Our company, Apotheco Pharmacy, is searching for a Floating Pharmacist. Primary location will be Paramus, NJ , but will need to travel to our satellite locations in Woodland Park,NJ, Parsippany, NJ, and Jersey City, NJ. 1-2 Saturdays 10am-2pm a month will be required.
Salary is based on upon experience.
The Pharmacist will oversee and provide direction related to quality, clinical and compliance for Apotheco Pharmacy operations initiatives in compliance with state and federal laws. The Pharmacist will report to the Pharmacist-in-Charge. Must have excellent customer service skills and be detailed oriented.
Work with Pharmacist-in-Charge to maintain and increase efficiency, quality, and customer service levels in the daily operation, as well as execute on department goals. Continue performing essential pharmacist functions of a staff pharmacist.

Duties and Responsibilities:
They include the following; other duties may be assigned;
1. Manage staff of techs, patient care coordinators, and customer service representatives while on duty.
2. Adheres to all Apotheco policies.
3. Adheres to all applicable laws pertaining to the practice of pharmacy.
4. Maintains a safe and clean pharmacy by complying with procedures, rules, and regulations designated by the Board of Pharmacy.
5. Maintains records by recording and filing physicians' orders and prescriptions.
6. Responsible for all cash transactions and depositing of all transactions.
7. Ensures medication availability by delivering medications to patients and departments.
8. Contributes to team effort by accomplishing related results as needed.
9. Knowledge of medication and dosage measurements.
10. Proficient at pharmacy software system and electronic ordering of all medications.
11. Perform final check on prescriptions annotating on the prescription and scanning in changes
12. Call physicians when applicable for refills, medication changes, and prior authorizations.
13. Making sure all drugs have a minimum of 6 months prior to expiration
14. Perform return to stock duties for prescriptions not picked up within 14 days
15. Counsel patients on all new prescriptions
16. Pharmacist responsible for ensuring overall compliance with state and federal statutes.
17. Responsible for adherence in state environment, for all federal regulations involving prescription medication fulfillment for retail pharmacy.
18. Facilitate and monitor compliance with Prescription Drug Monitoring programs. 19. Oversee pharmacy team operations compliance (e.g. transferring prescriptions, handling inbound/outbound physician calls, efficient drug utilization management, safe dispensing, providing accurate and timely medication dispensing, & ensuring adherence and compliance with all state and federal regulations).
20. Ensure quality and productivity standards are met and/or exceeded in a consistent basis.
21. Lead in communicating management of Pharmacy policy and procedures relevant to pharmacy compliance and quality standards; I. Facilitate implementation of new programs, enhancements. II. Maintain professional licensure, competence, knowledge and skills necessary to effectively complete responsibilities, enhance job knowledge and ability to actively pursue learning and self- development for you and your staff. III. Follows all policies and procedures relating to job responsibilities to meet corporate objectives. IV. CSOS ordering capability for site. Understanding how to order, file, and audit ordering C2 prescriptions electronically. V. Ensure Refrigeration logs are being filled out and monitored daily. Responsible for filing those logs electronically and in file cabinet. VI. PIC will be responsible for Alarm at site. PIC will have to be one of the people assigned on the alarm system or as assigned. VII. Recall of drugs responsible for returning drugs to wholesaler or direct vendor both legend and C2. Maintaining paperwork as required and contacting members when necessary. VIII. Expired drugs: responsible for determining procedures along with manager of fulfillment pertaining to disposal and credits of expired drugs. IX. Inventory: aid in the process of inventory validation as needed. X. Variances with inventory validate along with manager of inventory and finance any variances. XI. C2- all C2 prescriptions must be counted by a technician and then recounted by a pharmacist. C2 count one section per week or delegate to another pharmacist and record in controlled substance log. XII. Other activities as determined by management.

Prepare PowerPoints and present educational or compliance-related topics to AH constituents.
Draft compliance and HIPAA-related policies and procedures, as well as policies and procedures related to other assigned risk areas.
Assist in conflict-of-interest reviews, evaluations, and determinations.
Assist in conflict-of-interest endorsement requests reviews.
Qualifications:
• Education: A Juris Doctor degree from an ABA accredited law school and admission to the bar to practice law in one of the 50 States (or Washington, D.C.) of the U.S. is required. The holding of a Master's Degree in public health, healthcare, accounting, allied health, clinical-related studies, compliance, audit, computer science, finance, education, law, privacy, information security, information governance, or another field related to the responsibilities of the position at hand, is a plus.
• Certifications: Candidates who hold the “CCEP” or “CHC” designation from the Compliance Certification Board are strongly preferred. Any successful candidate who does not hold the “CHC” designation at the time of appointment will be required to obtain the same within 12 months of appointment. A successful candidate must obtain either the “CISA”, “CRISC”, or “AAIA” designation from ISACA within eighteen (18) months of employment.
Experience:
• Five years minimum of health care compliance experience, health care administration, legal, fraud, waste, and abuse; internal audit, organizational compliance, regulatory affairs or a filed related to the responsibilities of the position at hand, is preferred.
• Experience in conducting compliance investigations, legal internal investigations, or similar investigations is required;
• Managerial experience in a healthcare organization or related setting is preferred.
Technical:
• Proficiency in Microsoft Word, Excel, PowerPoint.

