As a member of the Eleos treatment team, the Discharge Planner completes individualized assessments, treatment planning, case management, and discharge planning for mental health clients during their inpatient stay. Discharge Planners engage with clients on the Crisis Stabilization Units (CSUs) on an ongoing basis to identify and confirm appropriate outpatient placement and sheltering arrangements upon their release from the facility. Attention to detail and thorough documentation of all efforts made are essential to this role. Shift: Monday through Friday, 8am - 4pm. Occasional overtime, weekends, or holidays may be required.
