Physical Therapist, Outpatient (Winthrop Clinic) Department: Outpatient Rehabilitation Schedule: 40 hours per week Reporting to the Rehabilitation Therapies Supervisor/Manager, the Staff Physical Therapist conducts comprehensive assessments of in and outpatients with physical and functional deficits, planning and implementing individual/group physical therapy treatments to promote mobility, reduce pain, restore function, prevent permanent disability and restore independence, using various treatment techniques. Collaborates with other disciplines to arrange for appropriate follow-up care and facilitates a successful return to home and work environments. Provides student training according to departmental guidelines and participates in continuous education.
JOB RESPONSIBILITIES:
1. Conducts physical therapy evaluation procedures according to professional and departmental guidelines.
* Responds to all consults in assigned area within 24 hours; completes evaluation and documentation within 24 hours of consult (IP).
* Reviews patient records, gathers necessary information, and obtains a physician's order prior to assessing the patient.
* Selects evaluation procedures that reflect understanding of the patient's diagnosis, abilities, and limitations.
* Administers evaluation procedures according to the guidelines of standardized tests (including range of motion, manual muscle testing, mobility, air entry and breath sounds); adapts evaluation procedures as indicated to accommodate the specific needs of patients.
* Interprets evaluation results, develops an integrated diagnostic impression, and makes recommendations.
* Performs re-evaluations according to departmental policies, change in patient's status or response to treatment.
* Educates patient and caregiver regarding evaluation findings and management of care recommendations.
2. Establishes treatment goals and plans that reflect the needs of the patients while meeting departmental, institutional, and third party reimbursement requirements.
* Formulates treatment goals and plans that reflect the patient's unique psychological and functional deficits.
* Establishes measurable and attainable short and long-term goals in collaboration with patient and/or caregiver.
* Modifies goals as warranted by changes in patient's status.
* Assesses discharge needs of the patient and caregiver, including but not limited to: insurance issues, equipment needs, follow-up appointments, and appropriate use of community services.
* Communicates patient care and discharge needs to team members as needed. Arranges for appropriate follow-up.
3. Implements comprehensive individual and group treatment. Consistently demonstrates clinical competence.
* Establishes treatment priorities based on evaluation results and the specific needs of the patients.
* Uses creative treatment approaches and explores alternative solutions to individual patient problems.
* Demonstrates clinical problem solving skills with a variety of patient problems and substantiates actions taken.
* Progresses patient treatment programs appropriately and terminates them as warranted by patient's progress and condition.
* Recommends and/or issues appropriate assistive devices or durable medical equipment, ensuring that it properly fits the patient.
* Develops and instructs patient and caregiver in home programs and/or use of recommended equipment; provides verbal and/or written instructions as indicated.
* Establishes and maintains a therapeutic relationship with all patients.
* Demonstrates ability to engage patients in treatment, provides reinforcement and support; sets limits appropriately.
* Demonstrates the knowledge and skills necessary to provide care appropriate to the age groups regularly served.
* Notifies supervisor/manager when patients in age groups not regularly served are present, and seeks assistance from individuals with the knowledge of the specialized needs of such patient age groups.
* Adheres to diagnosis-related precautions. Demonstrates good judgment in handling potentially hazardous situations, seeking assistance as necessary.
* Assigns appropriate patients to Physical Therapist Assistant and Rehabilitation and/or Co-op Aide. Seeks feedback.
* Monitors clinical supplies on assigned floors, requesting replacements in a timely manner.
4. Complies with departmental, institutional, and accrediting agency policies and standards.
* Completes area specific documentation (including letters, re-evaluations) in an accurate thorough and timely manner according to department standards scoring an average of 95% or greater on each indicator.
* Issues supplies/adaptive equipment in a cost-effective manner. Justifies issuance of equipment, demonstrates actual use by patient. Looks for alternative solutions.
* Completes patient care charges accurately.
* Meets area specific productivity standards at least 95% of the time. Identifies problems and makes adjustments to meet targeted productivity.
* Participates in the peer review process. Completes form thoroughly, gives meaningful and constructive feedback, and provides learning experience to peer.
