Summary:
Under the management Cardiac Interventional Services Manager, the Registered Cardiovascular Invasive Specialist (RCIS)/Registered Cardiac Electrophysiology Specialist is a part of a patient care team that performs various cardiac, neuro and peripheral vascular diagnostic and interventional procedures.
The RCIS must possess expertise in the theory and application of cardiovascular/peripheral vascular disease process, procedures specific to their specialty, as well as aseptic technique. The RCIS/RCES will be able to anticipate the next steps of the procedure by constant attention and assessment of the process and patient condition to promote a safe, expedient, quality patient outcome and positive patient experience.
The RCIS/RCES will scrub and assist the physician at the procedural table. They will also monitor, identify and record pertinent EKG and hemodynamic data. They will submit charges and document chronological events of the procedure. They will set up, operate and may troubleshoot Equipment pertinent to their specialty. RCIS - Intra-Aortic Balloon Pump (IABP), Rotoblator, Impella, Fractional Flow Reserve (FFR), Intravascular Ultrasound (IVUS), CSI in addition to any new technologies. RCES - Mapping, Programming, Recording with systems common to the field of electrophysiology. They will act as a resource to the physician by understanding the potential application, risks and benefits of all available procedural supplies and equipment. They will anticipate, gather and deliver required items to the sterile field using aseptic technique. The RCIS /RCES may precept students and new employees.

Job Responsibilities:
1) Participates in team huddle/time-out and all aspects of procedure.
2) Acts as a first assist during cardiovascular and Neurovascular procedures.
3) Maintains RCIS/RCES, AHA certification in ACLS and Healthcare Provider BLS.
4) Operates and maintains all diagnostic and therapeutic equipment under the direction of the performing physician.
5) Prepares patient for procedure in prep/recovery area and procedure room.
6) Explains process and reinforces patient teachings throughout procedure.
7) Sets up non-invasive monitoring. (EKG ,BP, Pulse Oximetry, etc.)
8) Preps and drapes patient using appropriate aseptic and sterile technique.
9) Facilitate insertion and manipulation of guide-wires and catheters into vasculature under fluoroscopic guidance for cardiovascular and Neurovascular procedures.
10) Obtains invasive hemodynamic data, cardiac output and blood samples as required by procedure.
11) Injects contrast manually or with the aid of a mechanical contrast device to visualize cardiovascular anatomical structures(angiography).
12) Performs Point of Care testing.
13) Assist and performs emergency resuscitation.
14) Performs patient monitoring as well as complete and accurate documentation of all aspects of procedure. Captures and inputs data for procedural calculations.
15) Manages access site and hemostasis, manual sheath removal.
16) Attends staff meetings, in-service and other designated training events that will enhance clinical and other required skills.
17) Performs at least minimum of on call rotations as determined by staffing requirements and manager discretion to ensure competency is maintained. (RCIS)
18) Dysrhythmia recognition.
19) Performs call duties for Cardiac Cath Lab as required.

