Job Summary: Hours: Per Diem, as needed Union: None Union Name: None Patient Facing: Yes BHCHP is currently seeking a provider with geriatric experience to provide high quality, patient-centered care to older individuals experiencing homelessness on a newly created geriatric care team. Responsibilities: A. Support Geriatric Team Advise a team consisting of a nurse and a case manager in the clinical care of older individuals experiencing homelessness. Meet regularly with the team to discuss individual cases, overall strategy, quality improvement projects, development of workflows, etc. Help the team design assessments, documentation templates, and other tools. B. Patient Care Provide episodic care to patients, as needed by the team, at BHCHP sites and possibly within the home, working within BHCHP's clinical and administrative guidelines. Coordinate patient care with onsite clinical staff at host institutions, including shelters, hospitals, detox facilities, etc. Refer patients when medically necessary for specialty, hospital, recuperative or other appropriate care such as detox, providing appropriate information for continuity of care. Provide direct patient care including, if needed, clinical administrative duties such as lab follow-up, quality assurance, and completion of patient forms. Provide comprehensive follow-up of all laboratory tests including notification of patients of all results according to BHCHP guidelines, and to provide appropriate follow-up of all abnormal results following the highest standards of medical care. Provide education to patients on health promotion and disease prevention. Provide consultation to internal care teams regarding aging individuals with complex health and social needs. C. Collaborative and Educational Responsibilities Educate peers and other clinic staff by providing talks in areas of interest and expertise. Participate in a collaborative agreement with nurse practitioners and physicians' assistants, ensuring the highest quality clinical care for patients co-managed. Participate in the teaching of students and act as a role model and mentor for students, residents, and fellows. Actively participate in department meetings, general staff meetings and other meetings including quality assurance, pharmacy, etc. Attend CME meetings on a regular basis as mandated by the appropriate State Medical Board to maintain the highest level of clinical competence. D. Administrative Responsibilities Maintain adequate medical records, completed in a timely fashion, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. Participate in peer review by performing regular chart reviews. Maintain an active DEA certificate, medical license, current hospital privileges, and managed care credentials. Perform any other reasonable and appropriate duties at the request of the Medical Director. Qualifications: MD, NP or PA licensed to practice in the Commonwealth of Massachusetts. Board eligible or certified in family practice, internal medicine, or other appropriate specialty. Interest in working within multidisciplinary teams to provide primary care to homeless persons. Life support training certification (e.g., CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. Bilingual candidates are encouraged to apply. Fluency in another language (especially Spanish and Haitian Kreyol, which are most reflected in our patient population) is desired. Familiarity with addiction medicine and/or Health Care for the Homeless program(s) is desired. Strong understanding of the complex health problems of homeless people, substance use disorder, harm reduction strategies, and the social determinants of health. Demonstrated interest in working with an underserved population. Compensation: The compensation is $77.00 per hour for licensed NP/PAs and $106.00 per hour for licensed MDs. This work is a part of a MassHealth initiative that provides qualified MassHealth enrollees with a service benefit called Behavioral Health Community Partners. As the BH-CP Care Coordinator, you will work as part of an interdisciplinary BH-CP team and take responsibility for coordinating the care for 50 enrollees. You will work closely with other care team members and will be responsible for engaging their enrollees regularly (several times a month) and making sure that the enrollee is getting connected to vital services and progressing towards the goals outlined in their Person-Centered Care Plan. Responsibilities: Communicate and engage with enrollees regularly and help enrollee progress on goals identified in Patient Centered Care Plan. Work with the nurse care managers and conduct timely Comprehensive Assessments. Document into data platform regarding enrollee progress on Care Plan goals, all enrollee encounters, and care coordination notes. The role includes offering frequent, non-judgmental check-ins within the respite milieu, assisting with behavioral concerns in partnership with the nursing team, and ensuring that sedated patients are triaged and safely monitored using a supportive, person-centered, and trauma-informed approach. Services provided will include clinical assessments, case management support, triage, medical monitoring and intervention, and ongoing harm reduction education tailored to each patient's goals and readiness for change. The EMT will also assist with the respite methadone protocols and coordinate with outside MAT programs as needed. Schedule: PER DIEM WITH HOLIDAYS Position Summary: Performs a variety of routine duties to assist in laboratory processes, such as, performing venipunctures on both inpatient and outpatient population in various areas of the hospital. Performs a variety of functions related to accurate and safe collection and processing of laboratory specimens from patients of all ages. Follows the procedure manual and operates in accordance with the policies of the hospital. Responsibilities: Lab Registration Registers and orders specimens received from clients into LIS. Receives, verifies, and enters patient, test, and information into LIS computer system according to policy. Generate barcode labels, label specimens, print outstanding specimen list. Specimen Collection Perform phlebotomy on inpatient and outpatient populations using the stated procedural policies. Perform therapeutic phlebotomies. Monitors workflow using the specimen tracker. Specimen Processing/Send-Outs Processes specimens including centrifugation, aliquoting, labeling. Prepares specimens to be sent to referral laboratories; obtains proper detailed information pertaining to sample requirements such as: test codes, appropriate specimen type and volume needed, process and deliver specimens to appropriate department. Package and send specimens to the appropriate Reference Laboratory per specimen requirements. Microbiology This job may also include plant specimens on correct media and incubate them properly. Performs assigned duties to ensure, an adequate inventory of supplies for assigned area. Monitors inventory levels and stores equipment and supplies in a neat and orderly manner. Maintains accurate records of processes performed, using manual and computer-based systems. Requirements: Education: High school diploma or equivalent Experience: Graduate of phlebotomy training program preferred Certification/Licensure: phlebotomy certification preferred, ASCP certification/eligible preferred Software/Hardware: EPIC, SunQuest Dexterity sufficient to manipulate instrumentation. Ability to read, understand and follow written procedures. Ability to communicate adequately with all laboratory clients. Basic clerical/data entry skills preferred. Primary Duties: Increase Corporate Funding. Work in partnership with Director of Development and Corporate Relations Manager to meet or exceed corporate fundraising goal by: Securing face-to-face meetings with decision-makers within corporations Expanding the number and breadth of major corporate donors by identifying new corporate prospects through Board and Philanthropic Advisory Council contacts, BHCHP vendor lists, participating in and/or hosting corporate breakfasts, researching corporate boards, Boston Business Journal, Chamber of Commerce, and local media. Development multi-faceted strategies to partner with companies via: corporate sponsors/partners, corporate foundation grant support, employee giving/match programs, and corporate volunteer opportunities. Expanding BHCHP's Philanthropic Advisory Council to include broader involvement from different business sectors as well as representation of BHCHP's Emerging Leaders program Using our Annual Gala, Fall Soiree, Boston Marathon program, and other special events as a vehicle to introduce (and secure support for) our mission to a wider corporate audience and secure sponsorship support Developing specific and creative opportunities for Corporate Sponsorships Develop new and leverage existing stewardship strategies to foster long-term relationships with institutional funders Increase Support from Individuals at the Major Gift Level.
