Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
Facilitate team conferences weekly and coordinate all treatment plan modifications.
Complete case management addendums and all required documentation.
Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.
Understand commercial contract levels, exclusions, payor requirements, and recertification needs.
Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
Perform assessment of goals and complete case management addendum within 48 hours of admission.
Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
Schedule and facilitate family conferences as needed.
Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate/timely referrals, including documentation to post discharge providers/physician
Ensure accuracy of discharge and payor-related information in the patient record
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

Role (Overview of the Position)
The Quail Care Specialist Farm Hand is responsible for the daily care, cleaning, and maintenance of the Steadfast Farms Coturnix Quail Flock, ensuring optimal conditions for egg production, hatching, and overall bird health. This role requires close attention to detail, accurate record-keeping, and adherence to strict biosecurity protocols. The ideal candidate is dependable, self-motivated, and able to work independently while maintaining open communication with farm management, bringing not just observed concerns, but proactive solutions, in alignment with our team's solution mindset. This position starts in the quail hatchery but may expand to other areas of the farm as the employee demonstrates proficiency, reliability, and dedication.
Responsibilities (Tasks Associated with the Role)
Daily cleaning and maintenance of the quail hatchery.
Accurately record data on egg counts, weights, hatching rates, and incubation progress on a daily and weekly basis.
Plan and schedule incubation cycles to ensure maximum production.
Communicate with management to schedule and plan harvesting cycles.
Monitor bird health and behavior, reporting any concerns immediately.
Track overall flock mortality and retirement schedules of breeding flock
Asist CT DOAG during quarterly NPIP testing days
Submit suggestions to management to improve any process or task that results in better efficiency, cost savings, and bird health
Follow biosecurity protocols to prevent disease, including proper handling, sanitation, wearing PPE, and restricted access to sensitive areas.
Maintain, track, and refill feed and water systems as needed.
Identify and relay supply needs (bedding, feed, equipment, etc.) to management in a timely manner.
Lift and move feed bins, bedding, and equipment weighing up to 200 lbs. with two-person assistance.
Work independently while maintaining a high level of accuracy and accountability.
Potential opportunity to assist in additional farm operations as needed, including poultry husbandry and general farm maintenance.
Comfortable with or experience with driving a 38 HP Hydrostatic Tractor with loader on various terrains and under load.
Ability to travel and maneuver around people, structures, and equipment without causing damage or injury.

Position Summary:
CSCU is developing a pool of applicants for potential part-time teaching positions in the Continuing Education and Workforce Development within the non-credit division at the College. The Non-Credit Lecturer must meet all teaching expectations, as outlined by the Director of Workforce Development.
Minimum Qualifications:
Bachelor's Degree required. Alternatively, related Industry Certifications will be considered.
One (1) or more years' teaching experience including using online teaching platforms including but not limited to Blackboard, WebEx and Microsoft Teams.
Related work experience.

Candidates must possess proven ability to effectively work with culturally, linguistically, and ethnically diverse faculty, staff, and students. Experience with reflective, interactive, culturally responsive pedagogical teaching techniques. They are expected to have excellent oral and written communication skills along with strong Information technology literacy skills such as Microsoft Office (Word, Excel, Outlook, Teams etc.)
Preferred Qualifications:
Prior community college teaching experience.
Practical experience in the related Workforce Education industry for which you are applying.
Demonstrate knowledge of the industry trends and workforce needs.

Healthy Communities is CHC's Public Health AmeriCorps program designed to create cohesion in vulnerable communities and to promote healthcare and support services through outreach, awareness, linkage to care and support. Healthy Communities also serves to identify the unique and integral needs of our communities and neighborhoods and to establish strategic plans to address them. Service delivery will directly address the public health crisis in vulnerable communities and make healthcare and support services accessible through flexible delivery of services. AmeriCorps members will conduct community outreach, education, patient engagement, and link patients to CHCI care and referrals to community based assistance. Additionally, AmeriCorps members will participate in statewide and local activities which promote awareness of health and health services.
This is an AmeriCorps Member role - Precision Medicine Peer Educator - that supports the All of Research Program (AoURP) throughout all CHC sites for the East and West regions, but primarily serving at Meriden site. The _All of Us_ Research Program (AoURP) is an NIH funded national initiative that seeks to enroll one million people living in the United States in a 10-year longitudinal study and takes into account an individual's variability in genes, environment, and lifestyle. The goal of the program is to help researchers learn more about what affects people's health. The program will give new information and tools to researchers. Their discoveries may lead to more personalized care and treatments. The Community Health Center, Inc. was awarded a grant to be part of the Health Provider Organizations that enroll patients into the program.
Terms & Program Benefits:
+ 9-month service term from December 1, 2025 thru August 28, 2026,
+ Serve a minimum of 31 hours a week for a total of 1200 hours during service year
+ $16,590 stipend over the course of one year, paid bi-weekly
+ An education award of $5,176.50 at the end of successful service term completion

As a Precision Medicine Peer Educator for the Community Health Center, Inc., you will:
+ Conduct monthly patient health education workshops; monitor the completion of pre-post surveys by workshop participants; and complete data entry for workshops and surveys.
+ Track and document all completed patient engagement activities in patient's electronic health record.
+ Developing new curriculum for a health literacy program for kids at the Hartford Public Library
+ Coordinate and facilitate the health literacy program at 3 Hartford Public Library branches and 1 possible community based organization (1-2 per week)
+ Evaluate activities and make adjustments as needed.
+ Develop social activities for adult and kid participants such as arts and crafts projects surrounding precision medicine and the All of Us research program.
+ Research and create content for patient educations sessions and social activities.
+ Create promotional content for precision medicine as well as patient education materials/pamphlets (digital and print).
+ Coordinate, conduct, and participate in community outreach and tabling events.
+ Develop community partners (i.e. Boys and Girls Club, YMCAs, etc) to educate local communities on precision medicine and the All of Us research Program and collaborate with them on events to host.
+ Participate in bi-weekly All of Us team meetings to provide updates on engagement strategies and success of events, activities and workshops.
Qualifications:
+ High school or equivalent required
+ Proficiency in Microsoft office and internet-related applications
+ Excellent time management and organizational skills
+ Excellent oral and written skills
+ Demonstrated ability to problem solve and remain calm during a crisis
+ Successful clearance of all required criminal history checks (NSCHC)
+ Able to travel between CHC sites and in state
+ Ability to serve in the evenings or weekends, occasionally, due to community events and educational sessions
Preferred:
+ Associate's degree in public health, social work related field
