Seeking a flexible, open-minded change agent who has demonstrable real-world Agile and Waterfall experience. In order to create an environment for transformation, this person must demonstrate the ability to influence the mindset and behaviour of teams and individuals. When working with teams, it is important for this individual to push back against prevailing norms, create a learning environment and lay the groundwork for slow but ongoing organizational and social change. This person should continually hone their facilitation and coaching skills, staying ahead of new developments in Agile and Waterfall. To guide each team's delivery process journey, they must embody the Agile Manifesto, Scrum Guide, Kanban practices and Waterfall process. This position requires Delivery Process excellence mindset, basing decisions on empiricism and identifying opportunities to continuously improve the way teams deliver projects, products and services. The Delivery Coach will work with individuals and teams within a designated functional or geographical area, as well as their executives, leaders, and managers. Additionally, this coach will need to partner closely with the Delivery Excellence Lead to ensure alignment with the strategic goals and value creation plans.
BASIC OBJECTIVE
Influence mindset and behaviours, including those of senior leaders of the organization.
Coach and support Project Managers, Scrum Masters, Kanban Facilitators, and teams as they adjust to and embrace software delivery ways of working
Instill Project Management and Agile principles through training and “in the moment” coaching with teams.
Ensure Project Managers, Product and Platform Teams focus on achieving business outcomes and goals.
Empower Teams to become self-managing, cross-functional with an attitude of relentless improvement
Facilitate Scrum Masters and Kanban Facilitators in aligning to Agile practices within a specific set of guidelines and boundaries provided.
Coach Project Managers using Waterfall Coaching Framework and perform regular assessment to mature in delivery process
SPECIFIC RESPONSIBILITIES
Functional Responsibilities
Assess and enhance Agile and Waterfall process maturity
Be an ambassador for the IT methodology - Waterfall or Agile and be able to explain the value of the process and various phases
Promote best practices in IPEC(Initiate, Plan, Execute, Close) for Waterfall delivery as it relates to waterfall as a process, activities within each phase of IPEC, generation and maintenance of project artifacts
Share findings that impact Waterfall/Agile adoption, process maturity and performance identified during Project Manager, Scrum Master and Product Owner sync-up meetings with Centre for Delivery Excellence (CDeX).
Partner with Delivery Excellence Lead in implementing Centre for Delivery Excellence (CDeX) OKRs and strategic initiatives within the team
Work with the Delivery Excellence Lead to complete metrics roll-up and reporting.
Perform Waterfall and Agile Maturity Assessment at regular interval of time and create coaching plan for each project or team.
Enable Agile way of working
Use the Agile Delivery Framework IPEC (Initiate, Plan/Activate, Execute, Close) for software delivery.
Partner with Delivery Excellence Lead to support Agile Activation Process by reviewing Team Charter, Conducting Scrum 101 or Kanban 101 Training, IPEC for Waterfall Trainings
Partner closely with Project Managers/Scrum Masters/Kanban Facilitators of assigned teams in evangelizing IPEC process.
Partner with CDeX - Tools team in setting up ServiceNow PPM or Scrum/Kanban boards in Jira and Confluence for documentation.
Collaborate with Business Leaders and Delivery Leaders in addressing Delivery Process adoption challenges.
Provide coaching, support, and guidance to team in partnership with the Project Managers, Scrum Master, Product Owner or Kanban Facilitator
Hold weekly one-on-ones with Project Managers, Scrum Masters, Product Owners and Kanban Facilitators to provide coaching, feedback opportunities and ad hoc training.
Support the Business Solution Managers(BSM) and Program/Project Managers during readiness discussions by guiding them in establishing alignment at critical checkpoints throughout the project lifecycle (e.g. PLAN, Kickoff, Project Closure)
Attend PM CoE and Scrum/Kanban Communities of Practice.
Participate in other shared learning events and activities with the community.
Facilitate Quarterly Product Planning (QPP) and Quarterly Business Reviews(QBRs) events.
Ensure teams are focused on delivering business outcomes and usable features of the highest priority
Coach Project Managers on Waterfall process guideline and Coach Scrum Masters, Product Owners and Kanban Facilitators on the customer and product thinking mindset.
Ensure Scrum Masters, Product Owners and Kanban Facilitators help the teams understand the WHY and the business value that is being delivered to the customer/end user.
Encourage the use of Qualitative and Quantitative metrics to help with continuous improvement for the waterfall project teams to become high performing delivery units.

