About the Role

Position Summary:
As a Discharge Planner, you will play a vital role in bridging the gap between hospital care and post-discharge needs. You will work directly with patients, families, and the interdisciplinary team to ensure a smooth transition to outpatient services or other support systems.
Key Responsibilities:
Provide pre- and post-discharge information, assistance, and referrals to patients and families.
Collaborate with the interdisciplinary team to coordinate discharge activities.
Facilitate adult, child, and adolescent group therapy sessions.
Assess patients' needs, including housing, transportation, and continuing care, and make appropriate referrals.
Notify outpatient providers of hospitalization with patient consent.
Maintain detailed and timely documentation of discharge plans and related processes.
Develop and evaluate educational materials for patients and families.
Participate in clinical supervision sessions and team meetings.
Support Utilization Review efforts to optimize patient care and insurance benefits.
Qualifications and Skills:
Bachelors degree in related field required
Strong written and verbal communication skills with the ability to adapt to diverse audiences.
Knowledge of community resources and services for patients and families.
Ability to work collaboratively in a multidisciplinary team environment.
