**ESSENTIAL DUTIES**
+ Perform and schedule general housekeeping and cleanup work.

**MINIMUM QUALIFICATIONS**
+ Associate's degree in Power or Process Plant Technology; or
+ Certificate of Completion in Power or Process Plant Technology; or
+ High school diploma or equivalent (GED or HSED) and 2 years of previous related full-time operating experience.
+ A valid driver's license.
+ The incumbent will work 12-hour rotating shifts.
**PHYSICAL AND ENVIRONMENTAL DEMANDS**
This position requires frequent walking, standing, stooping/bending, climbing stairs and ladders, kneeling, lifting/carrying up to 60 pounds, reaching above and below shoulder level; and occasional sitting, crawling, pushing/pulling up to 90 pounds, grip strength of 70 pounds, and use of a keyboard/computer and test equipment. Approximately 20% of the incumbent's shift is spent operating equipment. Duties may include opening and closing valves and railcar hookups without assistance. Occasionally requires working in confined spaces and high climbing areas on grating, ladders, scaffolding at heights up to 200 feet, which requires the use of safety equipment, including a harness, in accordance with ANSI Z359 standards. Over 50% of the incumbent's shift is spent outdoors. The incumbent will be exposed to heat, cold, dirt/dust, fumes, chemicals, and noise. Required to wear dust masks, respirators, and full-face Scott Air Paks when necessary. This position requires good color vision, finger dexterity, hearing, and the ability to speak. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. Assists the resident in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment and also provides range of motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident. Assists residents with meals and snacks, provides water and supplements as appropriate.
The NA aids providers and nursing staff members with procedures, if needed. Documents resident interactions as needed and the outcomes related to resident care, behavior, activity, and dietary. Communicates resident's changing condition and care related concerns/responses to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Provides assistance and documentation with wellness and recreational programs and encourages residents to participate. May participate in departmental/organization committees and task forces as needed.
May be responsible for completing tasks related to the Restorative Nursing program (including documentation, scheduling and leading restorative programs.
Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Two years' clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications.
May be required to complete the Restorative Nursing training within 6 months.
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor
vehicle report and proof of valid driver's license. Also must be medically qualified to perform the essential driving functions of
this position as per our Sanford Driving Policy per the leaders request.

Job Summary

Has knowledge of and utilizes appropriate age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care relating to the overall health care needs of neonatal, pediatric, adolescent, adult and/or geriatric patients/residents. Interviews potential residents and assesses residents in compliance with regulations, policy and procedure. Uses assessment information for admission and continued stay in the hospice community. Establishes a service plan for each resident and evaluates on an on-going basis. Ability to communicate with patients/residents, family members and others on the health care team.

Responsibilities of the RN Health Care Facility Surveyor:
The position is 100% remote with up to 75% travel. The Surveyor will serve as a team member or team leader on various types of surveys (i.e. re-certification, comparative, complaint investigation, and revisits) for long-term care and non-long-term care surveys, which can include; ambulatory surgical centers (ASC), Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICFs/IID), end-stage renal disease (ESRD), Psychiatric Residential Treatment Facility (PRTF), hospital, critical access hospital, and hospice facilities. Surveyors travel to healthcare facilities nationally conducting surveys to assess compliance with requirements and regulations guiding the quality of care for residents/customers of the facilities. The responsibility of the Surveyor is to apply approved survey protocols for conducting on-site surveys. Specific activities include the following:
Conduct on-site surveys of Medicare-Medicaid certified health care facilities to determine compliance with Federal regulations.
Surveys require preparations such as off-site preparation, tour of the facility, observation of care, record review, interviews of staff and families, etc. Determine if care, treatment, and services are consistent with Federal requirements.
Evaluate many aspects of the healthcare facility system from the quality of care to the consumers' rights and physical plant.
Communicate the non-compliance issues with key personnel throughout the site review and during the exit conference to assure an understanding of the deficiencies cited.
Determine if a plan of correction is acceptable.
Conduct follow-up visits and monitoring surveys to ascertain if facilities' plans of correction to resolve the deficient practice have been implemented.
Training: Assist with facility and bureau training. Participate in work groups as needed
Requirements
Must have an Associate or Bachelor's degree in nursing
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
Must have At least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
Must maintain current licensure to practice as a Registered Nurse (RN).
Demonstrated a history of independent decision-making skills to direct and effectively manage the survey process.
Ability to set priorities independently and collectively in performing survey tasks.
Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
Ability to travel up to 75% of the time on a regular basis is required.
