Evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.

Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver in accordance with the established guidelines and standards of nursing care. Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. The CRC also consists of nurses who take physician office referrals for patients not currently hospitalized. There is someone on-call 24/7 for the CRC to facilitate home care coordination for patients in emergency rooms, etc.

Responsibilities
Screens referrals to ensure patients meet agency admission criteria and comply with state and federal regulations.Verifies insurance benefits, obtains pre-authorizations as necessary, assists billing department, and interprets company services, admission criteria and medical policies to referral sources and individuals seeking information.Tracks current clients that are either hospitalized, placed in a short-term skilled facility or in a rehabilitation facility.Communicates and coordinates discharge planning and educates internal and external customers about Home Care Services and/or outpatient services.

Overview
BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families. We provide a multi-disciplinary team of healthcare professionals and volunteers, specially trained in symptom management, pain control, counseling and bereavement services for the dying. Our Hospice services include alternative therapies such as music, art and massage therapy. Our Hospice programs provide palliative care by helping patients manage their pain and symptoms while living their lives with daily peace and dignity.

Missouri Baptist Sullivan Hospital is a designated critical access hospital committed to providing quality health care service to residents in Crawford, Franklin and Washington counties.

Acute Care Medicine has patients with a full range of clinical issues-from significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma and a multitude of other conditions and diseases. Due to the patient acuity on the nursing division, we provide advanced training to bring new nurses to the highest level of functioning. Each unit is lead by a clinical manager and lead charge nurse. The unit is supported by an advanced practice nurse. Additionally each unit is assigned a case manager, social worker, physical therapist, dietitian, and pharmacist. This multidisciplinary team works in conjunction with the physicians, nurses and patient care technicians to provide quality patient care.

Maintains timely, legible, clear, concise, and well-organized records of services provided. Documents the results of intakes, screenings, and assessments; prepares and updates treatment plans; records notes of consumer participation and progress in therapeutic services. Prepares reports, progress notes, discharge summaries and other documents needed to satisfy professional and payer requirements, by using collaborative documentation.
Must be able to facilitate and manage crisis situations.
Must be able to help cover crisis access services, when needed.
Take weights and blood pressures and record results in client records.
Administer the AIMS to clients on regularly scheduled basis under physician supervision.
Manage the scheduling of clients to see the physician for medication services and communicate failed appointments to the proper community support worker and/or supervisor.
Provide education to clients regarding their medication, its benefits and side effects. Document educational sessions.
Collect and present them to the physician.
Follow-up with the laboratories regarding client compliance with prescribed laboratory tests.

Monitor sample medication supply.
