Under the direction of the Director of Case Management, the Discharge Planner is responsible for establishing an appropriate discharge plan for patient's in the acute care hospital setting.
Core Job Responsibilities
Responsible to perform psychosocial assessments to determine the patient's discharge planning needs
Participate in progression rounds to report anticipated discharge planning barriers
Act as an advocate for patients and families to ensure that they are aware of resources available to them in the community
Collaborate with the CM/Patient Type Status Nurses to ensure that the patients are in the correct level of care
Responsible to establish an appropriate discharge plan and arrange services to meet the needs of the patient
Ensure that there is clear documentation of the patient's anticipated discharge planning needs and a final note to ensure that all patient needs are met
Identify if the patient has a Health Care Proxy (HCP) and if not, offer assistance in helping them identify a HCP
Identify a Care giver and inform them of discharge planning arrangements
Ensure that arrangements are made for patient to have primary care follow up appointments or establishing them with primary care
Perform other duties as required.

Description:
The Behavior Technician (BT) will provide clinical skills instruction and behavior reduction protocols using principles of Applied Behavior Analysis (ABA) to children with Autism Spectrum Disorders and related developmental disabilities in the clinic, home, community, and school settings. BT will collect data on programs, assist with parent training, and assist supervisors with clinical assessments as needed. The BT also maintains accurate records, promotes good communication with families, staff, and supervisors, and provides accurate data collection. The BT implements goals and objectives designed by the Board Certified Behavior Analyst for the client through completion of assessments and gaining information from families.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Activities include but are not limited to the following:
Follow treatment plan and use methods of ABA to decrease challenging behaviors of individuals.
Provide direct care in one-on-one and group settings utilizing a combination of intensive teaching and natural environment training.
Follow the prescribed behavioral skill acquisition and behavior reduction protocols.
Collect and maintain data on academic, social, and behavioral goals.
Assist with educational, social, and behavioral assessment of individuals.
Assist with parent and caregiver training in line with the individualized treatment and behavior reduction protocols.
Assist supervisor with skill acquisition and behavior reduction assessments and in preparing individual materials as instructed.
Effectively communicate with parents and caregivers regarding individual progress as instructed by the Board Certified Behavior Analyst.
Complete all required documentation according to the Center's program regulations.
Participate in activities as part of the treatment team that may include meetings, trainings, and committees

EDUCATION AND EXPERIENCE:
High School Diploma or AAS Degree in Human Services or related field preferred
