Department/Position Overview:
Sentara Leigh Hospital Intensive Care Unit, AACN Gold Beacon Award recipient, is comprised of 20 licensed critical care beds and has an average daily census of 18 patients. Staffing ratios vary and can be 1:1 to 1:3 with 1:2 being the usual assignment.
The ICU RN's are required to have a wide array of knowledge, skill and ability to competently and safely provide critical care to our patients. Frequent ongoing education, new skill acquisition and skill validation are provided and supported by an experienced ICU CNS for new and experienced ICU nurses. Newly hired RN's will have between 2-14 weeks of orientation depending on prior experience. Our preceptors are purposely selected for their teaching ability and formally trained to meet the training needs of new staff. Specific attention is provided for new graduate ICU RN's as they participate in our Vizient AACN Nurse Residency Program and AACN's essentials of Critical Care Orientation online education. Experienced RN's are encouraged and supported with educational and financial assistance to achieve critical care certification.
ICU staff are supported by an interdisciplinary care team which in part includes unit based respiratory, physical, speech and occupational therapists. Interpreter, nutrition, palliative, chaplaincy and ethical services are readily available as well. Our ICU has onsite and eICU pulmonary critical care intensivists and their team of physician extenders 24/7. Physician lead, nurse driven interdisciplinary rounds are held daily for continuity of care and to address nursing's concerns or questions. These rounds are made educational and even fun at times by our very engaged intensivist MD's.

RAYUS Radiology is looking for a Scheduling Specialist to join our team. We are challenging the status quo by shining light on radiology and making it a critical first step in diagnosis and proper treatment. Come join us and shine brighter together! As a Scheduling Specialist, you will be responsible for providing services to patients and referring professionals by answering phones, managing faxes and scheduling appointments. This is a full-time position working 9:00AM - 5:30PM CST Mon-Fri, Rotating Saturday 7am-1pm CST.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
(85%) Scheduling Activities
Answers phones and handles calls in a professional and timely manner
Maintains positive interactions at all times with patients, referring offices and team members
Schedules patient examinations according to existing company policy
Ensures all appropriate personal, financial and insurance information is obtained and recorded accurately
Ensures all patient data is entered into information systems completely and accurately
Ensures patients are advised of financial responsibilities, appropriate clothing, preparation kits, transportation and/or eating prior to appointment
Communicates to technologists any scheduling changes in order to ensure highest level of patient satisfaction
Maintains an up-to-date and accurate database on all current and potential referring physicians
Handles overflow calls for other centers within market to ensure uninterrupted exam scheduling for referring offices
Provides back up coverage for front office team members as requested by supervisor (i.e., rest breaks, meal breaks, vacations and sick leave)
Fields 1-800 number calls and routes to appropriate department or associate (St. Louis Park only
(10%) Insurance Activities
Pre-certifies all exams with patient's insurance company as required
Verifies insurance for same day add-ons
Uses knowledge of insurance carriers (example Medicare) and procedures that require waivers to obtain authorization if needed prior to appointment
(5%) Other Tasks and Projects as Assigned

This is a full-time position, working 11:30am to 8pm.

Patient Care Coordinator LPN (100% Full Time, Days)- Family Medicine Woodlands Clinic

The Patient Care Coordinator-LPN works collaboratively in the ambulatory setting with providers and all other members health care team including diabetic educators, social workers and payer-specific and specialty specific nurse navigators to ensure the highest quality care for all patients regardless of payer type. This position is an integral part of the Comprehensive Primary Care Plus (CPC+) program, which serves as a form of an advanced Patient Centered Medical Home. This position will support Family Medicine at the Woodlands Clinic.
Duties/Responsibilities
Maintain accurate and risk-stratified patient panels for each provider or care team serving as office extension to the current population health team.
Serve as an in-office coordinator to align the needs of the patient with appropriate member of the care team: physicians, CNPs, nurses, MAs, nutritionists, pharmacy, diabetes educators, social workers, etc.
Identify support systems and community resources for all patients regardless of payer when appropriate.
Create care plans for all high-risk patients regardless of payer type.
Identify and close care gaps by scheduling needed appointments, creating actionable telephone notes, and utilizing standing orders across multiple diagnostic categories.
Review notifications of possible non-adherence and care gaps from pharmacies/insurers and create actionable telephone notes to send to the provider when appropriate.
Identify missed and overdue office visit appointments, including wellness visits, and then coordinate the appropriate scheduling of these appointments.
Meet with the patient in office at the time of provider appointment when appropriate and when requested by provider.
Perform pre-visit planning for appropriate patients and update electronic medical record as needed.
Ensure patients discharged from target hospitals are contacted within two (2) business days from time of discharge per CPC+ guidelines.
Minimum/Required Qualifications
Graduate of an Accredited Program in Practical Nursing
LPN-Licensed Practical Nurse in the state of Ohio
Preferred Qualifications
1-2 year's experience in a similar capacity/setting

This position guides prospective patients through the admissions process by taking initial calls, completing screenings, and scheduling outpatient evaluations.
This position is remote, but the ideal candidate must live in Pittsburgh, PA region or surrounding counties.
This position receives frequent supervision and instruction. Note: The position is Part-time, but we will require some evening hours.

Responsibilities
Completes non-clinical evaluation components consisting of and not limited to: answering initial calls and completing the pre-screening process, coordinating and scheduling outpatient evaluations.

Knowledge Skills and Abilities
