The Care Management Social Worker in the Care Management Department works effectively to coordinate discharge planning activities in cases requiring short- and long-term placement. Assists patients and their families in coping with illness and resolving complex predisposed or incurred, emotional, financial, and environmental difficulties which interfere with obtaining maximum benefits from medical care. Provides consultation to medical and paramedical personnel regarding impact of psychosocial factors on patient illness.
ESSENTIAL RESPONSIBILITIES / DUTIES:
Effectively coordinates discharge planning activity in cases requiring social work/Care Management team intervention.
Acts as case manager, mobilizing and monitoring all adjunct activities necessary to effect appropriate discharge plans, including securing appropriate financial resources, and supporting patients and families through clarification and communication of the discharge planning process.
Develops discharge plans based upon functional assessment in collaboration with patient, family, physician and health care team.
Implements discharge plans and arranges for appropriate post-hospital care by referring patients to appropriate transfer facilities or home health/home care agencies.
Provides consultation to physicians, health care team, and other professionals by providing essential psychosocial information diagnostic formulations, plans and treatment recommendations.
Participates in team meetings and coordinates patient care meetings with in-house and outside agencies.
Supports department policies, procedures and practices for documentation and providing high quality care, to ensure compliance with the guidelines of the J.C.A.H.O, the Department of Public Health, and other regulatory agencies as required.
Thoroughly reviews patient charts (as applicable) and always documents services
Evaluates community resources for quality and appropriateness and develops and maintains referral and transfer relationships.
Completes department statistical reports, agency specific referral forms, and other required documents from regulatory programs.
Completes Medicare and other application forms as is appropriate.
Provides information/referral services.
Provides consultation to Interdisciplinary Team.
Participates on committees as assigned regarding patient care issues/service enhancement.

REQUIRED EDUCATION AND EXPERIENCE:
Master Degree of Social Work, Psychology, or Counseling; and 0-2 years of overall experience working with patients and families; or equivalent combination of education and experience.

Job Summary
Home Base, a Red Sox Foundation and Massachusetts General Hospital program, is dedicated to healing the invisible wounds - including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, family relationship challenges and other issues associated with Military service - for Veterans of all eras, Service Members, Military Families and Families of the Fallen through world-class clinical care, wellness, education, and research.
The Massachusetts General Hospital seeks a dynamic Licensed Clinical Social Worker (LCSW)/Licensed Independent Clinical Social Worker (LICSW) to serve on the Special Operations Team. In this role, the LCSW/LICSW will provide and oversee the provision of psychiatric, psychosocial, and overall mental health services and referrals for military Special Operators into the Home Base program. They will also be dedicated to supporting the Special Operations Forces (SOF) program and will provide coverage to other clinical operations as needed. The LCSW/LICSW may help cover approximately 2-3 weekend shifts per year.
Summary
This position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

Essential Functions
* Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.
* Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
* Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
* Coordinate care for Special Operations Forces (SOF) veterans and military-connected family members seeking care at Home Base with the appropriate internal or external clinicians in a multi-disciplinary team, coordinated care model.
* Serve as a contact point for accepting and triaging SOF referrals from internal clinicians, outside clinicians and self-referred patients.
* Serve as primary case manager for patients in SOF admissions process in collaboration with Admissions Coordinator, including, but not limited to, the following responsibilities: conduct pre-screening to assess treatment and evaluation needs for SOF patients referred; coordinate signing of Release of Information forms, acquire relevant medical records, and coordinate with SOCOM where applicable; serve as point of contact for ongoing questions from the patient as they arise.
* Monitor patient's status in admissions process using appropriate documentation.
* May be asked to collaborate with SOF/ComBHaT team, patients, clinical team, and Resource Specialist to identify clinical and psychosocial needs in patients' home communities and identify appropriate resources.
* May provide direct clinical services to Veterans and/or family members, which may include group or individual therapy.
* May provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties.
* Collaborate with an inter-disciplinary team of clinical staff (social workers, psychologists, nurses, psychiatrists, physiatrists, physical therapists, etc.) to coordinate patient care and plan for discharge/aftercare when needed.
* Consult with team to facilitate mandated assessments when abuse is suspected (child, disabled adult, elder), and safety assessment when violence is reported. Collaborates with MGH resources (HAVEN and Child Protection Team).
* May assist with crisis intervention and management.
* Documents timely and relevant information in patient electronic medical record and Home Base database.
* Provide coverage for social work responsibilities in Outpatient Clinic as needed.
* For LICSWs: provide clinical supervision to LC-level and/or MSW students as needed.
* Additional responsibilities as assigned.

