Provides skilled nursing services to patients struggling with substance use disorders. Ongoing assessment, intakes, evaluations, triage, teaching, inductions, stabilizations, and monitoring of patients in Office-Based Addiction Treatment (OBAT) model. Provide psychiatric care of patients within the Addiction Psychiatry Treatment Program (APTP), as well as support and services to patients from other settings referred to program for stabilization and assessment. Collaborates with team members, clinic settings at BMC and outside referral sites, works with providers from addiction and community health settings in assisting patients and supporting their referral needs.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Initial assessment and intake obtaining social, medical, mental health, substance use and medication history.
Review intake assessment with nursing team and provider and establish appropriate plan of care for patient
Revise patient plan of care with the addiction treatment team as needed to address patient needs and issues
Collaboration, communication, and meetings with addiction treatment team as needed and required
Review consents and treatment contracts with patients and significant others, obtaining signatures and giving patient a copy and leaving a hard copy in patient paper chart
Ongoing management of patients receiving medications for addiction (opioids, alcohol, and stimulant use disorders)
Education and assessment of patients looking to access opioid treatment programs
Assessment of signs and symptoms of opioid withdrawal using the COW (Clinical Opioid Withdrawal) scale
Assessment and monitoring of patients in the medication induction, stabilization, and maintenance phases of treatment
Ongoing education and support of patients in all phases of treatment including; nursing visits scheduled and random, urine toxicology screens, routine labs and as needed, medication teaching, monitoring, pill counts, observed dosing, and medication refills.
Follow State and Federal guidelines in providing care to opioid dependent patients in collaboration with licensed, prescribing physicians and pharmacy
Complete appropriate documentation with excellent record keeping
Ongoing psychiatric care of patients referred to the Addiction Psychiatry Treatment Program
Assesses the patient in a systematic and ongoing manner for both overt and covert signs and symptoms
Collects pertinent data using appropriate assessment techniques
Prioritizes data according to the patient's immediate conditions and needs
Involves the patient, significant others and appropriate health care providers in the collection of data when appropriate
Consistently uses clear concise and effective communication written/oral and comply with departmental policies when sharing/documenting relevant patient care data.
Provides follow up care in person and via telephone when needed
Works with the pharmacy and the Addiction Psychiatry Treatment Program team in obtaining medication orders, prescriptions, refills, and communication as needed
Provides cross coverage to other nurses throughout Outpatient Psychiatry Clinics
Performs annual Preventative Health Screening visits
Perform other duties as specified by supervisor

JOB REQUIREMENTS
EDUCATION:
Graduate of an accredited nursing program. Bachelor of Science Degree in Nursing preferred.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
Massachusetts licensure as a registered nurse.
Maintain R.N. staff competencies on an annual basis.
OBAT RNs are required to obtain CARN Certification within one year.
Additional certifications a plus:
Certification in Treatment of Substance Abuse preferred. Certification from accredited clinical research program preferred.
EXPERIENCE:
3 - 5 years clinical experience required.
2+ years of experience with patients with substance use disorders and/or mental health conditions preferred.

* 42 bed Medical Stepdown unit equipped with bedside hemodynamic measuring (21 beds) and 24 hour monitoring of those systems.
* 21 medical beds with telemetry capabilities, as well as continuous O2 monitoring.
* Unit frequently cares for trached vent dependent patients as well as Bipap/CPap rescue needs and Hi Flow O2 delivery systems.
* Patient ratios of 1:5 on Medical unit & 1:3 on Intercare unit.
* This unit is supported by a nurse educator as well as an evening clinical nurse coordinator. Also supported around the clock by respiratory therapy and APP/MD on site, as well as ancillary support, ( monitor techs, ambulation techs, PCTS and Operations assistants)
* We provide supportive, individualized, and tailored orientation.
* The only major referral care center and the only Level 1 trauma center and pediatric trauma center in the area.

As manager of the Perianesthesia Department, the PACU Flow Manager has direct responsibility for maximizing efficiency of patient flow, controlling the transfer of patients to and from the Operating Room/Procedural Areas to the PACU.
As part of the management team, time will be spent directing and monitoring patient flow in the extremely complex, high patient volume PACU. Decisions made regarding patient flow and PACU department utilization have direct impact on the expenditure of resources and maximization of revenues.

The PACU Flow Manager works in collaboration with the anesthesia floor leader, nursing management, and surgeons in determining, through critical judgment and decision-making, that optimum utilization is attained and maintained.
The PACU Flow Manager will develop and promote a team philosophy and interdisciplinary collaboration among staff to foster a continuum of care for the perioperative patient.

