POSITION SUMMARY:
Hospital discharge is a dangerous time for patients: one in five will suffer an adverse event, such as a medication error, and nearly 25% will be readmitted within 30 days. This is especially true for patients who face communication barriers, including those with low health literacy, are elderly, or have a non-English language preference (NELP).
The goal of the proposed project is to deliver an educational intervention to patients recently discharged from the hospital. We will recruit patients with NELP currently admitted to the hospital. A nurse will deliver the intervention by phone within 72 hours of discharge. We will evaluate patient outcomes, including by a phone interview within 1-2 weeks of discharge.
JOB RESPONSIBILITIES:

Job Overview
This position is responsible for respiratory care for patients of all age groups, who are diagnosed with cardio-pulmonary dysfunction. Sets up various types of equipment to administer prescribed doses of medicinal gases and aerosolized drugs. Administers all types of respiratory therapy treatments including, oxygen delivery, mechanical ventilator management, diagnostic procedures (e.g., blood gas sampling and analysis, bedside pulmonary function testing, pulse oximetry, EKG's, BCLS, function and troubleshooting of respiratory therapy equipment). Works cooperatively within department and other services to create a system of quality health care.
Job Description
Minimum Qualifications:
1. Associate's degree from an accredited Respiratory Care program.
2. Licensed by the state of Massachusetts to provide respiratory care.
3. Registered Respiratory Therapist (RRT) or Certified Respiratory Therapist (CRT). If employed at TMC, must be RRT.
4. Basic Life Support (BLS) Certification.
Preferred Qualifications:
1. Bachelor's degree from an accredited Respiratory Care program.
2. One (1) year of experience as a Registered Respiratory Therapist.
Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Initiates patients on invasive and non-invasive mechanical ventilation; routinely checks and maintains proper function of ventilator in conjunction with the physician order.
2. Abides with infection control guideline for invasive and non-invasive equipment per departmental policy. Changes and adjusts ventilator parameters based on clinical assessment and blood gas analysis in accordance with physician orders.
3. Weans patients from mechanical ventilation and other therapies through patient assessment and adjusting parameters within prescribed protocols.
4. Administers inhaled medications in accordance with institutional guidelines derived from the department of respiratory care and pharmacy.
5. Administers oxygen using low flow, high flow, open and closed circuits.
6. Embodies patient and family centered care. Explains procedures to patients and family.
7. Provides services to analyze blood specimens for blood gases, glucose and electrolytes in neonatal and pediatric intensive care units.
8. Administers Positive Pressure Breathing, (e.g. CPAP or BiPAP), which consists of the application of positive pressure to the upper airway to promote better lung inflation and aerosol distribution.
9. Routinely assists in the transport of patients requiring life-support to and from referral hospitals.
10. Provides in-house transport of mechanically ventilated and other high-risk patients.
11. Provides non-traditional therapies, e.g. Nitric Oxide gas administration, as ordered.
12. Provides resuscitation and support services at all high-risk deliveries.
13. Monitors effectiveness of therapy using non-invasive techniques such as pulse oximetry, end tidal CO2 and transcutaneous PO2/PCO2, in addition to invasive techniques such as arterial blood gas sampling.
14. Analyzes inspired oxygen concentration using various oxygen analyzers.
15. Calibrates monitors and maintains Transcutaneous Oxygen Monitors, End Tidal CO2 and pulse Oximetry monitors.
16 Assists in Cardio-Pulmonary Resuscitation as part of Code Team primarily providing airway management and manual resuscitation. Commonly administers drugs via the endotracheal tube during cardiopulmonary rescue e.g. Lidocaine, Atropine Sulfate and Epinephrine.
17. Documents therapy, patient assessment, and interventions performed and test results in the progress notes of the medical record in a concise, accurate, and thorough manner.
18. Transcribes a detailed shift note for all patient's cared for during the shift.
19. Responsible for acknowledging the proper assembly of all respiratory therapy equipment prior to and during patient use.
20. Performs daily rounds and fairly bills patients for services rendered. Attend rounds daily with the intensive care unit critical care team and is an active participant in the patient's care.
21. Assists with development of in-service programs for departmental personnel.
22. Assists the Associate Director with the orientation of new employees; acts as a preceptor for new employees and is utilized as a clinical instructor for students enrolled in local universities.
Physical Requirements:
1. Able to walk and stand for approximately 90% of the time.
2. Able to transport respiratory therapy equipment (up to 100 lbs.) using carts or wheels on the equipment to and from patient areas.
3. Able to communicate with and assess the patient needs of different age populations.
4. Requires manual dexterity using fine hand manipulation to operate computer keyboard and or respiratory equipment.
5. Requires ability to see computer screen and reports.
Skills & Abilities:
1. Knowledge of medical terminology, physiology of the cardiopulmonary system, and Respiratory Science.
2. Ability to communicate effectively with all professional staff as well as all patients.
3. Ability to assess patient status and develop treatment plans.

