Responsibilities:
The Family Centered Treatment Therapist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
Qualifications:
Minimum of a Master's Degree in human services or related field is required.
Two years' experience working with youth or families.
Additional state specific requirements include:
NC, VA- QMHP or ability to register as a QMHP
MS- Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.

POSITION SUMMARY
The Mental Health Professional is a member of the multi-disciplinary treatment team and provides clinically appropriate services under the clinical direction of the Associate Director, Mental Health Programs. The Mental Health Professional will provide mental health screenings and assessments, clinical watches, treatment groups and individual sessions, as well as other clinically related activities.
DUTIES/RESPONSIBILITIES
1. Provide clinical services, including screening, assessment, treatment planning and crisis intervention.
2. Performs individual and group therapeutic counseling.
3. Performs administrative consultations for specialized social services as determined by the specific needs of the Institution.
4. Interviews inmates to obtain information concerning medical history, mental health history, or other pertinent information.
5. Observes inmates to detect indications of abnormal behavior.
6. Reviews results of tests, treatment plans and treatment cases with the behavioral health team to evaluate client needs and implement treatment.
7. Coordinates psychiatry services, which includes but is not limited to tracking and triaging patients' requests or staff referrals for psychotropic meds; administrative duties for psychiatry visits; tracking psychiatry services provided including signed consents and obtaining refusals.
8. Plans and administers therapeutic treatment such as counseling, behavior modification and psychosocial education to assist inmates in managing their mental disorders and other interpersonal or environmental problems.
9. Discusses progress toward treatment goals with inmates and ensures that the treatment plan is completed in a timely manner, adhered to as the barometer for treatment and is reflected in the progress notes.
10. Consults with behavioral health team concerning treatment plan and amends plan as described
11. Assists in planning and implementing the goals and objectives of programs and projects. May direct special projects as requested.

JOB REQUIREMENTS
LICENSING: Fully licensed, have temporary license or associate license in New Mexico a in Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW)

POSITION SUMMARY
The Substance Use Counselor is responsible and accountable for the delivery of contracted substance use disorder services in the facilities within the NMCD facility. Primary duties include the screening and assessment of psychiatric, medical, social services, employment and residential needs, as well as risk factors for re-incarceration as soon as possible after the individual's admission to jail. The Substance Use Counselor will also work with designated NMCD Staff to coordinate aftercare services in the community.
DUTIES/RESPONSIBILITIES
1. Assesses booking information on each new arrival to determine if there is a clinically significant problem with alcohol and/or other drugs of abuse.
2. Screens new arrivals with a substance use disorder to determine severity of need.
3. Reviews referrals received from healthcare providers, medical staff, programs staff and security staff to determine severity of need.
4. Effectively triages patients for addiction severity assessment, medical and/or psychiatric care focused on continuity of care and stabilization of condition.
5. Provides brief supportive interactions with individuals undergoing medically supervised withdrawal.
6. Completes Screening tools for particular substance use disorders.
7. Conducts assessment with referred patients to gauge the severity of an individual's substance use and provide a comprehensive overview of an individual's addiction-related issues.
8. Provides psychoeducational group therapy services focused on identification of addiction-related issues and the development of relapse prevention planning
9. Coordinates care with community providers and community supervision agencies, including the exchange of treatment records, communication of treatment needs and linkage of individuals with available services and support options upon release in concert with NMCD staff
10. Connects patients with any on-site services and programs available to assist with meeting needs.
12. Generates detainee passes for clinic visits and medication orders, collects patient signature and submits to the medical department staff responsible.
13. Obtains patient signatures on any refusals for medications and services.
14. Completes patient requests for health records in an effort to retrieve prior health records.
15. Communicates with individuals via request, triages the requests and schedules tasks within the electronic health record.
16. Coordinates discharge planning with designated NMCD Staff.
17. Ensures and maintains the confidentiality of all medical records information and patient care information.
18. Validates and ensures that forms for the release of information are current, accurate and complete as defined by HIPAA.
19. Releases information at the direction of the Psychiatric Provider, Mental Health Professional, Medical Director or Health Services Administrator.
* Refers to supervisor any requests of a questionable nature.
* Ensures the security of all active and inactive medical records.
20. Assists in preparing periodic statistical reports and other management information system requirements. Maintain logs for appropriate medical unit onsite staff.
21. Understands and complies with all state, facility and company policies, procedures and protocols related to the medical and mental health departments.
22. Meets timelines related to case management and departmental productivity guidelines.
JOB REQUIREMENTS
LICENSING: Currently licensed as Licensed Independent Substance Abuse Counselor (LISAC), Licensed Associate Substance Abuse Counselor (LASAC), or Licensed Substance Abuse Technician (LSAT). LISAC preferred.
CERTIFICATION: Current CPR certification.
EDUCATION: Master's Degree in Social Work, Counseling or related field
PREFERRED EXPERIENCE:
-Minimum of two (2) years addictions treatment counseling preferred
-Minimum of one (1) year correctional experience preferred
