$20.38 - $23.43 - $27.53Summary: Performs venipuncture and capillary punctures in an Inpatient setting. Proficient in phlebotomy, specimen requirements and handling, as well as skills in computer functions necessary for patient identification and management of test orders. Works with supervision. In all communications, treats all contacts with respect and collaboration, consistent with Department of Pathology Cultural Compass and Service Promise. Job Responsibilities: 1) Safety. Demonstrates skills/knowledge to: appropriately dispose of bio-hazardous waste; evaluate and handle difficult patient situations (combative, uncooperative, fainting, seizures, excessive bleeding, other patient complications, etc.); use various barrier precaution procedures; appropriately use equipment; and tasks related to safety record keeping. Essential staff in Plan D2) Technical. Demonstrates skills/knowledge to: accurately identify patients and label specimens; provide age related care; select appropriate site, assessing vein size, location and position of veins for vascular access and performance of venipuncture; basic specimen collection, processing and handling; capillary punctures; calculate minimum specimen requirements; procedures for combining orders, redraws, line draw in order to provide optimal care; specimen prioritization/handling procedures and regulations/procedures for transporting in a manner appropriate to maintain specimen integrity by utilizing the pneumatic tube and /or by walking specimens to the lab; equipment maintenance. Proficient in troubleshooting and problem solving.3) Work Relationship. Demonstrates: excellent communication, teamwork and customer service skills to work independently and in team environment; excellent client relationships on nursing units; appropriate interaction with nurses and physicians in making specimen collection decisions. 4) Information Technology. Displays skills/knowledge in use of basic computer functions/applications, Ex: password, logoff, e-mail; test ordering, standing and future order applications. Appropriately uses, maintains and safeguards beepers and CLM, cellphones and other electronic devices (as applicable).5) Clerical. Demonstrates ability to secure signatures and complete forms required to meet compliance regulations; complete tasks related to record keeping; print and deliver reports; order and maintain inventory; perform other clerical duties including copying, faxing, stocking and maintaining supplies.6) Additional duties. Proficient in Sunquest ordering/cancelling and CLM, to include consolidation of orders to avoid additional needle sticks. Proficient at identifying lines and other conditions that make arm unsuitable for venipuncture. Works in adult psych unit/learns to evaluate. CLERICAL DUTIES:
-Filing appropriately in a timely manner.
-Answering phone requests from floors, Windemere, or Physician offices.
-Faxing appropriately to outside agencies and offices.
-Requests tests accurately and completely through Epic.
-Logging in outside laboratory request and result.
-Delivering results to appropriate areas.
-Performs duties as requested by technical staff.
-Insuring requests contain proper information to be submitted to Accounts Receivable.
-Maintains Standing Order List.
-Maintains supplies and documents for Drug Testing Accounts.
-PHLEBOTOMY DUTIES:
-Recognizing appropriate collection tubes for requested testing.
-Good Venipuncture technique.
-Asking for help when venipuncture is missed.
-Good capillary collection technique, i.e., fingerstick, heelstick.
-Good pediatric venipuncture technique.
-Recognition of appropriate amounts to be drawn for requested testing, including microsampling.
-Looks up “send out” test requirements and processes the specimens appropriately.
-Communicating to testing personnel “URGENT PRIORITIES”, i.e., STAT, ASAP, or call results.
-Proper administration of Glucola, Xylose for tolerance/challenge testing.
-Re-stocking phlebotomy room and participating in inventory control.
-Proper collection of urine for outpatient drug screens (NIDA STD's).
-Delivers specimens to testing area in a timely manner.
-Demonstrates ability to serve patients in various age groups, pediatrics to geriatrics.
-TECHNICAL DUTIES:
-Performs Simplate Bleeding Time tests.
-Performs all duties to support the testing personnel as allowed by CLIA 88.
-Trains and guides new staff members.
-Performs capillary INR testing.
-REGISTRATION DUTIES:
-Ensures patient confidentiality prior to interviewing patient.
-Obtains all relevant information and enters into Epic.
-Ensures that diagnoses appear on laboratory requisition.
-Compiles and quantitates total lab outpatient volume at the end of each day.
-Performs insurance verification for all appropriate patients.
-LEAD DUTIES:
-Plans activities with the Laboratory Manager in direct support of patient care requirements.
-Provides support to other phlebotomy technicians in the department.
-Troubleshoots and assists the Laboratory Manager in resolving equipment problems and failures.
Qualifications
Education Requirements:
Minimum: High School diploma
Preferred: Successful completion of an accredited phlebotomy program.
Preferred: CNA, LPN, RN and/or appropriate healthcare certification that requires formal schooling.