Job Summary
GENERAL SUMMARY/ OVERVIEW STATEMENT:
Under the direction of the Lab Manager and with the general supervision from the Senior Tech Specialist, the technologist will be required to perform standard and complex all night sleep recordings as well as multiple sleep latency tests.
Monitors additional physiologic phenomena by means of appropriate technique. The polysomnograms may measure oxygen saturation, EKG, EEG, respirations, ETC02 levels, upper airway resistance, CPAP and BI-PAP levels and other parameters necessary for a complete sleep study. After an initiation period of 4-6 weeks the tech would be required to work independently not necessarily alone for up to 12 hours. The technologist must be able to make appropriate decisions regarding the diagnosis and treatment of benign to the most complex patients. May perform clerical and other laboratory duties required.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Examines physicians' orders and protocols for each sleep study. Reviews existing information in the patient's record and recognizes chief sleep/wake complaints. - Greets and interviews the patient to assess current medication, recent caffeine/alcohol ingestion, current sleep patterns and the need for treatment intervention during the study. Communicates with the ordering physician or unit director to clarify unclear of inappropriate orders and communications information regarding the patients' status.
* Must spend appropriate time in servicing the patient regarding the possible use of CPAP/BI-level equipment. The patient's fears apprehensions concerning CPAP should e reduced or eliminated by the in-service.
* Sets up/calibrates monitoring equipment with appropriate montage. Measures and applies necessary EEG electrodes using the 10/20 system. Applies appropriate EKG, EMG, and EOG monitoring electrodes. Places and securely attaches thermistor to measure nasal/oral airflow, respiratory effort, oximetry, and ETC02. Administers supplemental O2 under physicians' order.
* Performs physiologic calibrations, verifies the integrity of the recording. Institutes "lights out" and notes patient data every 15 minutes on a tally sheet. Monitors the polysomnogram for clinical and physiological events. Must be able to interpret severely abnormal data/EKG rhythms and report such findings to the sleep physicians. Notes and corrects equipment malfunctions. Understands indications, contraindications and adverse effects of nasal continuous positive airway pressure (NVPAP, BI-level and oxygen administration), applies and titrates therapy based on physicians' orders and established protocols.
* Recognizes and responds appropriately to the patient's medical problems and physiological events, especially cardiac arrhythmias, respiratory events and seizures. Alters the patient's environment or changes the patient's body position as needed to obtain the required sleep/wake data. Evaluates routine or abnormal EEG when it applies.
* Carefully wakes the patient and records post-sleep calibrations. Removes the electrodes, and ancillary monitors and assists Standard Precautions Infection Control procedures. Properly disinfects and sterilizes the equipment, the electrodes and the thermistors and monitors used during the study. Prepare a detailed report of the data and intervention used to diagnose and treat the patient.
* Attends seminars and professional meetings to keep knowledge and skills updated and maintain CEC required for AASM accreditation. Attends department meetings whenever held.

Qualifications
SKILLS/ABILITIES/COMPETENCIES REQUIRED:
* Acquisition of high quality studies - Following pediatric protocols for diagnostic polysomnography, multiple sleep latency testing, CPAP/bilevel/ASV titration, and oxygen titration.
* Thorough documentation of nocturnal events
* Troubleshooting of patient and equipment related issues during the night - Developing skills related to scoring of nocturnal studies
* Mask fitting and troubleshooting for PAP therapy
* Assessment and management of challenges related to obtaining polysomnographic data in small and/or medically complex children
LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:
* RPSGT certification preferred.
* All new employees have one year from date of hire to become BRPT registered. Unregistered technologists are required to sit for the exam at the next possible test date and subsequent dates until registration is achieved.
* BCLS for Healthcare Provider certification is required.
EDUCATION:
* High School Diploma required.
* Graduation from a certified hospital sleep, EEG, or respiratory therapy program is preferred.
EXPRIENCE:
1-2 years polysomnography experience required, previous pediatric experience is preferable.
WORKING CONDITIONS: Must be able to work flexible hours including some weekend shifts, with primary emphasis on night shift.

