Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals. Immediately report any crisis or serious concerns to appropriate staff. Utilize incident report to document any incidents when on duty following professional protocol. Escort clients to their scheduled groups and/or transport clients to their off-site appointments. Ensure clients are awake and report to nursing for medication rounds.
