They implement individualized therapy programs designed to restore, reinforce, and enhance performance. In addition, the Physical Therapist instructs, educates and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. This position provides a flexible schedule and work environment in various settings.
1. As necessary or applicable, supervision of Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.2. Responsible for adherence to department and patient treatment schedule, timely clinical and billing documentation requirements as well as work related meetings.3. Attends and contributes to patient care, staffing conferences and other related meetings.4. Makes referrals to other disciplines and agencies in order to address the comprehensive needs of the patient.5. Job Description
Mental Health Technicians are responsible for the following:
Assists the RN in delivering care, observing patient behaviors, and more.
Reports/documents any changes in patients mood or behavior in a timely fashion.
Provides one-on-one sessions with assigned patients as needed.
Mental Health Technicians will maintain a full patient caseload with an assigned primary nurse in an inpatient setting, and provide care for all patients effectively and compassionately.
Qualifications for this position include:
High School Diploma required
At Least 12 college credits, and full college degree preferred
BS or BA in behavioral health field preferred
One-year experience with behavioral health clients preferred
BLS through AHA required
Must be trained in PEAT and able to safely schedule and restrain an aggressively acting out patient

About Us
The Philadelphia Model: Addressing Serious Mental Illness (SMI) in Long Term Care (LTC) provides highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care and have serious mental illness. These individuals may have reached maximum benefit from other community or hospital-based mental health resources.
Our program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI and those with complex behavioral health needs and or prior criminal history can receive the care and services needed to address their holistic needs free of stigma.

Position Summary
We are hiring Behavioral Health Technicians (BHTs) to assist with direct care, behavioral support, and therapeutic engagement for our residents. This position is ideal for individuals with a passion for behavioral health, long term care, and teamwork.
Important: All new hires must be able to attend a mandatory 5-day orientation (Monday through Friday, 8:30 AM to 5:00 PM) before starting their schedule. Attendance is required for employment.
Key Responsibilities
Assist residents with personal care, hygiene, meals, and mobility
Participate in therapeutic and recreational group activities
Support residents in daily routines and treatment plans
Monitor behavior and provide accurate documentation in the Electronic Health Record (EHR)
Utilize de-escalation techniques during behavioral episodes
Accompany residents to medical appointments and interdisciplinary care meetings
Maintain safety and cleanliness in accordance with facility policies
Reinforce individualized goals using trauma-informed approaches
Qualifications
Bachelor's degree in human services OR High School Diploma or GED with at least 2 years of direct care experience
3 to 5 years of experience working with older adults preferred
12 or more college credits in human services strongly preferred
CPR certification required and must be from a hands-on course. Online-only certifications will not be accepted
Experience with psychiatric or addiction services preferred
Must meet requirements under the Older Adults Protective Services Act (Act 169)
Bilingual (English and Spanish) a plus

What will your day look like? You will:
Client safety is Epiphany's top priority, MHT's performs this by maintaining a safe therapeutic environment for clients at all times.
Maintain daily/weekly structure and schedule for clients as outlined in client schedule.
Observation of client medication that is considered a controlled substance
Ensure clients are filling out self-observation sheets for medication.
Completing rounds at the beginning, throughout and at the end of shift. When clients are placed on Q30 rounds, completing them accordingly with appropriate documentation
Overnight is responsible for hourly rounds unless otherwise specified for high-risk clients, all rounds documented in MHT chat
Transporting clients to and from appointments, clinical services, employment if applicable, and other outside activities. Ensuring Epiphany's driving policy is being followed and if an accident occurs, accident policy is followed
Performing Urinalysis as assigned, reading urinalysis results on a 12-panel, and reporting and recording appropriate documentation as well as communicating to appropriate supervisor.
Performing breathalyzers on clients as assigned. Reporting any results over 0.00 to on call supervisor
Reporting and completing shift notes for each shift
Following chain of command at all times. Being aware of when and how to report any, and all issues on shift, including but not limited to facilities, clients and other staff
Building positive relationships with clients and staff
Utilizing group chat when on shift for communication with other staff and supervisor
Flexibility - scheduling (coverage can be needed at any time). Client schedule changes, last-minute intakes, behavioral issues etc.
Searching client belongings upon intake, vehicles, and rooms as assigned by Supervisor
Reporting Suicidal Ideation and Homicidal Ideations of any and all patients
Completing client intakes, following all steps as outlined intake protocol
Crisis intervention included but not limited to client altercation (verbal or physical), relapse, behavioral issues
AMA blocking as needed
Facilitating “Empower Hour” multiple times a week with appropriate documentation
Client individuals weekly for all clients on the mental health track with documentation
Complete CSSRS as assigned for clients who report suicidal ideation
Facilitating client rules and responsibilities, and reporting non-compliance to supervisor
What you will need for this position:
Minimum of one (2) year in the mental health field (preferred).
Working knowledge of 12-Step Program, AA or NA (preferred).
High School Diploma or GED.
Mental Health Technician Certification (preferred).
CPR Certified (preferred).
ECPR: (preferred)
Standard First Aid Certification (preferred).
Valid Driver's License, with minimum 2 years CLEAN driving record; (required).