Assesses patient for post-acute care needs. Determines discharge plan through consultation with the Care Coordinator, Clinical Social Worker, patient and/or designee. 6. Advocates for patients and families as they negotiate the health care system so that effective care, treatment, and planning may occur.
7. Documents Social Work intervention and/or other duties related to discharge plan.

Position Summary:
Responsible for the coordination of digital health and community-based programs, including the Subacute Rehab At Home (SARAH) and Mobile Integrated Health (MIH) clinical programs. Facilitates each phase of the patients' MIH encounter or SARAH stay, including acquisition, onboarding, care coordination, and discharge planning. Collaborates with the treatment team to develop plans of care, as well as engage and integrate appropriate resources for the achievement of maximal clinical and financial patient outcomes. Functions in the roles of clinician, consultant, advocate, educator, and facilitator in his/her relationship with the patient, family, care providers, and payers.
Major Responsibilities:
1. Review referrals and collaborate with inpatient case management clinicians to identify patients appropriate for digital health services in conjunction with the research team. Assists the research team in verifying clinical eligibility and facilitates communication between the inpatient care teams and the SARAH clinicians to ensure all patients' clinical and social needs are met.
2. Facilitates the onboarding process of patients to the SARAH program by coordinating with various treatments involved in the patients' care plans, including the hospital team, rehab therapy providers, command center staff, and geriatricians, to ensure a smooth patient transition.
3. Lead inter-disciplinary team rounds for patients in the SARAH program to provide coordinated care management, goals of care, and set discharge targets.
4. Continually evaluate the patient's readiness for transition to a different level of care or service. Educates the patient, family, and care team regarding resource options, facilitates decision-making, initiates plans, and secures resources for the patient on an ongoing basis.
5. Procures the necessary services and coordinates the transition with the patient, family, care providers, payers, facilities, agencies, and vendors when patients transition to a different level of care, such as home health or discharge home with or without services.
6. Coordinates care plans and communication between MIH clinicians and ambulatory team members to ensure continuity and appropriate follow-up for patients.
7. Identifies and refers eligible patients to special MIH programs (including transitional care services) by performing outreach to inpatient teams, patients and caregivers, and MIH clinicians.

Job Summary
The Massachusetts General Hospital seeks a full-time, licensed social worker (LCSW or LICSW) for a one-year training program in Addiction Medicine starting in June 2026. The Integrated Substance Use Disorder Training Program (ISTP) is a yearlong fellowship that aims to train nurse practitioners, psychologists, and social workers in team-based addiction care, especially in community-based settings.
The Social Worker Fellow will work with other ISTP trainees to provide inter-disciplinary care for people with substance use disorders in a community-based health center setting. The training program will provide clinical experience in a variety of placements including an inpatient addiction consult service, a low-threshold walk-in clinic, and outpatient dual-diagnosis programs for adolescents and adults. Training and mentorship will be provided by experienced addiction practitioners who are faculty of this program.

General Summary:
The Clinical Social Worker (LCSW/LICSW) is a key member of the interdisciplinary team who provides and oversees the provision of psychosocial care for selected patients and families. Conducts bio-psychosocial assessments, provides intervention and treatment as indicated. Identifies high risk psychosocial factors of patients/families that impact health status and helps hospital staff understand the influence of those factors upon the course of medical care. Clinical Practice:

* Provides psychosocial assessment of families to determine family relationships/systems as they relate to care of the patient. * Employs a range of clinical interventions such as individual, group or family counseling. Provides caregiver/family counseling/support to promote family cohesiveness to provide care to patient and prepare families for end of life. * Develops comprehensive bio-psychosocial assessments responsive to age appropriate and cultural needs and concerns. Advocates on behalf of patients and families to gain access to services and resources.

* Reviews patient information for assigned caseload, determines anticipated length of stay and psychosocial barriers to plan of care transitions discharge plan in collaboration with the Nurse Care Coordinator.
* Interacts with home care, community agencies and facilities to ensure safe and timely patient care transitions.
* Negotiates with care coordination team follow up contact with patient/family, community agency or facility to evaluate the effectiveness of the patient care transitions and identifies problems in service delivery.
* Ensures coordination of the communication process with patient/family concerning the plan of care, including coordination of family meetings and warm handoffs.
* Ensures that patient/family is involved in all phases of the care process to the greatest extent possible.
* Maintains current knowledge of and identifies needs in service delivery within social, governmental, protective services and legal agencies.

The Family Planning Clinic Social Worker meets with patients in an outpatient setting and has the opportunity to work with patients and families by providing emotional support, options counseling, and grief counseling during often challenging times. Additionally, this social worker assumes applicable responsibilities for patients on the Labor and Delivery unit and in the ante/post-partum inpatient space as needed.
