. The individual is to align resources through engagement of the broader organization and enable strategic execution, including leveraging the functional expertise in Enterprise support.
This is a Remote Role, but requires local travel within PA.
MINIMUM QUALIFICATIONS:
Four (4) year college degree. Master's degree in leadership, organizational development, IO Psychology, Human Resources Management, or a related field preferred but not required.
Five (5) + years of Human Resources Business Partner experience managing/overseeing HR disciplines, talent management initiatives, employee engagement, leadership development, organizational development, and employee life cycle processes.
Track record of successfully developing employees/teams at various levels.
Experience implementing talent management and performance management best practices.
Demonstrate key leadership competencies as defined by the organization.
Must have strong intellect, business acumen, collaborative style and relationship building skills.
Primary Responsibilities:
Partner with Regional Director, Division Directors, and Office Directors to define and execute culture and people strategies that align to the goals of the practices, as well as the overall enterprise.
Contribute to the development of and responsible for responsible for leading talent/workforce planning and onboarding of talent retention, management development/training, performance management, succession planning, workforce planning, compensation, and employee relations consistent with overall enterprise-wide, and practice strategies.
Develop, execute, and support the Regional Director and Division Directors in retention, management development, performance management, succession planning, compensation, and employee relations.
Act as advisor and coach to the Regional Director, Division Directors and Office Directors on all matters related to people and culture.
Actively support regional talent functions including performance reviews, workforce planning, talent roundtables, succession plans, development plans and performance improvement plans.
Advise leaders on coaching contracts, and coaching services. Track and monitor outcomes for coaching within the divisions.
Assist operational leaders in assessing and understanding skills gaps within the divisions.
Work with practice operational and functional leaders to support the people component of mergers, acquisitions, and joint ventures.
Implement practice-wide talent strategies that align with business goals.
Assist regional leaders in assessing, developing, and upgrading talent.
Understand and assess leadership gaps within the region(s); create plans to address gaps in coordination with practice leadership and enterprise support.
Diagnose and treat team performance concerns within the region(s).
Partner with regional leadership to assume responsibility for employee engagement implementation, outcomes, and responses.
Partner to identify insights and trends from various employee feedback loops and to conduct effective follow-up in response to employee feedback to foster high employee engagement.

What we require of you:

RDMS certified or ARDMS Registry Eligible.
CPR/BLS certification required through ARC or AHA.
At least 1 year of experience performing adult echo exams/ultrasounds. Stress echo experience preferred.

The AC coordinates all hospice referrals and is the lead coordinator of all client referral/admissions activity. This role collaborates with different departments to obtain necessary billing and clinical preadmission information and build effective working relationships within the work group to benefit our clients and referral sources. The position works with and supports the families and clients directly at the beginning of their hospice experience, which may include explaining benefits, coordinating care, and sharing knowledge about the services provided. Ensure that operations remain in accordance with BAYADA policies and procedures, and applicable local, state, and federal regulations.
Accept all new referral calls and monitor all referral fax lines and portals.
Contact the client and family, liaison and/or Program Manager, referral source, and branch staff within one (1) hour of the referral.
Enter all information gathered accurately into HCHB.
Contact the insurance company for eligibility and authorization. Obtain sufficient client and insurance information to confirm the hospice service line.
Perform an initial screening to determine suitability for admission to Hospice care according to Medicare, Medicaid, and third-party payer guidelines.
Maintain discretion, as the position could be exposed to highly confidential information.
Explain benefits to families and clients directly at the beginning of their hospice experience, working with and supporting them.

Confirm the benefit period and explain hospice services, benefits, and philosophy to clients, family members, and significant others.
Ensure maximum reimbursement through initial payer source, benefits, eligibility verification, and authorization processes.

Coordinate hospice durable medical equipment/ comfort kits in place upon client arrival and with hospital staff upon discharge to ensure a smooth transition for the client when arriving home.
Confirm and audit client charts in HCHB, ensuring all necessary documents are signed, dated, and uploaded to HCHB.
Participate as a member of the marketing team by following up on pending referrals, developing and maintaining positive relationships with referral sources, and assisting in developing strategies and plans for site and company growth.
Provide the Billing Coordinator, or designee, with insurance and billing documentation, client location, and client level of care as needed for final confirmation prior to admission.
Maintain communication with pending clients, families, and referral sources and routinely follow the progress of any non-admissions.
Participate in the service office morning stand-up calls.
Ensure operations remain in accordance with BAYADA policies and procedures, and applicable local, state, and federal regulations.
Attend in-services and continuing education programs when appropriate and assigned. Remain current on innovations, developments, new procedures, and techniques as they may pertain to referral information.
Promote BAYADA's philosophy, mission statement, and administrative policies to ensure quality of care.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist Assistant, PTA, for a full-time or part-time opportunity performing home health visits for our Senior Living Facilities office, located in Moorestown, NJ. This office services adult and geriatric patients on a per visit basis in territories throughout Burlington and Camdem County.
As a home care Physical Therapist Assistant (PTA) you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist.
Each Physical Therapist Assistant (PTA) must:
* Have current licensure or certification in the state of New Jersey.
* Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or
* Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977
* Have at least one year's work experience under the supervision of a qualified Physical Therapist.
