Qualifications: Education/Certification/Experience Bachelor's degree in data science, computer science, healthcare administration, or a related field. Master's degree preferred. 3-5 years of experience in data analytics, with significant experience in value-based care analytics, ideally within a healthcare or primary care setting. Knowledge/Skills/Abilities Proficiency with SQL for querying and managing large datasets. Familiarity with LookerML for building data models is a plus. Job Description Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Eden Prairie, MN. Our unique, robust model of care goes beyond primary care services - our multidisciplinary care teams collaborate with patients, their families and other healthcare providers to deliver care that is preventative, proactive and tailored to their unique needs. Under our model of care, Bluestone patients experienced 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to patients with similar conditions and complexities over the same time period. Position Overview: Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
