Discharge Planner- Behavioral Health- PRN
Horizon Health is seeking a per diem discharge planner to work on our behavioral health unit at Great Plains Hospital in Elk City, OK. The Case Manager will be responsible for facilitating the continuum of care. Identifies discharge needs on admission. Engages in timely, efficient discharge planning. Works in close conjunction with the patient, family, physicians, unit staff and community resources to ensure safe discharge in the appropriate level of care with the necessary services. Arranges for after care and maintains follow up contact with patient, family or facility. As a member of the interdisciplinary team, works with the team to identify discharge needs and services, and is responsible to identify discharge needs in treatment planning. Works closely with physicians to secure prescription, physician orders and completion of physician documentation required for discharge. Develops and maintains patient discharge information for aftercare tracking. Develops and maintains relationships with community service agencies and residential facilities.
The hours will very between 8-20 hours per week based on census with an average of 16 hours per week. This will be Monday-Friday, no weekends.
Responsibilities:
Assesses and reassesses the needs of the patient and accurately documents assessments and discharge needs
Monitors on a daily basis the status of patients and communicates status to team members
Completes and coordinates discharge with the treatment team and physician within the designated time frame
Completes discharge planning forms and patient discharge forms by date of discharge
Documents on the patient chart the specifics of problems associated with implementing the discharge plan and actions being taken to solve problem(s) including the overall effectiveness of the action plan.
Communicates daily with physician to ensure complete documentation of primary/etiologic diagnoses, all appropriate comorbidities and complications
Reviews and discusses discharge plan with patient and/or family receiving input from them as well as providing explanation, information and support for acceptance and implementation of the discharge plan and obtains signature for involvement in treatment planning
Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities
Ensures that patient is discharged to a safe environment with necessary follow up care prior to discharge
Ensures after care medical and psychiatric appointments are made prior to discharge
Gathers follow up data on all patients at designated times
Utilizes feedback regarding performance and uses supervision to improve performance
Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships
Addresses issues in a timely manner including crisis and higher level of care referrals
Maintains appropriate ethical and clinical boundaries with client, family members and other staff members
Demonstrates knowledge of and adheres to LPS and Mental Health policies and procedures
Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon's confidentiality policies and procedures
Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as requested
Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data
Utilizes data to complete program's key statistical reports and generates reports as needed for the SVPO, VPCE, program director and client hospital leadership when requested
Assists in submission of reports and requests for documents from various regulatory agencies; re: PA Dept. of Health, CAP and Joint Commission
Keeps current on CEU requirements to maintain licensure status

Here at the Oklahoma Forensic Center the Nursing department is hiring for Mental Health Technicians that would provide direct care for consumers of behavioral health treatment in an inpatient setting. Responsibilities:

Job Summary: This person will provide support to students with disabilities who exhibit significant physical aggression due to the severity of their disability. In a one or one or small group setting, this person must be willing to work with students who may demonstrate physical aggression, engage in frequent outbursts, and who may be emotionally, physically and/or mentally impaired.
Essential Functions:
Assists the certified staff in devising special strategies for reinforcing learning materials and skills based on a sympathetic understanding of individual students, their needs, interests, and abilities.
Work with individual children or small groups of children to reinforce new skills initially introduced by the teacher.
Protect confidentiality of records and information gained as part of exercising professional duties and use discretion in sharing such information within legal confines.
Provide verbal support, prompting, redirection, and modeling to help the students access the curriculum and interact with certified staff members and learning activities.
Work cooperatively with other paraprofessionals, certified and licensed staff to facilitate positive interactions for the student with all staff and the ability to take instructions from a variety of school staff who are responsible for the student's education.
Adapts classroom activities, assignments and/or materials under the direction of the classroom teacher and the special services program coordinator for the purpose of supporting and reinforcing classroom objectives.