Insure that all specimens are appropriately and accurately labeled. Process and deliver specimens to correct laboratory departments. Job Summary
Under general supervision of the Supervisor of Clinical Support Services, the individual collects specimens for laboratory analysis including blood, urine and nasal swabs. Explains safety protocols and procedures to patients, is customer focused and provides certain clerical functions.
Collect blood using good judgment, reasonable and prudent care and according to policies and procedures, including but not limited to the following: -Check two patient identifiers and insure a complete match between requisitions, labels and patient identification card.
-Use hand hygiene procedures according to unit guidelines
-Select appropriate specimen collection equipment
-Select appropriate site for blood collection.
-Tie tourniquet and prepare site for venipuncture
-Collect specimen according to order of draw
-Ensure adequate mixing of specimen in tube by inversion or mechanical device.
-Label in presence of patient
-Package and transport specimens
-Monitor patient and ensure bleeding has stopped, and patient is stable.
-Review prior to collection a standard checklist which includes checking for potential adverse reactions such as history of fainting, and latex allergy.
-Is customer focused and puts the needs of patients first
-Maintains patient privacy, and follows all HIPAA guidelines
-With training, or if qualified by competency review, provide EKG, clerical and specimen transport functions.
-With training, or if qualified by competency review, provide pediatric and ancillary services to phlebotomy operations serviced by Laboratory Support Services.
-Provide reception and associated clerical functions at Specimen Collection center front desk including Fax receipt and send, filing of orders, answering telephones and placing requisitions in queue according to policies and procedures.
-Maintain adequate supply inventory, and complete monthly inventory checks as assigned.
-Satisfy annual required continuing education for phlebotomists and mandatory life safety, HIPAA and other required training.
-Work in collaboration with and practice good team working and communication skills with co-workers.

Insure that all specimens are appropriately and accurately labeled. Process and deliver specimens to correct laboratory departments. Enter all specimen and patient information in LIS or department logs. Generate accurate reports in a timely fashion and distribute appropriately. Perform phlebotomy on inpatient, outreach clients, home draws, nursing homes, and outreach locations. Maintain equipment and instrumentation. Qualifications
Equivalent to successful completion of a college-based, nationally accredited, phlebotomy program or a Medical Laboratory Assistant program.
Certification from the American Society for Clinical Pathology Board of Certification (ASCP BOC or equivalent) is required within two years from date of hire.

Job Summary
Under the general supervision of the Supervisor of Clinical Support Services, the individual collects specimens for laboratory analysis, including blood, urine, and nasal swabs. Explains safety protocols and procedures to patients, is customer-focused, and provides certain clerical functions.
Collect blood using good judgment, reasonable and prudent care and according to policies and procedures, including but not limited to the following: -Check two patient identifiers and insure a complete match between requisitions, labels and patient identification card.
-Use hand hygiene procedures according to unit guidelines
-Select appropriate specimen collection equipment
-Select appropriate site for blood collection.
-Tie tourniquet and prepare site for venipuncture
-Collect specimen according to order of draw
-Ensure adequate mixing of specimen in tube by inversion or mechanical device.
-Label in presence of patient
-Package and transport specimens
-Monitor patient and ensure bleeding has stopped, and patient is stable.
-Review prior to collection a standard checklist which includes checking for potential adverse reactions such as history of fainting, and latex allergy.
-Is customer focused and puts the needs of patients first
-Maintains patient privacy, and follows all HIPAA guidelines
-With training, or if qualified by competency review, provide EKG, clerical and specimen transport functions.
-With training, or if qualified by competency review, provide pediatric and ancillary services to phlebotomy operations serviced by Laboratory Support Services.
-Provide reception and associated clerical functions at Specimen Collection center front desk including Fax receipt and send, filing of orders, answering telephones and placing requisitions in queue according to policies and procedures.
-Maintain adequate supply inventory, and complete monthly inventory checks as assigned.
-Satisfy annual required continuing education for phlebotomists and mandatory life safety, HIPAA and other required training.

