About the Position:

We are looking for individuals to join our team of dedicated and hardworking nurses serving the many residents of our retirement and life communities. In this role, you will assist residents in Assisted Living, Hight Acuity Assisted Living, Memory Care, and occasionally in Independent Living +.
This position is responsible for filling part-time roles and/or filling in for vacations, leaves, and/or vacancies, completing nursing tasks, supervision of resident care and services, ensuring that residents receive proper care and treatment, that their health and safety are protected and promoted and that their rights are respected. Our Assisted Living nurses are responsible for the supervision of the Resident Care Assistants regarding distribution and documentation of medications, treatments and side effects of medications, physical and behavioral assessments of residents, creation and maintenance of Individual Service Plans utilizing staff input, maintaining compliance with DHS 83 & DHS 89, Program statements, and Bethany Lutheran Homes, Inc. policies and procedures.
Requirements:
Must be currently licensed to practice in the State of Wisconsin as either a RegisteredNurse or Licensed Practical Nurse.
Valid driver's license with safe driving record
Must have demonstrated interest in gerontological nursing including rehabilitative and restorative clinical practices, diabetic care, memory care and end of life care
Experience working in assisted living, long-term care and/or community health is beneficial, but not required-We will train the right fit for our team!

Our Wages:
Our starting rate for Licensed Practical Nurses is between $30.00+ per hour, based on your experience and qualifications and our starting wages for RegisteredNurses is between $39.50+ per hour, based on experience and qualifications. We'll review your wage at 6 months - we pay for high performance!

A Patient Logistics RN serves as the central coordinator for complex patient transitions across the healthcare continuum, expertly managing inter-facility transfers while balancing clinical priorities, bed capacity constraints, and transportation logistics. The Patient Logistics RN functions as a critical communication hub between referring physicians, accepting facilities, transport teams, and family members, ensuring that comprehensive clinical information is accurately conveyed to maintain continuity of care. *ESSENTIAL FUNCTIONS*
1. Facilitate and coordinate patient transfers between healthcare facilities, departments, or units
2. Work in a fast-paced, ever changing working environment.
3. Conduct clinical assessments of transfer requests to determine appropriate level of care needed
4. Communicate effectively with physicians, nurses, and staff at both sending and receiving facilities
5. Manages multi-phone call activity to minimize any delays experienced by customers
6. Document all transfer information accurately in electronic health records and transfer systems

In this role, the successful candidate will be responsible for the oversite and provision of medication management and treatment services, physical and behavioral assessments of the residents, actively maintaining Individual Service Plans using staff input, and maintaining compliance with CBRF regulations (DHS 83) and Program Statements.
What You'll Do
Become familiar with each resident's care needs, work with the residents and other care providers related to medical concerns, new admission and routine assessments as well as discharge planning.
This nurse will educate and supervise Resident Care staff in medication administration and resident care, delegating tasks as appropriate.
Collaborate in a supportive environment with our on-call Triage Nurse Team
