Thursday - Sunday Schedule, Home Health Visits
As a Weekend Home Health Registered Nurse, you will:
Provide admission, case management, and follow-up skilled nursing visits for home health patients on a Weekend Schedule (Thursday - Sunday fulltime).
Administer on-going care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager.
Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide hands-on care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
Coordinate appropriate care, encompassing various healthcare personnel (such as Physical Therapists, Occupational Therapists, Home Health Aides and external providers).
Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care. Monitor the appropriate completion of documentation by home health aides/personal care workers as part of the supervisory/leadership responsibility.
Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.

Minimum of one year nursing experience in Home Health or Hospice
Strong OASIS Proficiency
Home Care Home Base (HCHB) experience
Current and unrestricted Registered Nurse licensure

Strong organizational and communication skills
Valid driver's license, auto insurance and reliable transportation.

This role is heavily focused on leadership and management of your team. You will take care of hiring, onboarding, coaching, evaluating, and keeping your team focused and on track. Your team is experienced. Allow them to make decisions. They work together to accomplish the daily tasks and full-fill the needs of the customer. A key focus will be keeping your bench stocked and replacing people as they retire.
Daily, you will oversee inventory and ordering of product. Dispatch and scheduling of people and equipment. You will work with other staff to share resources as needed.

The Medical A floor at Mercy Jefferson is a fast-paced 30-bed unit that cares for patients of all ages. Many of the patients are chronically ill and require multidiscipline treatment plans and goals. As part of the health care team, you will help educate patients on a wide variety of topics ranging from procedural/surgical expectations to what medications patients will be taking once discharged. Potential health care workers who are compassionate, team-orientated, hard-working, flexible, enjoy caring for people from all walks of life, will find working on this unit very rewarding while expanding their clinical knowledge base and enhancing their skills.
Qualifications:
Required Education: Practical Nursing Degree from an accredited school
License: current LPN license in our state or can be obtained within 90 days
Certifications: Basic Life Support certification through the American Heart Association, IV Certification within 1 year.
