POSITION SUMMARY: As a key member of the Northpoint team, the Therapist/Counselor will oversee the therapeutic services provided to a caseload of patients. This individual will act as the treatment plan coordinator for the multi-disciplinary team in the development and implementation of the master treatment plan and the discharge plan for each patient. The Therapist/Counselor will provide individual, group and family therapy for assigned patients, as required by contractual and licensure obligations. This person will actively communicate with support staff, treatment teams, and outside agencies on treatment and safety issues pertaining to patients and their family members. The Therapist/Counselor will complete all documentation associated with these duties in a professional and timely manner. ESSENTIAL RESPONSIBILITIES AND DUTIES: Provide counseling, assessments and group facilitation to individuals and families Coordinate all aspects of direct clinical care from admission to discharge Consult with the multidisciplinary treatment team to provide quality patient care Provide orientation and education to patients, families and others to aid in the therapeutic process Collaborate with assigned patients in the development of written treatment plans; include patient and family involvement in assessment, treatment, education and discharge planning Monitor patients on an ongoing basis to continually assess needs and facilitate treatment and interventions Document clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality patient care Actively participate as a member of the multidisciplinary treatment team and maintain effective working relationships with employees and peers Maintain professional boundaries with patients at all times Oversee and participate in the quality assurance and compliance activities of the facility Provide clinical updates to utilization review and representatives of managed care to best coordinate reimbursement of treatment services Coordinate with community based providers and referral sources to ensure optimal levels of care and treatment outcomes, post discharge Case staffing and individual case supervision Facilitate individual and group therapy Adhere to all company policies and procedures Maintain confidentiality in accordance with established policies and procedures and standards of care Other clinically appropriate services and special projects as assigned QUALIFICATIONS/REQUIREMENTS: Master's Degree of Mental Health, Social Sciences or commensurate experience required Must hold an active substance abuse or mental health licensure in the state where treatment services are rendered Must maintain applicable state licensure requirements at hire, and for the entire duration of employment Ability to work with a diverse population and possess strong interpersonal skills and knowledge required for treating patients assigned Must hold strong personal boundaries and able to build rapport with patients Must understand and adhere to the ethical standards of the respective licensure governing board PREFERRED KNOWLEDGE AND SKILLS: Excellent organizational abilities Excellent written and oral communication skills Attention to detail and accuracy Patient service oriented (both internal and external) Creative and persistent problem solver Able to handle confidential material in a reliable manner Ability to interact and communicate with individuals at all levels of organization Strong interpersonal skills to handle sensitive situations and confidential information Ability to multi-task and prioritize workload in a fast-paced environment Proficiency with Microsoft Office Suite and EMR systems FULL-TIME BENEFITS INCLUDE: Subsidized Health Insurance Coverage for Employee, Spouse, & Dependent(s) 100% Employer Paid Basic Life Insurance equal to 1x annual salary, up to $100,000 100% Employer Paid Employee Assistance Program Voluntary Dental, Vision, Short-Term Disability, Supplemental Life & AD&D, Critical Illness, Accident, and Hospital Indemnity Insurance.