Qualifications
The Phlebotomist III is responsible for providing phlebotomy services in the lab to North End Waterfront Health (NEWH) patients as ordered by treating providers. The individual draws blood via venipuncture or skin puncture on infant, children, teens, adults and elders. Obtains urine samples and collects various samples for culture. Explains safety protocols and procedures to patients. Is responsible for daily quality measures of Point of Care testing and annual POCT competency of colleagues with final oversight by Nurse Manager.
Performs venipuncture and skin puncture on infant, children, teen, adult and elder patients.
Maintains, and updates knowledge of phlebotomy and specimen collection requirements including requisitions, handling and disposal.
Annually participates in hospital/NEWH required safety education, patient privacy education and skill competency verification.
Provides excellent customer service to patients and co-workers.
Checks patient identification against requisition prior to, and at the end of all specimen collection procedures by checking that all information on requisition (s), patient identification tools, and all labels are an exact match.
Locates vein and draws requested bloods following standard phlebotomy techniques; determines need for special equipment for problem veins.
Instructs patients in correct procedure for obtaining urine samples and provides patient with proper collection materials
Ensures adherence to schedules for timed tests such as glucose tolerance and drug levels, and special tests such as STATS, blood culture, and tests required to be obtained in a fasting state including POCT tests
Identifies a wide variety of tests; selects appropriate tubes for tests, according to the size and metric volume; estimates quantity of blood needed in relation to tests ordered; assembles all necessary equipment; obtains tubes according to the established correct order of draw; labels tubes with patient name, unit number, and date initials requisition; prepares samples for storage/transportation.
Performs guaiac on stool cards returned to practice in mail, sends results to patient and ordering provider.
Ensures specimens are ready for transport according to daily schedule.
Logs and records transport activity accurately.
Follows all safety (including infection control and hazardous material) policies and procedures.
Notifies Nurse Manager of unsafe conditions.
Delivers patient reports and assists in printing/collating of patient reports as needed.
Arranges STAT specimen transport to MGH or Quest Diagnostics when needed.
Knows how to respond to emergency in lab such as syncope with venipuncture.
Assists nurse manager with annual waived testing to other Phlebotomists, MAs, LPNs and RNs
Responsible for laboratory orientation and training of all new Phlebotomists, MAs, LPNs and RNs

* Selects appropriate venous puncture site based on knowledge of pathophysiology. Performs phlebotomy for collection of blood for clinical and research testing.
* Monitors individuals to ensure comfort, safety and adequate blood flow. Monitors individuals for adverse reactions.
* Selects and prepares collection tubes/containers, paperwork, labels and forms. Has knowledge of testing requirements for clinical tests including special handling requirements.
* Monitors and stock supplies. Notifies appropriate staff when re order levels are reached.
* Provides positive customer service that supports departmental and hospital operations. Performs and records quality control measures to insure accurate equipment performance, making and documenting necessary adjustments. Participates in continuous improvement efforts.
* Contributes to the teamwork within and between departments and organizations.
Minimum Qualifications:
* High School / GED required
Experience:
* At least 6 months Phlebotomy experience preferred
Licensure:
* Current certification as a phlebotomist by a national certifying agency (e.g. ASCP) is preferred
Shift/hours:
* Monday-Friday: 7:00am-4:30pm, including rotating weekends.

The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.
