The Behavioral Health Home Hospital (BHH) Research Social Worker/Case Manager supports an innovative care delivery model offering specialized psychiatric-care services in patients' homes as an alternative to inpatient psychiatric hospital admission.
Provides quality clinical services to patients in various community settings, delivering hospital/inpatient-level treatment.
Collaborates with professionals to address psychosocial, environmental, age-specific, and cultural issues to maximize well-being and effective use of healthcare and community resources.
Utilizes clinical social work techniques for consultation, assessment, treatment interventions, and advocacy.
Sees patients in person at their homes or remotely, managing up to 4 patients at a time.
Ideal for someone interested in a temporary position working on a research trial to broaden treatment opportunities for patients with active acute psychiatric conditions.
Position begins on 1/5 with schedule depending on caseload, approximately one hour per day.
Job Requirements
Must hold a professional license and/or certification as required for the position.
Primary source verification of professional license/certification within 30 days of start date and updated upon expiration.

Fit Test for N95 respirator within 365 days of start date and updated annually.
Criminal background checks including MGB Criminal Background Check within 45 days prior to start date, Massachusetts CORI within 30 days prior to start date, National Sex Offender Check within 30 days of start and updated annually, and Exclusion Checks within 30 days of start and annually.
Must provide 2 recent references, including at least 1 supervisor or above in skillset.
Must submit MGB Resume including past 7 years of work experience with city and state.
Completion of MGB Skills Checklist and Candidate Submission Coversheet required for submission.
Respirator Medical Clearance Attestation required if fit test is required.
Schedule Information
Schedule depends on caseload, approximately one hour per day.

POSITION SUMMARY:
Reporting to the Rehabilitation Therapies Supervisor/Manager, the OutpatientPhysicalTherapist conducts comprehensive assessment of outpatient clients with physical and functional deficits, planning and implementing individual/group physical therapy treatments to promote mobility, reduce pain, restore function, prevent permanent disability and restore independence, using various treatment techniques. Collaborates with other disciplines to arrange for appropriate follow-up care and facilitates a successful return to home and work environments. Provides student training according to departmental guidelines and participates in continuous education.
JOB RESPONSIBILITIES
Conducts physical therapy evaluation procedures according to professional and departmental guidelines.
Responds to all consults in assigned area within 24 hours; completes evaluation and documentation within 24 hours of consult (IP).
Reviews patient records, gathers necessary information, and obtains a physician's order prior to assessing the patient.
Selects evaluation procedures that reflect understanding of the patient's diagnosis, abilities, and limitations. Administers evaluation procedures according to the guidelines of standardized tests (including range of motion, manual muscle testing, mobility, air entry and breadth sounds); adapts evaluation procedures as indicated to accommodate the specific needs of patients.
Interprets evaluation results, develops an integrated diagnostic impression, and makes recommendations. Performs re-evaluations according to departmental policies, change in patient's status or response to treatment.
Educates patient and caregiver regarding evaluation findings and management of care recommendations.
Establishes treatment goals and plans that reflect the needs of the patients while meeting departmental, institutional, and third party reimbursement requirements.
Formulates treatment goals and plans that reflect the patient's unique psychological and functional deficits. Establishes measurable and attainable short and long-term goals in collaboration with patient and/or caregiver.
Modifies goals as warranted by changes in patient's status.
Assesses discharge needs of the patient and caregiver, including but not limited to: insurance issues, equipment needs, follow-up appointments, and appropriate use of community services.
Communicates patient care and discharge needs to team members as needed. Arranges for appropriate follow-up.
Implements comprehensive individual and group treatment. Consistently demonstrates clinical competence.
Establishes treatment priorities based on evaluation results and the specific needs of the patients.
Uses creative treatment approaches and explores alternative solutions to individual patient problems.
Demonstrates clinical problem solving skills with a variety of patient problems and substantiates actions taken.
Progresses patient treatment programs appropriately and terminates them as warranted by patient's progress and condition.
Recommends and/or issues appropriate assistive devices or durable medical equipment, ensuring that it properly fits the patient.
Develops and instructs patient and caregiver in home programs and/or use of recommended equipment; provides verbal and/or written instructions as indicated.
Establishes and maintains a therapeutic relationship with all patients.
Demonstrates ability to engage patients in treatment, provides reinforcement and support; sets limits appropriately. Demonstrates the knowledge and skills necessary to provide care appropriate to the age groups regularly served.
Notifies supervisor/manager when patients in age groups not regularly served are present, and seeks assistance from individuals with the knowledge of the specialized needs of such patient age groups.
Adheres to diagnosis-related precautions. Demonstrates good judgment in handling potentially hazardous situations, seeking assistance as necessary.
Assigns appropriate patients to PhysicalTherapist Assistant and Rehabilitation and/or Co-op Aide. Seeks feedback.
Monitors clinical supplies on assigned floors, requesting replacements in a timely manner.
Complies with departmental, institutional, and accrediting agency policies and standards.
Completes area specific documentation (including letters, re-evaluations) in an accurate thorough and timely manner according to department standards scoring an average of 95% or greater on each indicator.
Issues supplies/adaptive equipment in a cost-effective manner. Justifies issuance of equipment, demonstrates actual use by patient. Looks for alternative solutions.
Completes patient care charges accurately. Meets area specific productivity standards at least 95% of the time.
Identifies problems and makes adjustments to meet targeted productivity. Participates in the peer review process. Completes form thoroughly, gives meaningful and constructive feedback, and provides learning experience to peer.
Completes mandatory requirements (including PPD, competencies, and safety/infection control tests, CPR, and license renewal) within given time frame.