* Completes mandatory requirements (including PPD, competencies, and safety/infection control tests, CPR, and license renewal) within given time frame.
5. Shows evidence of on-going professional growth and knowledge of regulations and standards.
* Initiates independent learning opportunities, seeks out and integrates new information in daily practice. Communicates learning needs to supervisor.
* Stays current with advances in physical therapy treatments and techniques through attendance of professional meetings, continuing education, and reading of current literature.
* Shows knowledge of professional federal and state rules and regulations governing the practice of physical therapy.
* Assumes responsibility and accountability for the care of the patients being treated by the physical therapist assistant.
* Directs and supervises the rehabilitation and/or coop aide consistent with the State Practice Act in a safe and effective manner.
6. Participates in the development and evaluation of physical therapy programs
* Identifies assigned rotation/floor needs and communicates with senior therapists and supervisor. Contributes to the development and updating of unit files and other pertinent resource material.
* Assists in developing and achieving area goals.
* Collects data required to complete research projects, clinical outcome studies, and departmental quality improvement projects in compliance with established department standards.
* Contributes to the assessment, planning, and implementation of physical therapy programs in collaboration with supervisor and peers.
7. Collaborates with colleagues and other professionals to provide high quality patient care.
* Serves as clinical resource for other health care professionals on assigned floors.
* Works as part of the rehabilitation therapy team, helping and supporting staff members as needed; listens to and seeks feedback from others; shares knowledge with others in positive manner; expresses opinions clearly. Accommodates to change by being adaptable and flexible and maintains professional demeanor during periods of stress. Assumes a leadership role and takes charge in difficult situations.
* Attends and participates in all mandatory department meetings. Arrives prepared and on time.
* Accepts and completes additional assignments as requested by supervisor, including membership in department and hospital task forces and committees.
* Provides at least on evidence-based area in-service annually. Provides in-services to other hospital personnel as requested.
* Prepares for a verbal presentation using peer-reviewed journals and presents this information in a clear, organized, and concise manner.
* Participates responsibly in the supervisory process, follows through on suggestions and recommendations, and utilizes supervision time for own professional growth. Comes prepared to supervisory meets.
* Develops and meets annual goals with a high quality of performance.
8. Orients medical professional staff and the community to physical therapy. Supervises physical therapy students.
* Participates in pre-affiliation, class related, or other observation experiences.
* Orients medical professional staff and students and the community (e.g. volunteers, high school initiatives).
* Shows leadership and flexibility by taking students despite a new or challenging rotation.
* Takes additional physical therapy students to help meet the needs of the department/students.
* If not eligible to take own students, assists with occasional supervision of other clinical instructor's students. Provides a well-rounded learning experience for affiliating physical therapy students.
* Assesses the learning needs of assigned physical therapy students and provides training in patient evaluation and treatment procedures as practiced on assigned units.
* Facilitates development of clinical problem solving skills of the affiliating students by asking probing questions.
* Schedules weekly meetings with assigned students providing growth-promoting feedback and learning experiences.
* Keeps student program coordinator informed of student's performance. Assesses the learning needs of assigned physical therapy students and provides training in patient evaluation and treatment procedures as practiced on assigned units.
* Cooperates with student program coordinator and academic coordinator to develop and implement corrective actions when indicated.
* Conducts student performance evaluations, discusses evaluation with students, and submits the completed form to the student program coordinator in a timely fashion.

**Summary:**
Assists patients in reaching maximum physical performance while adjusting to disabilities or recovering from injuries. Treats patients using standard physical therapy procedures. Determines patients capabilities needs interests and goals and prepares individually tailored programs consistent with the physician's diagnosis. **Job Responsibilities:**
**1)** Evaluates all assigned patients, develops plan of treatment, provides assessment and assists in discharge planning as appropriate for each discipline. **3)** Completes necessary documentation to comply with professional standards department procedures for initial consult, progress notes, clinical conference notes, and dictate reports. Completes documentation of initial consultation of assigned patients within 24 hours of receipt. As appropriate, includes history, test results, clinical observations, diagnosis statement, prognosis, and recommendations. Documents patients progress toward specified RX goals, diagnosis, change in patient status related to goals and recommendations for progress notes and/or discharge report.
