**The Case Management Assistant responsibilities include:**
+ Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
+ Responsible for assisting with distributing regulatory documents to identified patients
+ Educating patients about the appeal process related to their discharge
+ Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