The ICOACH will work successfully within a team as well as professionally represent Waypoint, supporting individuals using in person coaching and the remote coaching platform. Requirements
Activating and connecting to each location monitored by cameras and sensors by establishing a connection to each site at the beginning of every shift. Reconnecting to each location and checking the status of the cameras and sensors on an hourly basis or as assigned (24/7). This check may be performed more frequently if requested by the individual living arrangement due to the specific needs of members.
Monitoring the presence of staff and compliance with minimum staffing ratios in homes/programs and contacting supervisors when staffing shortage is observed. When needed, dispatching staff to a location to resolve a situation as detected by the system or due to monitoring pre-scheduled needs of the consumers living at each residence. These needs may include, but are not limited to, turning, or repositioning a consumer at certain times, assisting with teaching interactions, and responding to safety or behavior issues
Problem solving with each individual team regarding ways to best support the needs of individuals served in each living arrangement by providing support and guidance to program staff as needed and keeping accurate records regarding issues occurring throughout their shift.
Ensuring the safety, and rights of persons served and responding to requests for assistance from program staff and/or persons served utilizing a variety of required communication tools, implementing home security measures, and using sound judgment to react to unplanned and emergency situations.
Remotely supporting persons served to increase their independence and to exercise their rights by remotely and in person prompting and teaching skills in various activities of daily living as natural teaching opportunities arise, using informal teaching procedures.
Provide support and guidance for members as outlined in their Personal Care Plan.
Exercise sound judgment to ensure the safety and welfare of members. Notify supervisor of any situation developing or occurring in the home which could have a negative impact on the home's operation or member's welfare and safety.
Acknowledge and respond to software notifications within appropriate timeframes, while prioritizing alert responses appropriately.
During remote coaching hours respond to safety concerns and respond accordingly, dispatching in person staff as needed.
Serve as an advocate for individuals receiving remote support by making recommendations to the appropriate supervisor/administrator, in areas where changes, updates and modifications can improve the quality of services/supports the member receives.
Respect each member's right to privacy and confidentiality.
Ensure that all medications are passed in a safe manner.

· Strong communication skills, desire, and ability to support and teach others remotely.
· A willingness to implement positive behavior supports, implementing structured activities, collecting accurate treatment data and behavior data, and demonstrating effective communication with members, families, coworkers, and other professionals.
· Prior experience with Assistive Technology or the use of Electronic Health Records is desirable. A willingness and demonstrated aptitude using computers, smartphones and tablets and focus attention on screens displaying the monitoring station software

Performs intake and assessment of patients with newly diagnosed or poorly controlled chronic conditions, screens for Health Home eligibility, and checks Health Home enrollment
Screens for behavioral health and substance use problems, including depression (PHQ), alcohol abuse (AUDIT), substance abuse (DAST) and smoking status.
Reinforces education provided by PCP or nurse on management of the chronic disease, provides self-management tools, and reviews how to use those tools.
Establishes goals and creates a care plan.the patient, Works with patient to mitigate impacts of social factors on health and functional status, e.g. by arranging transportation for patients.
Does post-visit review of next steps with patient.
Coordinates care, assists with referral management, and conducts between-visit monitoring and outreach.
Tracks and follows-up on test results to ensure patient and caregiver take appropriate next steps.
Serves BSFHC's first point of contact during post-discharge care transition from hospital or emergency department.
Works with DSRIP Coordinator and practice's performance improvement team.
Assists in scheduling huddles and other internal team meetings.
Education/Experience Required:
Certification from the National Board for Health and Wellness Coaches (NBHWC) required.
Strong background in nutrition preferred.
Minimum of 2 years' experience as a health coach, including conducting one-on-one and group coaching.
Strong background in health promotion, chronic condition management, and behavior change methodologies.