**4)** Provides information and consultation to patients, families, caregivers, and other health care personnel involved with patient management. As indicated, provides and documents patients and families counseling to enhance understanding of disorder(s) and goals of RX. Completes appropriate consultation with MDs during patients' course of RX. Completes consultation with nursing staff ancillary service and/or outside referring agencies as indicated. **5)** Completes department operational procedures, including scheduling, billing, equipment usage, time-off requests and caseload coverage assignments. Submits patient charges and/or crediting within 24 hours. Demonstrates appropriate usage of all department equipment and issuing of equipment. Adheres to department policy for requesting time off. Provides staff coverage during staff shortage for all assigned patients in an efficient manner.
**6)** Assist with on-the-job orientation and training for student interns. Assists students in developing in-service. Contributes to the review of and co-signing of all documentation completed by intern. Contributes to intern rating format at midterm and end of internship.
**7)** Demonstrates relevant knowledge and application of clinical skills in evaluation, treatment and counseling. Establishes and implements 1-2 specific professional development objectives to be reviewed annually.

The Rehabilitation Services Department at Brigham and Women's Hospital provides physical therapy, occupational therapy, speech language pathology, and orthopedic technician services to patients on a consult basis to our inpatient floors, ICUs, and the Emergency Department. The Brigham offers physical therapists the opportunity to rotate to seven teams: Orthopedics/Trauma, Cardiovascular, Thoracic/Pulmonary, Oncology, Neurology, General Medicine, and the Emergency Department. In addition to providing exemplary patient care, Senior physical therapists serve as mentors to other staff in the department. They will assist in staff education and training, triaging patient consults, program development, quality initiatives, and optimizing departmental policies and procedures. Senior therapists can also participate in a variety of Rehabilitation Department Committees to enhance their professional growth in addition to their clinical care. * Must possess effective communication skills (written and verbal) to interact with patients (up to 70% of work time), rehabilitation and medical team (up to 20% of work time), and patients' families (up to 10% of work time).
* Work requires prolonged walking and standing (up to 95% of work time); lifting, transferring, and positioning of patients and equipment (10% - 50% of work time), and the transportation of patients (up to 10% of work time).
* Work with critically ill patients (up to 10% of work time) produces a high level of stress and mental fatigue.
* Must be able to work with newborns (up to 10% of the work time) to the oldest old (up to 10% of the work time). The majority of patient care (70% of work time) is the 18- to 65-year-olds.
* The individual in this position must demonstrate appropriate knowledge of the principles of growth and development, which would include neonates to the oldest old. In addition, the therapist must demonstrate competency in assessment and treatment relative to age specific needs for the patient population for which they provide therapy, and as described in the department's practice standards.

* Works in hospital environments where there is some exposure to blood/body fluids/hazardous materials, communicable diseases (10% - 25% of work time) and unpleasant odors (up to 25% of work time).

We are seeking a dedicated physical therapist who has experience working with patients with orthopedic impairments and conditions. Staff therapists are expected to work with a variety of orthopedic diagnoses including, but not limited to non-operative and post-operative management of the spine, the upper extremities, and the lower extremities.
Therapists will also treat patients with balance and gait disorders, connective tissue and rheumatologic conditions, central and peripheral vestibular dysfunction, pain-related disorders, and oncology-related sequelae.
Staff Orthopedic Physical Therapists will be joining a team with diverse clinical backgrounds and expertise. Therapists should have an unrestricted license to practice physical therapy in the Commonwealth of Massachusetts.
Their primary responsibility is to provide patient care in accordance with State licensing regulations, departmental standards, and policies, and accepted professional standards of care. Additionally, we are looking for clinicians who will be able to treat patients with a variety of different backgrounds and identities.
Staff therapists will receive mentorship from experienced clinicians, many of whom are Board Certified Clinical Specialists. In addition to regular collaboration with other rehabilitation disciplines and the multidisciplinary team, therapists will have opportunities for clinical growth through internal continuing education programming, financial support for external continuing education courses, and conferences, as well as access to a formal mentorship program.
