In addition to acute and swing bed rehab, we offer a variety of outpatient services including sports and orthopedic rehab, pediatric rehab, neurological and stroke rehab, balance and dizziness rehab, wheelchair seating, and neck, back and extremity care. At SNLH, we have a large and well equipped outpatient therapy department located within a 25 bed critical access hospital. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon Physical Therapy license required. Healthcare Provider Level BLS required. Must be able to travel to multiple points of care. Conduct functional behavior assessments (FBAs) and develop and participate in the implementation of behavior support plans (BSPs) and crisis intervention plans (CIPs). Complete all required paperwork regarding the use of Nonviolent Crisis Intervention and restraint/seclusion procedures. Provide behavioral consultation to classroom staff and recommend, design, and implement alternative interventions. Communicate clearly and appropriately and make independent decisions within established policies and procedures. Train classroom staff in targeted intervention strategies and data collection procedures. Train, coordinate, provide modeling, coaching, observation, and feedback, and gather data regarding the job performance of Educational Assistant(s). Establish and maintain positive and cooperative relationships with ESD, local district, and community agency staff. Participate in professional growth activities. Comply with local district and ESD rules and policies. Perform physical requirements unaided or with the assistance of reasonable accommodation. Perform job-related tasks as designated by the supervisor. Minimum Prerequisites: TSPC licensure (Oregon Handicapped Learner, Severely Handicapped Learner, Speech/ Language Pathology, School Psychology endorsed license, or related field) or ODE authorization. Master's degree in special education, school psychology, counseling, or social work from an accredited college or university. Knowledge and experience with children with disabilities who have challenging behaviors and applicable intervention strategies.
