Program Overview:
Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program.
Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:
The Family Intervention Specialist:
Carries a caseload of 4 to 6 families
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)
Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan
Provides on-call crisis support to the youth and family (schedules vary by location)
Completes accurate and timely documentation in an electronic medical record system (EMR)

FUNCTION:
Machine parts of various materials to meet the customer's requirements on a multi-axis axis-milling/turning machines.

DUTIES AND RESPONSIBILITIES:
Works from routed instructions, manufacturing process plan, blueprint or verbal instructions.
Works under close direction of supervision.
Uses measuring instruments (scale, dial calipers, thread gages, indicators and micrometers).
Tram in machine head when applicable.
Make set-ups using vice, angle plate, v-block, rotary table or super spacer.
Mill, drill, tap and ream to produce machined parts of basic geometric shapes.
Deburr parts.
Completes required documentation.
Understanding of multi-axis CNC machining processes and machine functions.
Assumes other responsibilities as assigned.
DESIRABLE KNOWLEDGE/EDUCATION:
High school diploma or GED required.
Must be a US citizen.
DESIRABLE EXPERIENCE/SKILL:
Two-year high school vocational training in machine trades or equivalent experience in a CNC shop.
Mechanical and spatial aptitude.
Ability to amend G&M codes on the machine.
Ability to effectively interpret blueprints.
Ability to do math (addition, subtraction, multiplication, division, conversion to and from decimal points, geometry).

Coordination of Care

Anticipates patient needs based on disease processes and coordinates care accordingly.

Reviews outstanding orders during pre-visit review and obtains critical data from referral sources:
Obtains lab and diagnostic tests.
Obtains specialty office consult reports on all referred patients.
Works overdue orders “work list” and identifies gaps in patient care. Report these gaps to the appropriate care team for follow-up.
Proactively contacts populations of patients identified with specific health requirements for care based on their conditions, including monitoring of patients with missed appointments.
Contacts patients and coordinates care when follow-up is needed.
Assesses the patient's readiness for change and works with care team to provide appropriate referral/follow-up.
Appropriately facilitates patient information among care team members to assure all patient care issues are addressed.
Triage Nurse Communication
Maintains a positive attitude and supports a teamwork approach with co-workers, physicians, and all members of the care team.