Therapists can also participate in a variety of Rehabilitation Department Committees to enhance their professional growth in addition to their clinical care. These committees include: The Community Resource Committee, Practice Standards Committee, Staff Development Committee, Rehab Recognition Committee, and Research Committee. Primary Assigned Clinical Area: Outpatient Ambulatory Care 70 Francis Street in Boston, MA
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Examines and evaluates patient's physical therapy needs. In collaboration with the patient and/or caregiver, formulates a physical therapy diagnosis, and develops a treatment plan with treatment goals directed towards maximizing function.
2. Demonstrates competency in all clinical knowledge and skills specific to the patient case type(s) in assigned area of care.
3. Consults with other members of the health care team to ensure coordination and effectiveness of therapies provided.
4. Assists in implementation of triaging, prioritizing, monitoring admissions, referrals, and discharges to and from the service as assigned.
5. Manages patient caseload assigned.
6. Participates in activities that directed towards increasing knowledge and skill and professional development.
7. Participates in departmental meetings, which help to formulate policies, procedures and program development.
8. Supervises aides and other support personnel as appropriate.
9. Supervises students and other individuals seeking educational experiences related to physical therapy.

* 1. Must be a graduate of an accredited physical therapy program.
* 2. Must possess a current unrestricted license to practice physical therapy from the Commonwealth of Massachusetts.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
1. Must possess communication and interpersonal skills sufficient to manage a diverse patient population of varying diagnosis and case types.
2. Must possess analytical abilities and sound knowledge in Physical Therapy to effectively manage a varied caseload demonstrating sound clinical decision-making and judgment (up to 90% of work time).
3. Work requires up to three months orientation to acquire necessary familiarity with department and hospital policies and procedures, and the specific quality standards of care and competencies, and procedures unique to the assigned patient area.
4. Must possess effective communication skills (written and verbal) to interact with patients (up to 70% of work time), rehabilitation and medical team (up to 20% of work time), and patients' families (up to 10% of work time).
5. Work requires prolonged walking and standing (up to 95% of work time); lifting, transferring, and positioning of patients and equipment (10% - 50% of work time), and the transportation of patients (up to 10% of work time).
6. Work with critically ill patients (up to 10% of work time) produces a high level of stress and mental fatigue.
7. Must be able to work with newborns (if assigned) to the oldest old (up to 20% of the work time). The majority of patient care (70% of work time) is the 18- to 65-year-olds.
8. The individual in this position must demonstrate appropriate knowledge of the principles of growth and development, which would include neonates to the oldest old. In addition, the therapist must demonstrate competency in examination/evaluation and treatment relative to age specific needs for the patient population for which they provide therapy, and as described in the department's practice standards.
WORKING CONDITIONS:
1. Works in hospital environments where there is some exposure to blood/body fluids/hazardous materials, communicable diseases (10% - 25% of work time) and unpleasant odors (up to 25% of work time).
2. Work requiring physical effort for lifting, transferring, and positioning of patients and/or equipment (up to 50% of work time) may result in musculoskeletal strain or injury with improper body mechanics.
3. Work requires use of sharp and/or electrical equipment (up to 20%), which could result in, cuts or burns with improper safety.
SUPERVISORY RESPONSIBILITIES: Assists with supervision and education of assistants and aides.
FISCAL RESPONSIBILITIES: 1. Patient charges are entered in an accurate and timely manner as appropriately supported by the medical documentation. 2. Hospital and department equipment is managed in an appropriate and cost-efficient manner.
HOSPITAL WIDE RESPONSIBILITIES: Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.

* Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
* Performs psychological record reviews in timely manner for patients in the Brain Health programs, including ComBHaT, 2-day evaluation and Special Operations Forces in the Intensive Clinical Program.
* Documents timely and relevant information in patient electronic medical record and Home Base database
* May provide direct clinical services to Veterans and/or family members, which may include group or individual therapy.
* May provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties May assist with crisis intervention and management.

The Neuro-recovery Clinic supports the recovery and reintegration of patients who have sustained severe neurological injuries (e.g., major stroke or brain bleeding, aneurysms, brain injuries) that have led to significant disabilities. The Neuro-recovery Clinic social worker will work on an interdisciplinary team to help individuals and their caregivers following their discharge from the ICU adjust to illness and promote optimal functioning.
