BHCHP is currently seeking a provider with geriatric experience to provide high quality, patient-centered care to older individuals experiencing homelessness on a newly created geriatric care team.
Responsibilities: A. Support Geriatric Team
Advise a team consisting of a nurse and a case manager in the clinical care of older individuals experiencing homelessness.
Meet regularly with the team to discuss individual cases, overall strategy, quality improvement projects, development of workflows, etc.
Help the team design assessments, documentation templates, and other tools.
B. Patient Care
Provide episodic care to patients, as needed by the team, at BHCHP sites and possibly within the home, working within BHCHP's clinical and administrative guidelines.
Coordinate patient care with onsite clinical staff at host institutions, including shelters, hospitals, detox facilities, etc.
Refer patients when medically necessary for specialty, hospital, recuperative or other appropriate care such as detox, providing appropriate information for continuity of care.
Provide direct patient care including, if needed, clinical administrative duties such as lab follow-up, quality assurance, and completion of patient forms.
Provide comprehensive follow-up of all laboratory tests including notification of patients of all results according to BHCHP guidelines, and to provide appropriate follow-up of all abnormal results following the highest standards of medical care.
Provide education to patients on health promotion and disease prevention.
Provide consultation to internal care teams regarding aging individuals with complex health and social needs.
C. Collaborative and Educational Responsibilities
Educate peers and other clinic staff by providing talks in areas of interest and expertise.
Participate in a collaborative agreement with nurse practitioners and physicians' assistants, ensuring the highest quality clinical care for patients co-managed.
Participate in the teaching of students and act as a role model and mentor for students, residents, and fellows.
Actively participate in department meetings, general staff meetings and other meetings including quality assurance, pharmacy, etc.
Attend CME meetings on a regular basis as mandated by the appropriate State Medical Board to maintain the highest level of clinical competence.
D. Administrative Responsibilities
Maintain adequate medical records, completed in a timely fashion, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc.
Participate in peer review by performing regular chart reviews.
Maintain an active DEA certificate, medical license, current hospital privileges, and managed care credentials.
Perform any other reasonable and appropriate duties at the request of the Medical Director.
Qualifications:
MD, NP or PA licensed to practice in the Commonwealth of Massachusetts.
Board eligible or certified in family practice, internal medicine, or other appropriate specialty.
Interest in working within multidisciplinary teams to provide primary care to homeless persons.
Life support training certification (e.g., CPR, BLS, ACLS) from a course that includes a hands-on, in-person component.
Bilingual candidates are encouraged to apply. Fluency in another language (especially Spanish and Haitian Kreyol, which are most reflected in our patient population) is desired.
Familiarity with addiction medicine and/or Health Care for the Homeless program(s) is desired.
Strong understanding of the complex health problems of homeless people, substance use disorder, harm reduction strategies, and the social determinants of health.

Compensation:
The compensation is $77.00 per hour for licensed NP/PAs and $106.00 per hour for licensed MDs.

This position is part of a study entitled, "Stepped Care for Posttraumatic Stress Disorder (PTSD)," which aims to provide a stepped care model for PTSD in BMC primary care patients 18+. The Research Assistant will assist with data collection by conducting clinical interviews and administering surveys; participant scheduling and correspondence; reviewing recorded clinical interviews; and attending team meetings
STEPPS duties include recruitment of individuals with PTSD, pre-screening/scheduling, data collection through interviews, surveys, and electronic medical record data extraction, and coordination of study visits and meetings associated with conduct of a clinical trial study. The RA will have the unique opportunity to collaborate with a multidisciplinary team across primary care and psychiatry. There will also be opportunities to collaborate on posters and papers, if interested.
JOB RESPONSIBILITIES:
* Recruits individuals to participate in the study; conducts the enrollment of study participants, including explaining research procedures & protocols, and obtaining informed consent of participants. Identify participants with significant clinical concerns and relays this information to the Principal Investigator.
* Schedules appointments of study participants; conducts reminder phone calls and/or sends mailouts. Administers questionnaires and assists study participants in navigating through questionnaires.
* Reviews the data collection forms for each participant for completion and quality; checks work of junior staff performing data entry to ensure timely and accurate entry (reviews protocols, abstracts secondary data from existing records, notes, etc.)