Maintain HIPPA patient/employee confidentiality
Knowledge of EMR systems a must

The Schedule:
The majority of classes happen in the mornings or early afternoon, The ideal candidate would have the availability on some of the days and times listed below:
Monday-Friday: 8:00am-4:00pm
Weekends: 9:00-12:00
Note that you do not need availability during all of these times, we work together to determine the schedule that works best
The Locations:
Classes are held across schools, parks, and recreational centers. Coaches are expected to travel up from 5 mins to an hour to get to a class location. Admin staff will staff coaches in the closest locations.
Coaches are expected to arrive 15 minutes before classes start, and are paid 15 mins beforehand and 15 minutes afterwards, 30 mins extra per shift.
Other details:
Part time position
Hours can be anywhere from 5 hours-20 hours per week
Coaches hold on to sports equipment
Patience is a key attribute for success in this role. We are teaching sports, nutrition and muscle groups to 2 - 8 year olds using fun activities.

Deliver pre-designed workouts to groups of 4+ members
Program and implement individualized workouts for members

Provide options and challenges for specific exercises to accommodate various skill levels
Attend weekly/monthly fitness staff meetings and required educational presentations

Qualifications
Have a strong knowledge of physiology, exercise technique, and body mechanics
Must acquire a fitness certification/personal trainer certification within 90 days of hire from an AF fitness-approved education company such as ACSM, ACE, NASM, NSCA, ISSA, NSCA, etc.
Prefer NASM CSCS certified
Ability to multi-task and stay organized

Job Descriptions/Duties The Family Success Network Family Coach provides direct services to families including administration of screening tools, home visiting, parent education programs, resource management and referral, concrete supports, financial literacy and family coaching. Specific activities include, but are not limited to, the following:
Provide ongoing supportive consultations and prevention case management services delivered either virtually or through a combination of phone calls, emails, skype, or in-person through individual meetings, including but not limited to:
Assessing and administering universal screenings, financial literacy training, and/or concrete supports.
Universal screenings will assess families for trauma, substance use, and mental health. The tools utilized may include but are limited to: the Adverse Childhood Experience (ACEs) Survey; the Child Trauma Assessment Checklist (CTAC); UNCOPE (substance abuse screening); and a mental health screening tool.
Provide short-term, intensive, in-home services:
Provide interested families a series of one to two home visits each month and access to the full array of the six pillars (family coaching, parent education, basic life skills, financial literacy, service referrals, and concrete supports) of service delivery.
Serve as a Family Coach to develop a prevention services plan, outlining the families' goals and will utilize evidence-based programming to enhance the families' protective factors and increase the family's stability.
Provide parenting programming and facilitate access to Life Skills training, as well as Family Coaching, which will incorporate Motivational Interviewing to assist the family in moving towards behavioral change.
All families engaged in Family Success Network services will work with a Family Coach who will complete the following tasks, as agreed upon by the family:
assess the strengths and needs of families
provide concrete support in times of need
develop and work with families on family success plans
provide financial literacy
utilize trainings to engage families and address identified needs, including but not limited to Triple P, Transition to Independence Process Model, Motivational Interviewing, and Strengthening Families Framework
provide services directly to families in their own homes or neutral setting
Manage an ongoing caseload of up to 30 families at one time, engaged in and receiving services.
Attend meetings amongst collaborative members.
Develop rapport with local social service partners and community resources to build and maintain a professional, responsive relationship with key community partners.
Document services consistent with requirements of OCTF Program Manager and FSN Program Manager.
Submit regular programmatic and fiscal reports to the Program Manager, as well as to other identified partners requesting data.
Actively engage with family and child serving systems, including attendance at community meetings, such as Family and Children First Council.
Represent the OCTF at any required national and local trainings and meetings.
Serve as point of contact for the county of service; and
Other duties, as assigned.
Qualifications
Minimum high school diploma is required; college preferred.
Experience working with families for a minimum of one year.
Must have a valid driver's license and reliable transportation.
Must be flexible, knowledgeable of community resources, and a team player.
Ability to work with diverse population is required.
