Also assists in securing appropriate services including vocational support, medical support, finding community supports and services, assisting with medication and psychiatric follow up and providing recovery services. Services are designed to promote independent living and mental health stability. REQUIREMENTS: Bachelor's degree is required. Human Services related field preferred. Knowledge of counseling techniques and mental illness. Education must be from an accredited school, college, or university. Must pass a level II background check and drug screen. Valid Florida driver's license, own transportation vehicle and vehicle insurance required, with vehicle work coverage insurance strongly encouraged. Minimum one year of experience working with the severely mentally ill, community services and resources experience with property management/tenant-landlord issues preferred. PRIMARY JOB FUNCTIONS: Work directly with consumers to perform assessments and screenings, to monitor and document mental health status, medication compliance, progress and performance toward independent living, provide, crisis management services and plan, conduct and document consumer sessions in accordance with policies and procedures. Also, provide individual counseling and obtain financial resources, as needed. Work with consumer to develop a comprehensive service plan to stabilize their housing and implement service plans for each assigned consumer and update plans every six months. Document all consumer services provided within a forty-eight hour period and maintain productivity standards to level set by the department Provide direct and/or indirect assistance, guidance and instruction in tasks related to the maintenance of independent living skills. Document required reports and basic living skills service provision in accordance with policies and procedures and provide one-to-one counseling and monitor consumer progress. Promote safe and clean residences, ensuring needed repairs are reported and completed, locate potential and suitable community housing and negotiate with landlords to assure resolution of problems and disputes. Transport consumers in own vehicle or agency vehicle, when available. Maintain required insurance coverage on vehicle and notify insurance company vehicle will be used for work. Maintain regular communication and coordinate consumer care with team members and other service providers and participate in group meetings/professional staffing to ensure appropriate delivery of services. Assist consumers with completion of paperwork for available community resources (i.e. food stamps, social security benefits, vocational rehabilitation, HUD, etc.) and develop supports and/or services for consumers that promote more effective independent living. Maintain necessary tenant files, records and other documentation in accordance with internal and/or external requirements. Prepares written narratives of service plans and interventions. Assist consumers with resolution of disputes/problems, ensuring proper procedures are employed regarding the handling of grievances or complaints. Make referrals to providers as appropriate and communicate program services to internal and external case management and DCF personnel. Perform monthly apartment inspections and complete documentation if consumer lives in a Boley-owned apartment. Working within our Supported Housing and Residential Services department, the Case Manager Coordinator will manage a caseload of clients, assisting them in maintaining stable housing and accessing essential resources to support their overall well-being. PRIMARY JOB FUNCTIONS Identify High Utilization individuals for care coordination needs: Includes individuals with a serious mental illness (SMI), substance use disorder (SUD), serious emotional disturbance, or co-occurring disorders, who demonstrate high utilization of acute care services, including crisis stabilization, inpatient, Statewide Inpatient Psychiatric Program services (or equivalent out of state treatment) and inpatient detoxification services that experience: Three (3) or more acute care admissions or evaluations at an acute care facility within 180 days, acute care admissions that last 16 days or longer or are awaiting placement in a state mental health treatment facility (SMHTF) or awaiting discharge from a SMHTF back to the community. Client Engagement and Assessment: Engage clients in person and by phone to build a supportive, trusting relationship, conduct thorough assessments to understand each client's needs, strengths and goals and develop and implement individualized care plans that address mental health, housing stability and other essential needs. Care Coordination and Resource Linkage: Coordinate services with healthcare providers, social service agencies, and other community resources to ensure clients have comprehensive support. Provide referrals to appropriate services for mental health, substance abuse, medical care, financial assistance, and vocational support. Assess the individual for eligibility of Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), Veteran's Administration (VA) benefits, housing benefits, and public benefits, and assist them in obtaining eligible benefits. When applying for SSI or SSDI benefits, providers must use the SSI/SSDI Outreach, Access, and Recovery (SOAR) application process. Free training is available at ********************************************************************************************** Assist clients with activities of daily living (ADLs), as needed, to support housing retention. Care Coordination serves to assist individuals who are not effectively connected with the services and supports they need to