Assists in developing and achieving area goals.
Collects data required to complete research projects, clinical outcome studies, and departmental quality improvement projects in compliance with established department standards.
Contributes to the assessment, planning, and implementation of physical therapy programs in collaboration with supervisor and peers.

This part-time position reports to the Associate Director of Brain Health and is responsible for performing clinical assessments and interventions as needed for Home Base patients in the Brain Health Program. The Clinical Social Worker Senior provides support to those who are dedicated to patient and family-centered care that values personal self-determination, are skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. * Formulates biopsychosocial assessment, disposition and treatment plans where appropriate.
* Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessment when violence is suspected Plan discharge from care facility to home or other care facility.
* Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
* Performs psychological record reviews in timely manner for patients in the Brain Health programs, including ComBHaT, 2-day evaluation and Special Operations Forces in the Intensive Clinical Program.
* Documents timely and relevant information in patient electronic medical record and Home Base database
* May provide direct clinical services to Veterans and/or family members, which may include group or individual therapy.
* May provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties May assist with crisis intervention and management.
* Provide coverage for social work responsibilities in Outpatient Clinic as needed.
* For LICSWs: provide clinical supervision to LC-level and/or MSW students as needed
* Prepares and maintains all pertinent records, statistics, and progress notes.

The Behavioral Health Home Hospital (BHH) Research Social Worker/Case Manager will support an innovative care delivery model, Behavioral Health Home Hospital. The program offers a subset of patients an alternative to brick-and-mortar inpatient psychiatric hospital admission, in which patients can receive highly specialized psychiatric-care services in their home.
The BHH Research Social Worker/Case Manager will provide quality, clinical services to patients in various settings in the community.
Patient care is delivered within organizational philosophy, policy and standards of nursing and community health practice.
The responsibilities are performed in accordance with Brigham and Womens Faulkner Hospitals policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
The BHH Research Social Worker/Case Manager will collaborate with other professionals to provide clinical services to patients/families that address psychosocial, environmental, age specific and cultural issues in order to maximize emotional, social and physical well-being and the effective use of health care and community resources.
They utilize clinical social work techniques and theory to provide consultation, assessment, treatment interventions and advocacy for BHH patients and their families.
The BHH Research Social Worker/Case Manager can expect to see patients in person in their homes or space they are residing in during their hospitalization or remotely.
Additionally, the BHH Research Social Worker/Case Manager can expect to have up to 4 patients on service at a time.
The BHH Research Social Worker/Case Manager will be providing hospital/inpatient-level treatment to patients in the community.
This is an ideal role for someone interested in a temporary position and working on a research trial that will help broaden the treatment opportunities for patients with active acute-psychiatric conditions.
This position will begin on 1/5 when the trial begins - the schedule will depend on caseload but anticipate about an hour a day.

* Identifying and developing clinical knowledge and skills through participation in professional organizations, attending continuing education programs, peer review, and other related internal and external resources. Sharing knowledge gained from professional development activities with the department.
* Serving as a clinical instructor in a departmental student externship program upon completing eligibility requirements. Planning learning objectives and arranges related clinical experiences. Monitoring performance, providing guidance and feedback, and preparing and conducting student evaluations.
* May participate in departmental training and staff development programs. May supervise assigned direct reports, including physicaltherapist assistants (PTA) or rehab aides.
Minimum Qualifications

* Bachelor's degree in physical therapy
Experience:
* Minimum of 3 plus years of licensed PT experience required
* Previous Pediatric PT experience highly preferred
