Practice Coordinator Responsibilities:
Manage the administrative functions for a team of 500-600 patients and 3-5 physicians, nurse practitioners and physician's assistants
Ensure the care needs of a complex, homebound patient population are met by providing exceptional customer service, coordinating timely visits, and demonstrate superior communication across care teams
Build trusted relationships with internal and external stakeholders including physicians, partner community leadership, patients and families, and Bloom senior leadership
Own team performance measures that translate to exceptional patient outcomes, including provider productivity, patient satisfaction, and timely answering of calls
Demonstrate superior time management and prioritization skills by balancing competing demands including referrals, visit, records and supply requests
Ensure patients are seen in a timely fashion as requested by providers, nurses, and incoming requests
Build efficient provider schedules to ensure all patients are seen within most effective driving route
Other duties:
Creating provider schedules
Answering incoming phone calls
Confirming appointments
Scheduling urgent appointments
Processing incoming electronic faxes
Processing provider requests for patient records
Ordering DME for patients as requested by providers
Ordering oxygen for patients as requested by providers
Sending referrals to specialists, home health or hospice as requested by providers
Demonstrating superior communication across care team
Practice Coordinator Knowledge/Skills/Abilities:
College degree required

Position Summary
The Clinical Support Specialist plays a key role in supporting the clinical operations of Bloom Healthcare's primary care teams. This position ensures efficient communication, accurate documentation, and timely completion of clinical administrative tasks that directly impact patient care. Working closely with Bloom's support staff, and providers, the Clinical Support Specialist helps maintain smooth day-to-day clinical workflows and supports continuity of care for our patients.
Key Responsibilities
Manage and process medication refill requests in accordance with established protocols.
Coordinate prior authorizations for medications, procedures, and services.
Request, receive, and upload medical records and documents from external facilities.
Update immunization records, allergies, and health maintenance data in patient charts.
Follow up on outstanding labs, imaging orders, and specialist consults to ensure results are completed and available.
Support accurate and timely charting for providers and nursing staff.
Communicate with pharmacies, and external offices to gather or relay request clinical information.
Collaborate with the clinical support and nursing teams to streamline workflows and improve efficiency.
Maintain compliance with HIPAA and organizational policies.
Qualifications
Education & Licensure: Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), or Medical Assistant (MA) required.
Experience: Minimum 1-2 years of clinical or administrative healthcare experience preferred (primary care or home-based care experience a plus).

Job Summary:
Bloom providers serve as the primary care provider for patients and see patients based on medical necessity. In addition to comprehensive primary care and care management, Bloom can also provide add-on services such as on-demand care, offering diagnosis of acute conditions and immediate clinical interventions, wound care, hospice, real-time blood draws, etc.
Bloom treats patients longitudinally through a holistic primary care model. Additionally, treatment also encompasses screening and management of social determinants of health, thorough patient and caregiver education, medication reconciliation, fall risk assessments, home food assessment, advanced care planning, etc.
Bloom's data stack is integrated with the Colorado health information exchange to track and monitor patient hospitalizations. Our providers strives to see patients post-discharge within 72 hours to ensure patient stabilization and understanding of any new medications or instructions.
Responsibilities:
Help elderly patients to age in place
Deliver comprehensive primary care at home
Assess and address factors such as socioeconomic status, housing stability, and cultural influences to optimize patient outcomes.
Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners

Practice Coordinator Responsibilities:
Manage the administrative functions for a team of 500-600 patients and 3-5 physicians, nurse practitioners and physician's assistants
Ensure the care needs of a complex, homebound patient population are met by providing exceptional customer service, coordinating timely visits, and demonstrate superior communication across care teams
Build trusted relationships with internal and external stakeholders including physicians, partner community leadership, patients and families, and Bloom senior leadership
Own team performance measures that translate to exceptional patient outcomes, including provider productivity, patient satisfaction, and timely answering of calls
Demonstrate superior time management and prioritization skills by balancing competing demands including referrals, visit, records and supply requests

We are currently looking for Nurse Practitioners to join our Team!
Responsibilities:
Deliver comprehensive primary care at home visits to 10 patients per day
Provide data-driven, condition-specific specialty visits to patients as needed
Manage the health outcomes of an empaneled patient population
Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners
Engage with and appropriately utilize population health data to drive superior patient outcomes
Attend ongoing educational opportunities
Commute to assigned assisted living communities to provide care at home
Nurse Practitioner Benefits:
Scheduled Days: Monday - Friday (5 days)
$10,000-$15,000 annual quality incentive opportunity

Volunteers play a crucial role in enriching the lives of our patients by offering valuable companionship and specialized therapies, including music therapy, pet therapy, and other alternative therapies. Volunteer Bereavement Counselors are an essential part of the Bloom care team in supporting patients' families, friends, and other impacted individuals after the patient's death. Volunteer Bereavement Counselors provide individual grief counseling for adults, adolescents, and children.
Responsibilities:
Provide in person and virtual counseling for deceased patients' family, friends, and other impacted individuals
May assist in providing specialized therapies such as music therapy, art therapy, pet therapy, and other alternative therapies
Work closely with the Bereavement Coordinator and hospice care team to ensure bereavement needs are met
Complete administrative and documentation tasks timely and thoroughly
Adhere to all health and safety guidelines, as well as patient privacy policies
Qualifications:
Possess applicable license (LSW, LCSW, LPC, NCC, LMFT, chaplain with completion of two units of CPE) OR have a master's degree in social work, counseling, or a related field with two years of experience as a grief counselor.
Demonstrated empathy, compassion, and excellent interpersonal skills
Availability to commit to a regular volunteer schedule for this unpaid position
Must pass a background check and comply with all Bloom Healthcare policies and procedures
