How you will change lives
As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Proficiency in all USRC clinical applications required within 90 days of hire.
Preferred
Previous experience in providing social services to dialysis patients preferred.
Other Requirements
Must meet any practice requirement(s) for the applicable state.
Additional license requirements may be applicable depending upon state.

Responsibilities

Monitor and adjust behavior plans throughout the treatment period in accordance with the child's progression.
Conduct Functional Behavior Assessments (FBAs) as needed.
Participate in bi-weekly BCBA group meetings and monthly 1:1 meetings with the Clinical Integrity Officer (CIO) to discuss client progress or concerns.
Provide regular parent/caretaker training
Maintain knowledge of different programs and certifications through CEU events.
Assist with scheduling
Provide direct coverage as needed
Be present in the center during business hours
Assist with any clinical needs as they arise
Support kiddos and RBTs with a minimum of 28 impact weekly hours
Supervisory Responsibilities
Supervise RBTs who are working towards their BCBA certification
Conduct weekly competencies for newly hired RBTs and monthly competencies for RBTs who have been with the company longer than 1 month.
Review RBTs session notes for accuracy and provide detailed feedback to RBTs.
Provide initial corrective feedback to RBTs, when needed, as part of progressive disciplinary action procedures.

Qualifications
Education and Experience
BACB Board Certified Behavior Analyst (BCBA) required
A Master's degree in special education, psychology, or related field, with specialized knowledge of behavioral health and treatment philosophies and professional practices
Possess extensive knowledge and training in behavior analysis.
Previous experience in supervision preferred.

Work collaboratively with Professional Learning Communities (PLCs), teachers and campus and district administrators toward meeting identified district and campus improvement goals. 10. Develop and maintain a confidential, collegial relationship with all personnel.
Work cooperatively with the ELL Department Director and ELL Department staff to support the goals and commitments of the ELL Department for teachers and students.
Facilitate the development of district common assessments that facilitate the TELPAS process.

Attend Bilingual/ESL department meetings and trainings as scheduled.

As a Registered Behavior Technician (RBT) you will provide one-on-one, play-based, ABA therapy services to pediatric and adolescent patients with Autism Spectrum Disorder. Under the supervision of a BCBA, teammates will follow Behavior Intervention Plans (BIPs) and help patients further develop communication, motor skills, academic readiness, social skills, and independence.
Responsibilities:
Facilitate 1:1, naturalistic therapy services within a clinic-based environment
Track patient progress over time by collecting data and submitting clinical documentation
Following behavior intervention plans (BIPs) as outlined by your BCBA Supervisor
Communicate professionally and consistently with clinical management and families
Assist patients with daily living skills such as feeding and toileting
Adhere to patient confidentiality by following HIPAA guidelines
Provide behavioral support within speech or occupational therapy sessions
Obtain RBT Certification within your first 60 days of employment

Registered Behavior Technician job responsibilities include:
Provide direct client care in 1:1 and group settings utilizing a combination of intensive teaching and natural environment training
Follow the prescribed behavioral skill acquisition and behavior reduction protocols as outlined in the client's individualized treatment plan built by BCBA/BC
Collect, record, and summarize data on observable client behavior
Assist with parent and caregiver training in line with the client's individualized treatment and behavior reduction protocols
Ensure documentation is completed on time and following established procedures
Effectively communicate with parents and caregivers regarding client progress as instructed by BCBA/BC
Utilize safe and appropriate procedures when working with clients and appropriately report all critical incidents and safety concerns

Willing to obtain CPR and First Aid certification

Implement an instructional, therapeutic, or skill development program for assigned individuals served in a 1:1, 1:2, or group-format.

Participate in staff and treatment team meetings on a regular basis or as assigned
Conduct ongoing assessments of individuals served achievement through formal and informal testing at the directive of the clinical supervisor(s)

Document interactions and session summaries within electronic health record
