This position is an every weekend position and candidates may decide between either a full time (36 or 40h) employee. The weekend shifts are each 12-hour shifts.
This innovative opportunity is for a Nurse Practitioner (NP) to collaborate with attending hospitalists on the Hospital Medicine Unit at the Massachusetts General Hospital. This opportunity includes direct patient care from admission to discharge and a cross-coverage model for general inpatient medicine patients and subspecialty services.
Each NP will follow 6 patients on average, with one admission per day. The NP promotes a collaborative relationship and effort between the hospitalists, private physicians, nurses, and the remainder of the interdisciplinary health care team.
Applicants for this position should be patient-centered, motivated, and energetic clinicians that want to be part of a large academic team with MD/DO colleagues and NP/PA peers. Under general supervision and direction of the Cardiac Surgeons and Heart Center Intensivists, and in accordance with the rules and regulations of Massachusetts governing Nurse Practitioner or Physician Assistant practice, the Advanced Practice Provider (APP) will be responsible for the care of patients on the Cardiac Surgical Service, primarily those patients that are admitted to the Cardiac Surgery/Heart Center Intensive Care Unit.
* The APP will participate in daily rounds with nurses, other Advanced Practice staff, Intensivists, residents, and multidisciplinary caregivers; perform bedside diagnostic and therapeutic procedures, participate in the development and implementation of care plans, coordinate tests and consultations, implement treatment plans, communicate with referring physicians, perform patient education, and prepare patients for discharge, all while keeping Surgeons and Intensivists updated as required.
* The APP maintains open lines of communication between Cardiac Surgeons, Residents, Fellows, PAs, Nursing Staff, Cardiologists, Heart Failure team, Intensivists, and patients/families. Promotes a collaborative relationship and effort between the medical staff and professional nurses toward continuity of patient care and efficient use of resources. Serves as a role model for professional APP practice.

Full time, rotating shift
This innovative opportunity is for a Nurse Practitioner (NP) to collaborate with attending hospitalists on the Hospital Medicine Unit at the Massachusetts General Hospital. This opportunity includes direct patient care from admission to discharge and a cross-coverage model for general inpatient medicine patients and subspecialty services.
Each NP will follow 6 patients on average, with one admission per day. The NP promotes a collaborative relationship and effort between the hospitalists, private physicians, nurses, and the remainder of the interdisciplinary health care team.
General medical inpatient or ICU experience is preferred with a minimum of at least 2 year of NP experience required; however, applicants with other NP experience may be considered. Candidates with direct patient care experience as a NP on an inpatient medicine floor or in the ICU are strongly preferred. Current Massachusetts State Licensure in good standing, with all associated active certifications, as a Nurse Practitioner, is a requirement. MCSR and DEA licenses Qualified in all basic (BLS) and advanced cardiac life support (ACLS) standards as established by the American Heart Association.

The MGH Chelsea Healthcare Center Adult Medicine Practice, a large academic internal medicine practice located at MGH Chelsea servicing a very diverse population in the Chelsea Community.

Primary Responsibilities:
Clinical Practice: Participates in daily huddles and shares information with the team that is vital to enhance the patient office experience, delivery and coordination of care and incorporates the patient as an active team member.
* Provides independent, direct, and indirect nursing care, performing physical examinations, ordering diagnostic studies, evaluating results and prescribing treatment as appropriate.
* Follows patients appropriately, within Advanced Practice Role as delineated within the state of Massachusetts and MGH.
* Establish the diagnosis(es) and resolves a plan of care including order laboratory and radiology test and treatments for:
o Short term and chronic health problems
o Exacerbation of health problems
* Chronic disease and health promotion management
o Discuss with patients or their delegates: end of life issues, chronic disease management, and educational materials regarding patient's health.
o Refer patients to appropriate physicians, clinics, or other health care providers, including community providers for management or consultation.
o Write prescriptions according to Massachusetts Authority to Prescribe laws and consistent with the NP's and supervising physician's scope of practice.

4. Plan of care reflects discharge planning utilizing appropriate resources and referrals including community resources. As ordered by the physician, discharge care includes blood tests, skin tests, pulmonary function test and reversal, EKG, immunotherapy and/or X-rays.

The UM Outpatient Clinical Review Nurse contributes to the overall success of the Utilization Management department by performing clinical reviews for outpatient, including determining the member's overall health, reviewing the type of care being delivered, and evaluating medical necessity, in compliance with governing regulations, internal policies, and procedures.
Responsibilities
Performs pre-service, concurrent, and retrospective reviews of members for appropriate care and setting to determine overall health and appropriate level of care, using applicable coverage documents and clinical guidelines
Collects, documents, and maintains review findings and actions taken on member medical records in health management systems, according to utilization management policies and guidelines
Works with healthcare providers to approve medical determinations or provide recommendations based on requested service and review findings
Collaborates with Medical Directors, when determination to deny a request is indicated
Assists with providing education to providers on utilization processes to ensure high quality, appropriate care to members
Collaborates with care management on referral of members, as appropriate
Performs clinical reviews for member and provider appeals related to outpatient services
Participates in the weekend call program, as well as flexible work hours, as needed within the department
Provides input to the Medical Policy Committee regarding the development/updating of medical policies
Education/Experience:
Current MA RN License required
Bachelor's degree preferred and 3-5 years nursing experience
Knowledge of utilization management, Medicare and Medicaid strongly preferred
Highly developed critical thinking skills- prior nursing experience ICU or ER
Computer literacy and ability to navigate through computer systems