1. Prior to or within 24-48 hours of admission the RNCC, by interview of the patient/family, discussion with physician team and/or attending MD and other team members, develops a provisional treatment program and tentative discharge date.
2. Reviews daily treatment plan with physicians, nurses and patient / families to insure interdisciplinary communication and coordination is occurring.
3. Participates with nursing staff and physicians in patient care rounds to contribute to plan of care and monitor and report patient progress.
4. Collaborates with other departments to expedite sequencing and scheduling of interventions, consults, treatments and ancillary services.
5. Provides for daily continuity with patients to assure patient needs related to discharge are met.
6. Incorporates knowledge of utilization management principles and payer contracts into patient plans of care. Keeps physicians and nurses informed of implications.
7. Presents alternatives to inpatient stay to attending MD, team and patient / family based on assessed patient level of care and insurance benefits.
8. Seeks assistance and/or consultation from Care Coordination leadership with plans for outlier and potential or actual resource intensive patients.
9. Interacts with internal and external health care providers to facilitate patient care including post discharge services.
10. Contributes to the development, implementation and monitoring of practice guidelines.
11. Identifies attending, resident and nurse learning needs related to case management and works with service leaders to develop educational plan.
B. Discharge Planning
Coordinates and executes the discharge planning process for patients, ensuring each patient has a discharge plan.
1. Assesses continuing care needs in conjunction with other caregivers.
2. Coordinates and schedules interdisciplinary meetings with the patient and family regarding discharge needs and plan as appropriate.
3. Assures patient education consistent with discharge plan has occurred.
4. Identifies service, treatment and funding options for post-hospital care.
5. Promotes interdisciplinary patient/family communications and documentation that facilitate discharge planning striving to finalize plans the day prior to discharge.
6. Performs patient/family follow-up after discharge to monitor and support desired outcomes.
7. Initiates contact with home health agencies and extended care facilities to insure prompt and effective transition of care.
C. Utilization Management
Collaborates with appropriate individuals, departments, and payers to insure appropriateness of admission, continued days of stay, and reimbursement.
1. Identifies patients who are likely to have unmet insurance and resource needs and communicates with and/or makes referrals to other members of the health care team and other appropriate departments.
2. Communicates as needed with third party payers regarding patient's progress with treatment plan.
3. Identifies need for and issues Medicare notices of non-coverage, providing appropriate documentation of the process and communication to patient/family and other members of the health care team.
4. Utilizing industry accepted utilization and/or medical management criteria (InterQual) identifies, monitors and reports variances from established treatment plan, including appropriateness of admission, continued stay, delays in treatment, and discharge plan.
5. Conducts documented utilization reviews to insurers or intermediaries.
6. Identifies SNF and AND days for Medicare and Medicaid patients.
7. Initiates actions concurrently to reduce and/or eliminate inappropriate hospital admissions and days, and system delays.
8. Works with payers and physicians to concurrently address level of care concerns effecting claims and reimbursement.
9. Contributes to utilization and practice improvement efforts by reviewing reports with colleagues and providing feedback on utilization trends and payer issues.
10. Serves as the primary patient information source to third party payers.

QUALIFICATIONS Qualifications
Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required for newly licensed nurses and external candidates.
1+ years of acute care experience required
1 year inpatient acute hospital Case management experience required

JOB SUMMARY:
The Nurse Manager of Behavioral Health is a Registered Nurse who functions as a patient care provider, supervisor, and clinical resource for staff. The Nurse Manager in collaboration with the Director of Behavioral Health to implement patient management and clinical activities. Coordinates clinical activities to ensure that the nursing process is operationalized utilizing standards of care and practice, as well as maintaining consistency with the Behavioral Health Department's regulatory standards.
ESSENTIAL RESPONSIBILITIES / DUTIES:
Develops, incorporates and maintains the standards of clinical practice as defined by the Nursing Division.
Provides direct supervision to nurses and subordinate staff, ensuring appropriate assessment and treatment of patient health. Serves as key problem-solving resource for staff on patient issues. Manages and utilizes personnel effectively to achieve proper staffing and optimum resource management. Conducts performance review meetings, regularly provides feedback and coaching and is responsible for the overall development and succession planning of the staff
Studies patient needs/trends and suggests solutions to ongoing patient care problems and treatments
Develops budgets and operates within budgetary guidelines, justifying variances as needed
Participates in unit-based CQI activities and assists staff in identifying and monitoring the quality indicators that are most pertinent to patient care goals.
Actively participates in all collaborative group activities and appropriate committees, task forces, interdisciplinary forums that are concerned with patient care

$104,000.00- $151,000.00