Boston Health Care for the Homeless Program (BHCHP) is seeking a part-time Harm Reduction Technician to work at the Barbara McInnis House (BMH), our 103-bed medical respite facility in Boston. While working in respite, the EMT will be required to provide harm reduction-centered education and support, monitor sedation, and provide compassionate engagement with patients in collaboration with the Behavioral Health team. The role includes offering frequent, non-judgmental check-ins within the respite milieu, assisting with behavioral concerns in partnership with the nursing team, and ensuring that sedated patients are triaged and safely monitored using a supportive, person-centered, and trauma-informed approach.
Services provided will include clinical assessments, case management support, triage, medical monitoring and intervention, and ongoing harm reduction education tailored to each patient's goals and readiness for change. The EMT will also assist with the respite methadone protocols and coordinate with outside MAT programs as needed.
Responsibilities:
Provide direct clinical care to meet patient needs in accordance with established policies and procedures, as well as Department of Public Health regulations
Deliver supportive, compassionate, and non-judgmental care to individuals with substance use disorders, fully aligned with harm reduction principles

The Medical Respite Social Worker delivers comprehensive case management and behavioral health assistance to patients at the Barbara McInnis House. This role involves completing biopsychosocial assessments within 48 hours of admission, coordinating discharge plans, and connecting patients to benefits and community resources as needed. The compensation ranges from $66,000 - $105,600 annually and increases based on years of experience.

ESSENTIAL RESPONSIBILITIES / DUTIES:
The Clinical Informaticist must be a collaborative professional who can co-lead major ITS initiates to support the overall mission and values of Boston Medical Center. Serves as a role model and liaison for bridging the functions of clinical care and information technology.
Provide clinical expertise as a project lead within the BMC ITS Department in the analysis, design and development of clinical related components of BMC's current and future EHRs. Provides guidance and subject matter expertise to assist with integration of related processes (i.e. clinical workflow) necessary to achieve successful integration of new technologies.
Co-leads cross functional project work groups in making design decisions by building consensus, engaging the necessary resources and escalating issues to appropriate project management and governance councils. Ensures workgroup goals are aligned with overall project/departmental goals. Is responsible for co-managing the workgroups project timeline, scope, deliverable and budget in conjunction with the project manager/lead utilizing standardized project management templates.
Partners with the clients/clinicians to understand their work processes and business goals to adequately gather information to document project/request requirements. Ensures the client/clinician understands the work required implementing a new solution/enhancement and how the new solution/enhancement may change their current workflow. Identifies desired results of an initiative and drives toward achieving that result.
Assists project, functional or business manager with the development of a comprehensive testing strategy including the development of test plans, test scripts and test conditions based on the Business and System Requirements. Tests solutions/enhancements to validate whether requirements have been met.
Assists project, functional or business manager with the development of a comprehensive operational readiness strategy including designing the training program, creating a communication plan and forming a go-live support plans. Participates in post-deployment phases. Assesses and evaluates all feedback to ensure that the data collected is utilized to make future improvements.
Participates in the creation of executive summaries and presentations for management's review on identified initiatives. Participates in a number of organizational committees representing ITS.
Identifies opportunities for improving operational and system performance and escalates issues and problems requiring immediate attention according the standardized incident management process. Identifies opportunities for improving efficiency/reliability of ITS applications.
Interfaces with vendor and contractor to answer application and requirements questions and to resolve issues related to work assignments.
Serves as a liaison to BMC clinicians, including physicians, nurse, pharmacists and ancillary staff to effectively execute assigned tasks and projects.
Works is directed by limited supervision, using established procedures. May instruct, guide and oversee work of other ITS staff while managing project requests.

JOB REQUIREMENTS
EDUCATION:
Work requires a Bachelor's Degree. Graduate degree (MBA) preferred.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
Current RN license to practice in the State of Massachusetts is required.
Formal training and education in Clinical Informatics preferred.
EXPERIENCE:
Requires at least 5 years of health care experience of which at least 3 years are in clinical role.
Experience in organizing, planning and executing projects from vision through implementation, involving internal personnel, contractors and vendors is preferred.
Prior leadership experience preferred.
KNOWLEDGE AND SKILLS:
Knowledge of clinical workflow and processes.
Understanding and execution of change management process: utilizing proven collaborative, decision making, delegation and follow up skills.
Project management experience including co-leading the planning and implementing multiple projects in a cross functional environment.
Familiarity with process redesign and quality improvement principles.
Ability to co-manage project team; embracing a collaborative and multi-disciplinary approach to problem solving.
Willingness to take accountability for decisions and to take corrective actions where necessary.
Strong written and verbal communication skills.
Demonstrated leadership skills to facilitate meetings drive decisions and assigned task ownership.
Strong customer service skills
Proficient with MS Office tools including: MS Project, Visio and PowerPoint.
Compensation Range:
$104,000.00- $151,000.00