An Interventional Radiology (IR) Technologist specializes in assisting with minimally invasive procedures that utilize imaging techniques such as fluoroscopy, CT scans, and ultrasound to guide physicians in the diagnosis and treatment of various medical conditions. IR Technologists work closely with interventional radiologists and other medical professionals to perform procedures that can treat diseases, blockages, and injuries without the need for traditional surgery. They are responsible for preparing patients, operating imaging equipment, and ensuring a sterile and safe environment during procedures.
Key Responsibilities:
Pre-Procedure Preparation:
Prepare the interventional radiology (IR) suite by ensuring all necessary imaging equipment, sterile instruments, and supplies are available and functioning.
Review patient information, medical history, and previous imaging studies to ensure the correct procedure and appropriate imaging protocols.
Verify patient identity, allergies, and consent forms before the procedure begins.
Explain the procedure to the patient, address any concerns, and provide reassurance to reduce anxiety.
Assisting During Procedures:
Assist the interventional radiologist in performing a wide range of minimally invasive procedures, including angiograms, biopsies, catheter placements, stent insertions, and drain placements.
Operate imaging equipment (e.g., fluoroscopy, CT scan, or ultrasound) to guide the physician during procedures, ensuring accurate imaging to target specific areas of the body.
Maintain a sterile environment by assisting with sterile draping, setting up sterile instruments, and handling devices as required during the procedure.
Administer contrast agents or dyes, as directed by the physician, to enhance visibility of blood vessels or organs on imaging.
Monitor the patient's vital signs, including heart rate, blood pressure, and oxygen levels, during the procedure and report any changes to the medical team.
Patient Safety and Comfort:
Ensure patient comfort and safety throughout the procedure, positioning the patient appropriately to obtain optimal images while preventing injury.
Take precautions to minimize radiation exposure to both the patient and the healthcare team by following radiation safety protocols, using shielding, and optimizing imaging settings.
Monitor patients for signs of complications, such as bleeding, infection, or adverse reactions to contrast agents, and report findings to the physician.
Post-Procedure Care:
Monitor patients post-procedure for any immediate complications, including bleeding, infection, or changes in vital signs.
Assist in the transfer of patients to recovery or another unit after the procedure, ensuring they are comfortable and stable during transport.
Provide post-procedure care instructions to patients, including guidance on activity restrictions, wound care, and follow-up appointments.

Troubleshoot any equipment malfunctions during procedures to ensure minimal disruption to patient care.
Keep accurate records of equipment use, maintenance, and any issues that arise with the imaging equipment.
Collaboration with Medical Team:
Work closely with interventional radiologists, nurses, anesthesiologists, and other healthcare professionals to ensure the success of each procedure and high-quality patient care.

Participate in discussions before and after procedures to review patient conditions, procedural outcomes, and follow-up care.
Documentation and Record-Keeping:
Accurately document all patient information, procedures performed, imaging parameters, and medications administered in the electronic health record (EHR).
Ensure that all records are completed in compliance with hospital or clinic policies and regulatory standards.
Maintain detailed logs of imaging procedures, contrast agents used, and patient conditions during and after the procedure.
Radiation Safety and Compliance:
Follow strict radiation safety protocols to ensure that radiation exposure is minimized for both the patient and the medical staff, including wearing protective equipment such as lead aprons.
Adhere to healthcare regulations and standards, including HIPAA, infection control measures, and hospital accreditation guidelines.
Stay updated on the latest radiation safety practices, imaging techniques, and procedural advancements.
Professional Development and Training:

