Position Overview:
Case Management consists of services that help clients gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. JOB SUMMARY AND QUALIFICATIONS:
Responsible for the overall Clinical services to families in the Family Recovery Program in Hardin County, to include children. It requires a reasonable combination of the following knowledge and skills; Work with individuals with substance use disorder, ability to use and implement cognitive behavioral therapies and use of community resources and ability to serve as an appropriate role model. Compassion towards people living with mental illness, substance abuse disorders, and HIV/AIDS and ability to convey respect to the target populations are critical. Must learn and apply agency, personnel and program policies and procedures. Must have knowledge and understanding of the current treatment modalities. Preference given to those that have the ability to train and supervise other clinical staff and interns.
WHAT YOU SHOULD HAVE FOR THIS ROLE:
The position requires a minimum of a Master's degree in social work or a related field AND the accompanying certification (CSW, MFT, LCPA) with a preference given to a clinical license (LCSW, LMFT, or LPCC) Preference given to candidates who have completed a practicum working in behavioral health and substance use issues.
RESPONSIBILITIES:
1. Manage caseload and client needs
2. Work effectively with referral sources.
3. Document services through current, complete records.
4. Counseling on personal, family financial, employment, behavioral, crisis and substance abuse matters.
5. Advise other staff on treatment matters and coordinate team treatment as appropriate.
6. Discuss and review client progress
7. Participate in client review.
8. Maintain a current CPR and First Aid certificate.
9. Participate in weekly clinical supervision.
10. Conduct group and individual sessions daily.
11. Maintain billing requirements.
12. Performance quality improvement (PQI) duties as assigned by supervision & PQI committee.

Position Overview:
Case Management consists of services that help clients gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case Management includes: (1) assessment of the eligible client to determine service needs; (2) development of an individualized integrated care plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation.

The Case Manager (CM) is part of the Transitional Living Care Team and carries out case management responsibilities to coincide with the work being done by other Care Team Members to move residents toward a goal of healthy independence.

Program Support for Transitional Care and Independent Living
Uphold Refuge for Women culture, program requirements, rules and standards of conduct per resident guidelines
Assist and empower residents to be in control of their ongoing needs related to medical insurance and food stamps.
Alongside the Program Director, work to empower residents to be proactive in their overall well-being with reminders to make appointments for medical, psychological, dental, prescription pick-ups, etc.
Empower and support residents to find means of transportation to appointments, work, church, etc.
Along with PD, assist each resident with creating an individualized transitional case management plan including short term and long term goals
Coordinate all services for each individual client, including advocacy, giving information about community resources, etc.
Conduct group sessions weekly and assist with transportation to and from meetings
Provide accurate documentation of services, including identifying resources, writing progress notes, detailed summaries of all services and contracts and submits as required or outlined. Completes monthly progress reports as required.
Complete and submit case management notes weekly.
Complete documentation regarding all contacts and information regarding the case
Responsible for continued outreach and engagement of clients through telephone calls, home visits, and ongoing communication with all members of the program services team.
Oversees the collection and review of all collateral information including but not limited to: social histories, psychological evaluations, judicial records, previous substance use treatment information, DCBS reports, and obtaining a release of information for everyone involved in the case.
Other Responsibilities
Meet weekly with the Program Director to go over goal progression
Coordinate with vocational partners (Made 180, REEL KY, Fair Chance Works, SE KY Economic Development) to help residents stay engaged with ongoing training classes, social entrepreneurship and financial management classes
Enroll each resident immediately in Career Center through assigned liaison
Skills and Expectations
Strong written and verbal communications skills including Word, Excel and Google email and applications
Strong project management, servant leadership and teamwork skills
Self-motivated, organized
Ability to adapt to frequently changing priorities and handle crisis situations
Qualifications
BA degree in human service field, preferably Social Work
Case Management certification in proper jurisdiction required