Partner with the Medical Director of Medical Specialties to provide medical oversight for cardiology offices and hospital-based cardiology services.
Supervise APG cardiologists, advanced practice nurses (APNs), and physician assistants (PAs).
Oversee hospital-based cardiology procedures.
Facilitate effective communication with leadership regarding practice progress, challenges, and achievements.
Analyze referral barriers (internal and external) and develop strategies to enhance relationships with referring providers/institutions.
Regularly review financial data, metrics, and performance for both practice-based and hospital-based cardiology services; ensure alignment with budgetary goals.
Develop targeted strategies for underperforming practices or physicians in collaboration with the Medical Director of Medical Specialties.
Drive initiatives to improve physician engagement and satisfaction within APG Cardiology.

Support the development and growth of the newly instituted cardiology fellowship program.

Responsible for assisting patients referred to AHS for assistance in accessing and utilizing health care resources due to their complex chronic illness and co-occurring psychiatric diagnoses. Improves health outcomes through coordinating care, educating patients, building trust between patients and medical practitioners, and enhancing communication and the continuity of care. Teaches, counsels and monitors patients on health issues relevant to their care. Consults with other health care team members to coordinate the services of patient education, preventive care and disease management.
Principal Accountabilities:
1. Performs care coordination for high-risk patients to ensure continuity of care and quality healthcare delivery across the care continuum.
2. Utilizes established clinical pathways to facilitate comprehensive and consistent chronic disease specific condition education.
3. Maintains patient confidentiality with all activities in the Care Coordination Program.
4. Provides care coordination by collaborating with a multidisciplinary team.
5. Promotes patient adherence to individualized plan of care.?
6. Collaborates and communicates patient findings to primary care provider, Care Coordination Team, and healthcare providers, and identified care-partner(s) as appropriate across various health care settings via timely, accurate, and clear communication modalities.
7. Utilizes evidence-based risk-stratification tools to proactively assess chronically-ill population, identify patients at risk for disease-related sequelae and deliver patient-centered evidence-based interventions to prevent future progression or exacerbation of illness.
8. During exacerbation, intervenes at the earliest opportunity through multidisciplinary collaboration with all members of the care team, including patient and identified care-partner to return the patient safely to optimal level of health/function in an optimal setting.
9. Identifies opportunities for health promotion and disease prevention. Identifies Social/Behavioral Health needs which impacts patient's outcomes and appropriately refers to Social Work team member.
10. Assesses/Refers patients to available community support services as deemed necessary.?
11. Collaborates with multidisciplinary team to develop innovative strategies addressing the needs of the population.?
12. Assesses the self-care needs of the high-risk patient and identifies need for additional support. Provides patient/care-partner(s) education regarding prevention/recognition/treatment of disease exacerbation and promotes self-management of chronic condition and comorbidities.
13. Participates in evidence-based research, performance improvement, and quality initiatives as appropriate.?
14. Assumes responsibility and accountability for professional development and pursues ongoing continuing education to deliver patient-centered care. Demonstrates successful completion of ongoing proficiency and compliance with regulatory requirements.
15. Assists in orientation and training of new staff as requested. ?Participates in provider outreach and education. Serves as a community resource, providing educational programs for the community.
16. Collaborates with AHS leadership on additional tasks as requested.? ?

Principal Accountabilities: 1. Applies surface electrodes to patient's head, using adhesive paste or tape, to obtain electroencephalogram (EEG) measurement and attaches other combinations of sensors and electrodes to patient to obtain measurements such as electromyogram (EMG), electrooculogram (EOG), electrocardiogram (EKG), air flow respiratory effort, and oxygen saturation, as requested by physician and following established procedures.
2. Operates closed circuit television camera to observe patient during test and to record patient's sleep activities.
3. Operates polysomnograph equipment to record electrical activity of brain waves and other physiological variables and records notes on graph to eliminate from consideration physiological measurements caused by such activities as patient opening eyes, turning head, or turning body.
4. Observes and studies polysomnogram to recognize arrhythmias and abnormal respiratory patterns and calls physician or other emergency personnel if needed.
5. Measures duration of brain waves recorded on polysomnograms, using millimeter ruler.
6. Studies characteristics of completed polysomnogram tracings and summarizes data showing stages of sleep, abnormal breathing events, periodic leg movements, arrhythmias, and other information, applying knowledge of polysomnograph testing principles.
7. Enters data into computer and writes report incorporating patient's medical history, completed patient questionnaires, previous and current polysomnogram information, presence and type of abnormality, and other information for analysis by physician or other health professional.
8. May apply electrodes to patient's head.
9. May supervise and coordinate activities of other technicians.
