JOB SUMMARY
The Chaplain ministers to patients, families, and staff by providing appropriate spiritual services such as listening, pastoral counseling, ethical consultation and prayer regardless of the individual's personal faith tradition. The individual participates in educational programs and organizational committees related to the Chaplain's area of ministry and acts as a resource for physicians, staff, volunteers, and the larger faith community. The Chaplain helps promote and maintain an atmosphere of quality patient-centered care by providing personal presence, pastoral ministry, and in-service education.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Master's degree in theological, ministerial, or related field. Must include two units of Clinical Pastoral Education or one unit and one year chaplain internship. Endorsement by Professional Chaplain Organization and/or Denominational Representative.
Preferred/Optional: None
EXPERIENCE
Minimum Required: One year of experience in chaplaincy.
Preferred/Optional: Three years of experience in chaplaincy in any healthcare setting.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: None. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.

Purpose of the Position
Rainbow Community Care is currently accepting applications for a part time Chaplain/Spiritual Counselor. The Chaplain is a member of the interdisciplinary team and provides spiritual and bereavement support according to patient's/family's wishes and belief system consistent with Rainbow Community Care's policies and procedures.
Key Responsibilities
Assist in the implementation and coordination of Rainbow Community Care's spiritual and bereavement services programs and the delivery of those services to patients/families.
Provide direct spiritual support and/or counsel to patients/families while keeping with patients'/families' beliefs.
Work with staff, clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families experiencing terminal illness and loss.
Participate in patient conferences by exploring and assessing the potential spiritual needs of patients/families and reporting on services as indicated.
Provide bereavement follow-up services as defined by Rainbow Community Cares's procedures.
Maintain proper records of visits to patients/families.
Make contact with clergy or appropriate representatives of patients/families as indicated.
Perform occasional liturgical assignments such as the annual Memorial Service and memorial services with staff.
Conduct or make arrangements for funeral or memorial services when indicated.
Other assignments as needed.
Qualifications
Prefer graduate of accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry.
Demonstrated ability to be open, sensitive, flexible, and ecumenical.
Demonstrated experience in working with patients/families with life-threatening illness and death and grief.
Valid Wisconsin driver's license and proof of current car insurance are required upon hire and must be maintained throughout employment.
Must be able to drive to and from care facilities, patient homes, and/or other locations as necessary for your role.
Must be able to maintain healthy boundaries and positive interpersonal relationships.
Must be tactful and courteous in communication with others, whether in-person, by phone, or in writing.
Must act with integrity and demonstrate good work ethics including reliable attendance and punctuality.

Major Responsibilities:
Responsible for delivering a person-centered ministry of pastoral care, counseling, consultation and sacraments for patients, families, physicians, and hospital personnel.
May assist families with their needs in relation to the patient's illness so they are enabled to provide supportive care to the patient; assist family members in understanding procedures surrounding a death and in contacting families or other significant persons.
Provides pastoral care services, consultation and short term counseling to patients, families, physicians, and hospital personnel as assigned and in accordance with department policies.
Serves as message bearer between family and patient or family and medical staff during critical care/crises treatment
Facilitates sacramental ministries such as Holy Communion, Baptism, Confession and Absolution, Sacrament of the Sick and such rites
Responds to Medical emergencies, Deaths, Codes Blue , Rapid Response, Code triage and other emergencies as appropriate.
Provides pastoral consultation and support to hospital personnel experiencing the death of a patient or a critical incident in relation to patient, family or staff.
Facilitation of decision making related to medical ethics through identification and clarification of ethical dilemmas
Facilitates and encourages discussion and decision making related to medical-ethical dilemmas such as termination of treatment, organ donations, do not resuscitate (DNR) orders, living wills and health care power of attorney, and ensures that religious/faith issues are addressed.
Provides consultation to members of the health care team to aid in assessing the role of a patient's faith perspective and faith community in recovery.
Responsible for liaison between the hospital and religious communities.
Supports the hospital ministry of parish clergy with referrals, information and consultation.
Represents the values of the religious communities to health care professionals and employees of the medical center.
Education/Experience Required:
B.A./B.S. or equivalent Religious: Ecclesiastical endorsement R.C. Sister, Brother, Permanent Lay Deacon preferred.
Knowledge, Skills & Abilities Required:
1 unit of CPE accredited by ACPE. NACC units may be considered.

Physical Requirements and Working Conditions:

Ability to lift up to 35 pounds without assistance. Job Description:
KEY ACCOUNTABILITIES:
* Provides direct emotional and spiritual assessment and care for patients and families to meet identified emotional and spiritual needs.
* Adjusts approaches to reflect developmental level and cultural differences of population served including spiritual/religious informed care or service to deliver individualized patient-centered care.
* Provides ethics consultation in collaboration with the bioethics committee to navigate through difficult patient care situations.
* Collaborates with area clergy and faith community leaders to best support patient or family needs.
* Works with the interdisciplinary team to identify and respond to the spiritual and emotional needs of patients, families, and team members.
* Participates in the provision of education and training as appropriate, and participates in improvement activities that involve pastoral care services to improve overall quality of care.
QUALIFICATIONS:
* Functional knowledge of the religious principles and practices of major faiths
* College and seminary degrees; (MDiv- preferred), Clinical Pastoral Education (CPE): 4 units CPE required, from an accredited center. Ecclesiastical or endorsement as a religious leader from a federally recognized faith group or community.
* Professional certification - eligible for or board certified as a chaplain by a national certifying organization (i.e. Association of Professional Chaplains, National Association of Catholic Chaplain, etc.). Board certified within two years of hire.
PHYSICAL DEMANDS:
* Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance

Requirements:
* Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred
* Must have experience working with death and dying, individuals/family/caregiver
* Hospice experience preferred

Position Specific Responsibilities
Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
Submits appropriate documentation and paperwork to facilities at the completion of patient visits; documentation is completed and submitted timely according to company policy.
Articulates the regulatory framework governing the role of the Spiritual Counselor (federal and state, if applicable).
Participates as a member of the IDT, including development and implementation of the plan of care.
Provides spiritual care support to the patient and family; supports the involvement of community clergy as the patient/family request.
Assists in identifying the need for intervention of other IDT members.
Effectively communicates patient and family needs to IDT.
Collaborates with IDT to coordinate spiritual care support for the patient and family to ensure appropriateness, continuity, and quality of care.
Maintains updated, comprehensive care plans which reflect current problems, goals and interventions for patients.
Maintains respect for the family's environment and belief systems, and remains nondiscriminatory regarding age, race, religion, sex, sexual orientation, national origin, physical or mental disability, and other personal matters.
Counsels in accordance with patient's and family's beliefs.