The COVID Recovery Clinic focuses on assisting patients who are suffering from symptoms caused by Long COVID. The new, multidisciplinary COVID Recovery Center treats patients who have recovered from COVID-19 infection but still experience lingering symptoms or complications from COVID treatment. The Center provides a centralized location to evaluate post-COVID symptoms and coordinate care to help patients achieve the best possible recovery.
Job Summary
Conducts assessments to identify high risk psychosocial factors of patients/families that impact health status and helps hospital staff understand the influence of those factors upon the course of medical care. Works effectively as part of interdisciplinary health care teams. Participates on Departmental, Hospital, Satellite, community task forces and committees.
Supports the academic, teaching and research missions of the BWH Departments of Psychiatry and Social Work.
The Clinical Social Worker reports directly to the assigned Social Work Manager.
Clinical Triage Function:
* Review and assess referrals to Department of Psychiatry, via chart review, patient outreach and communication with referring provider to determine appropriate engagement in BWH psychiatric care.
* Conduct biopsychosocial assessments to determine risk and appropriate pathway for psychiatric care.
* Make arrangements for direct referral to appropriate facilities as needed; mobilize personal, hospital and community resources for identified patients. Assist medical staff in placement of patients in Department of Psychiatry or in the community.
* Act as a liaison to a wide range of collateral systems for effective patient care, including affiliated health centers and outside community agencies.
* Work actively to engage patients in entering treatment. When appropriate, provide brief treatment, such as motivational interviewing.
* Obtain consultation from Social Work Manager and leadership team and meet regularly to review and problem-solve complicated cases as needed.
* Communicate in timely and clear manner with providers who have referred to Psychiatry about triage process and referrals put in place for identified patients.
* Provide mandated assessments when abuse is suspected (child, disabled adult, elder) and safety assessment when domestic violence is reported. File reports as indicated.
* Identify patients' psychosocial, financial, legal, psychiatric or substance use that effect patient care management and collaborate with the team to facilitate patient care process.
* Coordinate family/team meetings, as needed and when appropriate. Provides psychosocial consultation on patient care planning and patient/family management and community resources.
* Participate in formal and informal clinical case reviews, research, data collection, quality initiatives, and special projects as requested.
Qualifications
QUALIFICATIONS
Education: Master's of Social Work Degree from an accredited program required.
Licensure: Current Massachusetts Licensed Clinical Social Worker (LCSW) required. Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) preferred.
Experience: Previous clinical social work experience in a hospital setting preferred.

POSITION SUMMARY:
Reporting to the Rehabilitation Therapies Supervisor/Manager, the Outpatient Physical Therapist conducts comprehensive assessment of outpatient clients with physical and functional deficits, planning and implementing individual/group physical therapy treatments to promote mobility, reduce pain, restore function, prevent permanent disability and restore independence, using various treatment techniques. Collaborates with other disciplines to arrange for appropriate follow-up care and facilitates a successful return to home and work environments. Provides student training according to departmental guidelines and participates in continuous education.
JOB RESPONSIBILITIES
Conducts physical therapy evaluation procedures according to professional and departmental guidelines.
Responds to all consults in assigned area within 24 hours; completes evaluation and documentation within 24 hours of consult (IP).
Reviews patient records, gathers necessary information, and obtains a physician's order prior to assessing the patient.
Selects evaluation procedures that reflect understanding of the patient's diagnosis, abilities, and limitations. Administers evaluation procedures according to the guidelines of standardized tests (including range of motion, manual muscle testing, mobility, air entry and breadth sounds); adapts evaluation procedures as indicated to accommodate the specific needs of patients.
Interprets evaluation results, develops an integrated diagnostic impression, and makes recommendations. Performs re-evaluations according to departmental policies, change in patient's status or response to treatment.
Educates patient and caregiver regarding evaluation findings and management of care recommendations.
Establishes treatment goals and plans that reflect the needs of the patients while meeting departmental, institutional, and third party reimbursement requirements.