Job Summary
Under the direction of the Practice Manager, the Urology Technician II (UT- II) is responsible for assisting the physicians, advanced practice providers, and nurses with management and treatment of patients during office visits and other tasks related to the visit consistent with his/her training and skills. The UT- II works with the care team in providing a wide variety of clinical and technical support functions in the delivery of care within the practice. His/her primary role is to assist with outpatient procedures, assure efficient patient flow through the clinic, to maintain a clean, organized, and safe patient care environment and to promote patient satisfaction by interacting with patients in a professional, respectful, and friendly manner. In addition, the UT - II is expected to actively collaborate with the Practice Manager to ensure effective UT efforts across the clinical practice, and to function as a mentor to the UT - I.
General Responsibilities:
Ensures efficient patient flow through the clinic:
* Greets patient and family in waiting room and escorts to exam room.
* Identify patient using 2 identifiers (name & DOB).
* Alerts patients to possible delays in visit timing.
* Obtains and documents accurate vital signs, height, weight, pain level and allergies.
* Ensure that pre-visit paperwork has been completed.
* Prior to or during the patient appointment, performs appropriate ancillary services (urinalysis) unless otherwise directed by the physician.
* Obtains (verify using 2 identifiers), processes, and documents results for urine specimens using the Clinitek device.
* Under direction of physician sends urine specimens for culture & sensitivity or cytology (verify using 2 identifiers).
* Anticipates needs of physician during the appointment, setting up special equipment, supplies and paperwork as needed for visit.
* Alerts provider when the patient is ready to be seen and communicates all pertinent clinical information.
* Assist patients with check-out process.
* Cleans and prepares exam room for next patient
* Responds to the need to expedite processes, trying to maintain schedules and timely patient flow.
* As appropriate, assists physicians with patient care during patient visits.
* Adheres to MGH hand hygiene and standard precaution guidelines.
* Provides care appropriate to age group of patients in the practice setting and documents data per practice protocols.
* Supports a variety of treatments and procedures according to Department/MGB standard policies and procedures.
* Correctly and appropriately uses Ambulatory Patient Tracking System to enhance patient flow via timely and effective communication of patient movements in practice.
Maintains neat, clean, and safe exam rooms and treatment areas:
* Ensures:
* Patient care areas are clean, organized, and ready for patients.
* Exam and treatment rooms are properly stocked, equipped, and secured as applicable.
* All equipment is cleaned after use per office protocol and is in good working order with a current biomedical inspection sticker.
* Supplies are put away in a timely manner.
Participates in daily operational activities necessary for a safe patient care environment:
* Checks unit emergency equipment for lock integrity and biomedical safety checks.
* Checks all refrigerator temperatures and maintains records in the Safety Manual.
* Performs all quality checks for the POCT.
* Performs all quality checks for the Cidex OPA and the Steris system.
* Assumes responsibility for the maintenance of neat, organized and well stocked exam rooms.
* Understands and monitors appropriate cleaning of all equipment.
* Maintain patient confidentiality.
* Maintains MGB-JCAHO standards on the unit.
* Effectively communicates any care-patient issues with peers, nursing staff, Practice Manager, and Operations Manager.
* Effectively supports and functions as a member of the care team.
* Promotes patient/customer satisfaction by interacting with patients in a professional, respectful, friendly manner consistent with their needs.
* Adheres to Human Resource policies with respect to attendance and dress code.
* Adheres to MGH customer service philosophy in all patient interactions. - Interacts in a professional, supportive, and collaborative fashion with other members of the care team.
* Accepts delegated responsibilities in a positive manner.
* Offers to help others in the practice when able.
* Before leaving the practice for breaks/lunch/meetings, ensures that adequate coverage is available.
* Engages in constructive problem solving with others in the practice as problems arise.
* Participates in MA in-services as scheduled.
* Attends and participates in staff meetings.
* Completes all annual required training.
Specific Urology Duties:
* Principally focus on support of the procedural areas of the Urology ambulatory practice.
* With the assistance of a peers, manage up to 50 procedural patients daily.
* Develop and maintain an inventory of procedure-specific equipment and supplies to par with inventory status and location communicated to staff, care team, etc.
* Monitor procedure schedules 48 and 24 hours prior to procedure dates to ensure proper scheduling of exams. Collaborate with procedure team to resolve any mis-scheduled exams.
* Prepare procedure rooms with the necessary supplies, equipment, etc. for each unique procedure.
* Maintains systems of care and preventative maintenance of all equipment.
* Able to set up Urology procedure suites for varied procedures, including but not limited to cystoscopies, bladder and prostate biopsies, UDS, vasectomies and laser procedures.
* Contacts vendor with equipment issues and communicates to staff and care teams in such scenarios. If needed, arranges for alternative testing times and locations for patient care impacted by equipment outages.
* Maintains eye wash station and corresponding log sheet.
* Prepares Urology procedure suites consistent with procedures scheduled to be performed (e.g. surgical clamps for vasectomy; ultrasound probe for biopsy; flexible cystoscope for cystoscopies).
* Prepares for and assists in Urodynamic (UDS) testing procedures, including cystometrogram and uroflowmetry. Proficiently and safely operates UDS equipment.
* Efficiently prepares procedure rooms for emergency and unscheduled procedures.
* Within procedural support, observes all universal protocols and precautions.
* Orders procedural and other supplies for Urology practice.
* Understands the reprocessing of all utilized equipment. Cleans urological instruments using proper precautions, sterile technique, and appropriate cleaning agents.
* Maintains equipment with support of MGH Bio-Med, vendors & manufacturers · Trouble-shoot equipment maintenance problems and promptly report major issues to the vendor and/or Operations Manager.
* Prepares (undress, prep, drape) and properly positions patients for Urological procedures.
* Assist Urologists in procedure rooms throughout Urology procedures.
* Reinforce instructions to patients regarding appropriate post-procedure care and limitations following ambulatory Urological procedures.
* Assist patients in completing various research-related questionnaires and health surveys.
* Reviews providers' daily and weekly procedural schedules to plan for staff, space & supply requirements and availabilities.
* Collaborates with team & other disciplines to formulate solutions for potential & real problems.