transition successfully from higher levels of care to effective community-based care. Effective transitions and warm hand-offs - current providers directly introduce the individual to the care coordinator. The “warm hand-off” is both to establish an initial face-to-face contact between the individual and the care coordinator and to confer the trust and rapport the individual has developed with the provider to the care coordinator. Culturally humble and linguistically competent - the Care Coordination process demonstrates respect for and builds on the values, preferences, beliefs, culture, and identity of the individual served, and their community. This includes services and supports that affect an individual's overall well-being, such as primary physical health care, housing, and social connectedness. Crisis Intervention: Provide frequent contact for the first 30 days of services, ranging from daily to a minimum of three times per week. Care coordinators should consider the individual's safety needs, level of independence, and their wishes when establishing the optimal contact schedule. This includes telephone contact or face-to-face contact (which may be conducted electronically). Leaving voicemail is not considered contact. If the individual served is not responding to attempted contacts, the provider must document this in the clinical record and make active attempts to locate and engage the individual. Provide 24/7 on-call availability. Monitor client progress and assess for any potential crises or risks, intervening when needed to prevent hospitalization or loss of housing. Support clients in crisis situations, following established protocols to maintain client safety and stability. Documentation and Compliance: Maintain accurate, timely, and confidential client records in accordance with Boley Centers' policies and funder requirements. Ensure documentation meets compliance standards and supports program objectives. Utilize at least 50% of allocated funds in OCAs MH0CN and MS0CN to serve the following populations. Adults with a serious mental illness (SMI), substance use disorder (SUD), or co-occurring disorders who demonstrate high utilization of acute care services, including crisis stabilization, inpatient, and inpatient detoxification services. Adults with SMI, SUD, or co-occurring disorders who are at risk of re-entry into crisis stabilization, inpatient, and inpatient detoxification services. Adults with a SMI awaiting placement in a state mental health treatment facility (SMHTF) or awaiting discharge from a SMHTF back to the community. The following populations may receive Care Coordination from the remaining balance of OCAs MS0CN and MH0CN allocated funds. Individuals with serious emotional disturbance (SED), SMI, SUD, or co-occurring disorders who are involved with the criminal justice system, including: a history of multiple arrests, involuntary placements, or violations of parole leading to institutionalization or incarceration. B. Caretakers and parents at risk for involvement with child welfare. C. Individuals identified by the Department, Managing Entities, or Network Service Providers as potentially high risk due to concerns that warrant Care Coordination. Advocacy and Education: Advocate for clients' needs within the community and with other service providers. Educate clients and families on managing mental health symptoms, navigating housing processes, and utilizing available resources. Will comply with Compliance Plan and standards of conduct and report any non-compliance to the appropriate official. Will attend all required trainings, including participating in ongoing continuing education and complete other projects/duties as assigned. Additionally, certain metrics will be monitored, and assistance will also entail social entitlements such as food stamps, Medicaid, social security, and connecting clients to housing supported programs. JOB FUNCTIONS: Completes client intake, admission, and discharge to the program. Complete the assessment, mental status, medical/dental health, use of drugs or alcohol; education and employment; social development and functioning, activities of daily living, family structure, baker act history, and incarceration history. Is willing to publicly identify as a person living in recovery for the purpose of educating, role modeling, and providing hope to others about the reality of recovery and has had the proper training and experience to work in a provider role. A valid driver's license is required as some driving and/or transportation may be required to take clients to medical appointments, job sites, social activities and other community resources. Use of personal vehicle required with mileage reimbursement provided. Must pass a Level Two background screening for Peer Recovery Specialist. PHYSICAL REQUIREMENTS: Ability to sit, stand, type, and drive for extended periods. Capable of working in diverse environments and responding to emergencies effectively. Must complete Non-Abusive Psychological and Physical Intervention (NAPPI) training and demonstrate the ability to apply its techniques. Ability to communicate clearly and effectively, both orally and in writing. SPECIAL WORKING CONDITIONS: Regular exposure to clients and various program environments, including potential long and irregular hours, such as weekends. May encounter emergency situations, including exposure to verbally abusive or potentially violent individuals. Willingness to work alternative hours as needed, including local travel.

EDUCATION AND EXPERIENCE: High school diploma or GED required. Six months of experience working in a human services organization. 30 hours of college credits can be substituted for experience.
