How you'll contribute
An Outpatient Therapist who excels in this role:
Responsible for providing group therapy for patients suffering from psychiatric and substance abuse issues. Ensure accurate and timely patient documentation.
Conducts group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems.
Displays active involvement in treatment planning process.
Provides family session counseling to all patients to ensure appropriate communication and involvement of family members and support groups.
Actively communicates with clients, families, and outside referral sources. Demonstrates proactive communication with those involved with the patient's treatment.

WellBe LISW - Social Worker
What to Expect:
Practices the WellBe mission: To help our patients lead healthier, meaningful lives by delivering the most Complete Care.
Performs psychosocial assessments and provides therapeutic services to patients and their families.
Work closely with Behavioral Health Medical Director to manage complex patient cases and transitions of care
A resource for the WellBe communities in managing complex Behavioral Health cases.
Manages complex behavioral and psychosocial needs that result in improved clinical and financial outcomes
Maintains constant communication with patients when addressing their concerns and goals and helping them keep positive attitudes
Builds rapport with patients and their families and relay all concerns to WellBe provider/IDT
Assists members to effectively utilize available resources to meet their personal health goals and help them develop their own capabilities.
Monitors patient progress and adjusts patient treatment plan as needed
Coordinates with other healthcare providers, agencies and community resources in order to create a thorough treatment plan addressing social, cultural, and financial needs
Collaborates with the interdisciplinary team and participate in regularly scheduled team meetings
Ensures compliance with regulatory agencies and WellBe clinical guidelines
Following a thorough assessment completed by an Advanced Practice Clinician (APC), the WellBe social worker partners
Partner with community team to refer patients to appropriate no or low-cost community services that support health and independence such as Meals On Wheels (MOW), local Area Agencies on Aging (AAA), Senior & Assistance Programs, and transportation resources.
Involves the patients' families and primary caregiver(s) as needed to achieve the best care decisions and outcomes.
Effectively collaborates with all those involved in the members' care, including health services contractors (i.e., Home Health, Hospice, Community Agencies), to meet the patients' care goals.
Consults with and advises the Clinical Team regarding the relationship of social, emotional, and cultural factors to health and medical care, and to the availability of social services in the community.
Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to their supervisor/team. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.
Consistently meets or exceeds WellBe targets for productivity, customer service, quality assessment, and performance targets.
Understands the importance of community involvement and participates as appropriate in activities that link WellBe to its communities.
Maintains current written progress notes and other documentation on the member Medical Record including signed and dated documentation for all service performed the day the service is provided.

Must Haves:
Educational/ Experience Requirements:
Master's Degree in Social Work
5+ years clinical social work experience with the geriatric population
Two years social work experience, preferably in health care, and minimum one-year experience working with a frail/elderly population.
License, Certification, Registration
LISW Licensure required
Required Skills and Abilities:
Current unrestricted LISW license in applicable state(s) required.
Age specific competency in working with the elderly and knowledge of community services for the elderly and their families.
Ability to make psychosocial assessments and develop and implement viable care plans
Must be familiar with EHR medical documentation
Strong computer skills, including Word, Excel, and Powerpoint
Strong verbal, written, presentation, and interpersonal communication skills

This position will be working directly in a residential therapeutic setting with individuals in need of behavior stabilization with a diagnosis of autism spectrum disorder. Constant engagement with and supervision of individuals will be a requirement for staff. This will include programs specific to the individual's needs based on their IP; implementation of Applied Behavioral Analysis (which is a highly scheduled, intensive therapeutic approach to teach expected behavior and life skills); implementation of highly structured Behavior Support Plans and other approaches that the team determine appropriate for the individual. Crisis intervention is likely for issues including self-injury, aggression, and high magnitude disruptive behavior. This position will take lead on each shift to ensure the treatment team is functioning smoothly and all daily requirements are complete. On call rotation required.

Assists in maintaining household files for compliance with applicable local, state and federal standards as needed; reports unusual incidents and communicates unusual medical situations as required; performs first aid and other procedures as trained and designated.

The Camp Behavior Specialist position is responsible for providing behavioral support to campers, and for providing coaching to seasonal staff regarding behavioral management of campers with mild developmental disabilities, intellectual disabilities, physical disabilities, and/or behavioral challenges. Camp Behavior Specialists may work with various locations, including Camp Hannah, Camp Friendship and Camp Begins.
Camp Behavior Specialists work flexible schedules between 7am- 6pm. Typical hours are 25-30 hours a week. Internship credit is available.
* Develop, plan and implement personalized behavior plans for campers which are developmentally appropriate and supportive to campers while attending Gahanna's summer camps.
* Effectively communicate with parents and/or guardians to collaborate in the creation of behavior plans to best reach the child, and provide parents and/or guardians with updates about their child's behavior throughout the summer.
* Provide Camp Counselors with clear direction for the implementation of camper behavior plans, which includes facilitating a learning session at pre-season training.
* Assist Camp Counselors with handing interactions where campers are having difficulty with their intervention plan.
* Facilitate small group social-emotional learning activities and conflict resolution exercises, as needed.
* Maintain documentation of incidents, behavioral plans, and behavioral progress tracking.
* Abide by all mandatory reporting and communication policies, as described in the staff manual.
* Evaluate the effectiveness of programs and recommend modifications to existing programs.
* Support other department functions as assigned.
* Provides internal and external customer service and maintains relationships.
Regular, predictable, and punctual attendance is required.
Any combination of education, training, and experience as listed below which provides the required knowledge, skills, and abilities to perform the essential functions of the job. A typical way to qualify is by having obtained (or currently enrolled in a program to obtain) a bachelor's degree for Applied Behavior Analysis or a closely related field.
* Licensure or Certification Requirements
* Current and valid Ohio driver's license with an acceptable driver's abstract, to meet criteria for insurability established by the City of Gahanna.
* First Aid/CPR/AED certification or willingness to obtain.
* Assist other members of the Leadership Team in planning all-camp activities, community involvement programs, and enrichment sessions for the specified camp program area.
* Demonstrate enthusiasm, while leading, assisting, and/or actively participating in all program areas, as assigned.
* Maintain exceptional standards of cleanliness, health, and safety in all camp operations, always adhering to American Camp Association standards.
* Be alert to equipment and facilities, to ensure utilization, proper care and maintenance is adhered to. Promptly report repairs needed to the camp management team and/or supervisor.
* Maintain an appropriate level of formal/professional conduct, in an informal environment.
* Provide parents/guardians and the public with appropriate feedback and information, when addressing questions and concerns, with assistance from the Leadership Team or supervisor, as needed.
* Be available for camp staff training prior to the camp season and for in-service training/meetings during camp season; some of these may occur outside of regularly scheduled program hours. Pre-season availability is required.

What we're looking for