Job Summary
This is a part-time position for someone interested in helping patients with complex psychiatric, medical and social problems improve their access and utilization of psychiatric/health care services.
A Community Health Worker (CHW) is a trusted member of the community who helps patients better access and coordinate their psychiatric care. We believe that CHWs have the skills and experience to understand what patients are going through and help them address the social and medical problems that lead to poor health. The goal of a community health worker is to assist the most high-risk patients with the tasks of getting psychiatric/medical care, working on health goals (such as arranging care, filling medication prescriptions, planning healthy meals, or finding time to exercise), and to help them deal with the "real-life" issues that keep them from staying healthy. Although a CHW is not in a clinical role, having the capacity to learn basic clinical concepts in order to identify when a referral to a licensed clinician is appropriate is an important skill.
The CHW will work with patients receiving care at Brigham and Women's Hospital outpatient psychiatric clinic. CHWs are integrated into the outpatient psychiatric team, serving as a bridge between the team and patients in the community. As a CHW in our department, you will develop trusting working relationships with your patients and be supported by a psychiatric team that includes social workers, nurses and psychiatrists.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Provide community health work services for patients identified as high-risk due to psychiatric or psychosocial challenges.
* Attend initial and continuing education training programs including self-directed reading and in-person and online learning.
* Work with patient and interdisciplinary team to set goals for patient's care.
* Meet patients in the emergency department, primary care clinic or hospital to reinforce and advance patient goals.
* Coordinate resources for identified problems including community mental health services, homelessness, substance abuse and food insecurity.
* Make weekly follow-up calls to patients.
* Motivate patients to meet their health goals.
* Provide culturally sensitive services to patients from different cultures.

Position Summary:
The Certified Occupational Therapy Assistant, under the direct supervision of the OTR/L, assists with the occupational therapy, evaluation, and treatment of patients in individual and group settings on our Geriatric Psychiatric unit. The COTA will also assist with the daily recreational therapeutic responsibilities of the Senior Behavioral Health Program.
Responsibilities:
Facilitate therapeutic group programming under the supervision of the OTR/L
Adhere to the current schedule using performance metrics to guide documentation
Modify activities as needed for patients during group sessions
Provide 1:1 treatments/interventions as necessary for patients unable to attend group sessions
Participate in interdisciplinary team meetings
Establishes and maintains effective therapeutic and working relationships
Supports and upholds the hospital's Excellence in Service initiatives
Receives continuing education required for job function
Requirements:
Education: Associate's Degree from ACOTE accredited program
Experience: Minimum of 1 year experience in an occupational therapy setting; geri-psych experience preferred

Chaplain (Suffolk County Jail Project) Department: Psychiatry Schedule: Part Time The Chaplain will serve as a core member of the treatment team within a new, 45-bed men's behavioral health unit operated by Boston Medical Center (BMC) located in the Suffolk County Sheriff's Department's (SCSD) Jail. In collaboration with Clinical and Administrative leadership, this role will help ensure the smooth provision of spiritual health services to incarcerated persons. Treatment will include spiritual assessment, individual counseling, family counseling and facilitation of group sessions. This role requires practicing non-judgmental patient centered care and cultural humility in order to care for an impacted patient population who is often marginalized within the healthcare and justice systems. Experience within corrections is not required, as the goal of this program is to deliver community level and quality of treatment inside the jail.
As part of this innovative new collaboration between BMC and SCSD, this role will report into the BMC Department of Psychiatry, directly reporting to the program's Clinical Director. Work will be completed within office and clinical settings, both housed within a setting of incarceration. This is a full time position, preference for those able to work evenings and/or Saturdays. The person who fills this position must adhere to all of BMC's RESPECT behavioral standards.
JOB REQUIREMENTS
EDUCATION:
* Master's degree in Divinity or Theology, PhD or DMin.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
* Certified by the Association of Professional Chaplains (preferred) or eligible for certification.
EXPERIENCE:
* 2 years of chaplaincy experience, experience as chaplain in correctional setting preferred.
* Must have endorsement by a recognized religious body or authority preferred
* Preference for experience within correctional environments and/or working with individuals with unmet behavioral health needs
KNOWLEDGE, SKILLS & ABILITIES (KSA):
* Ability to work collaboratively in a diverse and interdisciplinary team
* Knowledge of counseling and clinical pastoral care, including training in behavioral health (mental illness and substance use disorders), spirituality, and religious practices common to the community generally acquired through the completion of a degree program
* Skilled at engaging difficult to engage patients-build rapport, trust and utilizing de-escalation skills
* Ability to work in a fast paced environment, inclusive of organizational skills, time management skills, critical thinking, and problem solving abilities
* Strong interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with healthcare providers, patients and families, general public, staff members, external agencies, and other organizational leaders.
* Ability to work independently and to make decisions based on established policies and procedures.
* Proficient oral and written English communication skills.
* Bi-lingual and/or bi-cultural candidates preferred
* Intermediate Microsoft Office suite experience (i.e. MS Word, Excel, Access, Outlook)

ESSENTIAL RESPONSIBILITIES / DUTIES:
* Responsible for coordinating on site pastoral ministry in alignment with program philosophy as a member of an interdisciplinary care team.
* Manages day-today spiritual health needs of patients within the behavioral health unit.
* Collaborate with broader team of physicians, social workers, case managers, and peers to assess and adjust treatment plans, participating in relevant clinical meetings to support holistic recovery of patients
* Collaborates with SCSD staff to schedule chapel for religious services.
* Facilitates therapeutic groups for patients.
* Provide necessary oversight of any volunteers or students/trainees providing spiritual health care.
* Participate in staff meetings and clinical care conferences as needed.
* Complete any necessary trainings to ensure high quality EBPs are utilized on unit.
* Adhere to departmental/organizational policies and procedures including maintaining HIPPA HIPAA standards and confidentiality of protected health information.
* Comply with established metrics for performance and adheres to documentation and work flow standards including clearly documenting all progress and submitting required data within expected time frames