Summary:
Daily activities are coordinated by the RN. Performs a variety of technical tasks and bedside functions. Responsible for maintaining the lab area and all associated competencies for POC testing, phlebotomy and upholding regulatory requirements to maintain POC testing ability. Possesses knowledge in upholding department goals of patient care by maximizing throughput and expediting care in this emergency department setting.

Job Responsibilities:
1) Provides age-appropriate care to patients.
Demonstrates the knowledge of developmental stages for a variety of age groups and applies developmental theories and concepts when implementing care for patients across the lifespan.
2) Monitors basic physiological parameters.
Takes and records routine vital signs within the specified timeframe as indicated by periodic review of documentation. Obtains and documents weights/heights, abdominal girths, head circumference (Pediatric Service) as per physician/nurse order 100% of the time as indicated by monthly review of patient records. Consistently reports any significant deviations in vital signs as outlined in the procedure immediately. Records and reports, 100% of the time, any change in patient's normal drainage pattern, i.e. gastric, wound, urine, blood. Documents findings in the patients EMR.
3) Administers treatments and procedures in accordance with BMC policy/procedures.
Changes dressings within one hour of specified timeframe in accordance with established procedure. Provides postmortem care as directed by RN according to BMC policy. Performs all procedures outlined in nursing procedure manual within area of responsibility in accordance with specifications. Assists physicians with exams and simple procedures. Assures identity of patient by checking identification bracelet and identification band prior to performing any procedure consistently utilizing PPID lab collection manager system. Follows established process and protocols for phlebotomy.
4) Assists patients with elimination of body wastes.
Makes available or offers bedpan, urinal and emesis basin immediately after patient request as indicated by absence of patient complaints. Accurately labels and sends specimens of urine, feces, wound cultures and sputum with 30 minutes of specified time frame unless discussed with RN as evidenced by random review of reports. Accurately measures and records intake and output in accordance with established policy as indicated by random review of intake and output forms and documents in the patients EMR.
5) Performs POC testing to include department specified testing, ISAT, Urisys, Glucose testing, Pregnancy testing and all QA associated with the POC testing function. Performs proficiency tests and participates in unit inspections to maintain laboratory regulatory status.
6) Assists with treatment by following specified orders as delegated by the RN or provider treatment plans. Monitors patient's condition during treatment and notifies appropriate personnel of unexpected occurrences. Provides information to RN based upon observation made during patient related care. Provides treatment within the specified time frame. Documents treatments and activities consistently.
7) Communicates effectively with team and patients.
Interacts with patients in a respectful and informative manner. Clearly communicates information related to patient care to the team. Recognizes culturally based patient and family needs. Keeps the patient's experience a priority.
8) Is knowledgeable of and initiates emergency procedures.
Responds effectively in emergency situations as observed by team members. Maintains Patient safety at all times. Communicates effectively with appropriate personnel.
9) Functions as a member of the team
Assists team members with clinical activities. Provides feedback related to the team process. Assists team members with non-clinical activities.
10) Maintains knowledge on current throughput and patient progress initiatives to maximize patient flow in the Emergency department. Actively engages in patient flow initiatives.
