Position Overview:
We are seeking a highly motivated and detail-oriented individual to join our team as a Part-Time Clinical Documentation Improvement (CDI) Specialist. The primary responsibility of this role is to conduct thorough patient chart reviews to identify opportunities for providers to capture risk adjustment diagnostic codes accurately. The successful candidate will play a crucial role in ensuring proper documentation to support appropriate and accurate disease capture and documentation by Bluestone providers.

Schedule: Part-time (32 hours per week), weekdays during regular business hours, no evenings, weekends or holidays.
Location: This remote role MUST be located in one of the Bluestone Markets (Minnesota, Wisconsin or Florida).
Salary: $29.00 - $37.00 per hour. Salary will be commensurate with experience.
Responsibilities:
Perform comprehensive reviews of patient charts to identify gaps in documentation and opportunities for risk adjustment coding improvement.
Collaborate with Bluestone providers and other clinical staff to educate them on the importance of accurate documentation for risk adjustment purposes.
Provide ongoing training and support to Bluestone providers to enhance their understanding of risk adjustment coding guidelines and documentation requirements.
Offer guidance and feedback to providers to facilitate improved documentation practices and ensure compliance with coding standards.
Act as a resource for clinical staff regarding coding inquiries and documentation best practices.
Maintain accurate records of chart reviews, coding opportunities identified, and outcomes of provider education efforts.
Stay current with updates and changes in risk adjustment coding guidelines and regulations.
Assist in the development and implementation of CDI initiatives to optimize coding accuracy and capture disease burden among Bluestone's patient population
Qualifications:Education/Certification/Experience
Bachelor's degree in Health Information Management, Nursing, or related field.
Certified Risk Adjustment Coder (CRC) certification, Risk Adjustment Coding (RAC) or related risk certification required
Minimum of 2 years of experience in healthcare coding, with a focus on Hierarchical Condition Category (HCC) coding and risk adjustment.
Knowledge/Skills/Abilities
Proficiency in reviewing and analyzing medical records for documentation deficiencies and coding opportunities.
Strong understanding of ICD-10-CM coding guidelines, particularly as they relate to risk adjustment.
Excellent communication skills with the ability to effectively interact with Bluestone providers and clinical staff.
Demonstrated experience in providing education and training to Bluestone professionals.
Detail-oriented with strong analytical and problem-solving skills.
Ability to work independently and manage time effectively in a remote or part-time role.
Knowledge of healthcare compliance regulations and privacy laws.

Position Overview:
Join our team as a Care Coordinator where you will work with the senior population managing Department of Human Services (DHS) and Center for Medicare/Medicaid Services (CMS) required activities for Minnesota Senior Health Options (MSHO/MSC+) members living in the community and assisted living facilities. Responsibilities:

We are seeking a professional, self-motivated Clinical Assistant (LPN or CMA) to join our team that delivers primary care services to patients who reside in senior living communities. A Clinical Assistant is an essential care team member in our unique care model. Working alongside our healthcare providers, a Clinical Assistant's duties include pre-visit planning and chart preparation, conducting patient screenings, reconciling medications, maintaining accurate patient charts, and performing minor procedures under provider supervision.
The Clinical Assistant provides collaborative nursing support working alongside a provider team traveling to senior living communities.
Schedule:
Full-Time Position with Day Shift Hours-No evenings, weekends, or holidays
Monday - Thursday: 8:00 AM - 4:30 PM (In the field, patient-facing)
Friday: 8:00 AM - 3:00 PM (Typically a Work-From-Home/Administrative Day)
Location: Fridley, MN & Surrounding Areas
Salary: $23.00 - $29.00 per hour. Salary will be commensurate with experience.
Additional compensation: $1,000 sign-on bonus!
Responsibilities:
Foster positive relationships with patients, families, and community staff members.
Pre-visit planning such as chart prepping, reviewing patient records, and following up on previous orders since the last visit.
Perform screenings for depression and cognitive function as part of Annual Wellness Visits.
Ensure medical records are updated accurately during and between visits.
Medication reconciliation.
Setting up e-prescriptions and orders under provider supervision.
Performing minor procedures under provider supervision, such as ear lavages, swabbing, and immunizations.
Monitor and respond to portal messages.
Qualifications:
Education/Certification/Experience
Active Licensed Practical Nurse (LPN) required or
Active Medical Assistant certification (CMA, RMA, or CCMA)
Valid driver's license required
Knowledge/Skills/Abilities
Knowledge of medical terminology and understanding of patient care notes
Ability to assist providers and facilitate an efficient, organized clinical flow
Ability to problem solve and work well under pressure
Detail oriented and accurate
Ability to deal with change and ambiguity
Excellent interpersonal and customer service skills
Excellent written and verbal communication skills
Comfortable navigating electronic medical record systems and working with patient care notes
Computer proficient, 45+ WPM typing
Demonstrated compatibility with Bluestone's mission and operating philosophies
Demonstrated ability to read, write, speak, and understand the English language

Qualifications:
Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
Board Certified/Board Prepared through AANP, ANCC, or NCCPA
Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
Ability to obtain and /or maintain DEA certification for the State of Minnesota
Proven ability to communicate effectively with strong verbal skills
Excellent interpersonal and customer service skills
Must have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required
Preferred Qualifications:
2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
