Respiratory therapist job at Boston Medical Center
Provides advanced level of care, assessment, and education in Respiratory Therapy. Institutes, monitors, controls and evaluates complex life support systems that ventilate acute or chronically ill patients.
Demonstrated knowledge of Anatomy, Physiology, Microbiology, Pulmonary Physics, pulmonary Pathophysiology, Advanced Respiratory Care, Mathematics, Blood Gas Chemistry, and Pharmacology at a level generally acquired through completion of a two year AMA approved program in Respiratory Therapy.
Current or the ability to obtain board certification and RT licensure prior to start date.
Requires knowledge of related policies, procedures, standards and equipment at a level generally acquired during two to three years as a Respiratory Therapist. Knowledge of basic medical terminology required. Demonstrated ability to read and communicate in English language. Must have the ability to prioritize tasks and effectively work in a team environment, as well as excellent interpersonal and communication skills. Requires a caring and sympathetic attitude and ability to explain therapy and goals to patients and family members and to communicate effectively with other health care professionals. Basic computer proficiency inclusive of ability to enter and access data/information. Must be able to maintain strict confidentiality of all personal/health sensitive information. Must have the physical ability to meet core job requirements in accordance with practice setting and demands for the patient populations regularly served.

This position is responsible for providing all aspects of respiratory therapy in accordance with the policies and procedures of the Respiratory Care Department. Works with considerable independence to perform routine and complex patient care in all assigned areas. The respiratory therapist provides evidence-based patient focused care in a collaborative environment.
I. Major Responsibilities:
1. Works as part of a team to ensure optimal patient care.
2. Reviews patients electronic medical record for appropriate history, reason for admission, pertinent laboratory/radiological data, current orders.
3. Administers aerosolized medications.
4. Draws and analyzes Arterial Blood Gases. Reports Critical Values per hospital policy.
5. Consults with physician staff concerning the most efficacious forms of therapy and makes recommendations to enhance patient care (such as type, method, duration of therapy, suggested medications/dosages) compatible with physician orders and communicates these effectively with appropriate members of the healthcare team.
6. Monitors patient's physiological responses to therapy (such as vital signs, arterial blood gases), and consults with physician if adverse reactions occur.
7. Provides emergency care including assisting with intubation, Rapid Response Team, High Risk Delivery and Code Team.
8. Inspects, tests, cleans, and maintains respiratory therapy equipment as per department policy.
9. Demonstrates and instructs respiratory care procedures to new staff, other healthcare personnel, patients, and patient's families.
10. Provides clinical and general guidance to new, agency, per-diem, and student therapy personnel.
11. Provides direct supervision for respiratory therapy students.
12. Sets up and operates devices such as basic oxygen therapy, mechanical ventilators, CPAP/Bi-level support, NIPPV, high flow oxygen therapy in accordance with department policy and procedures.
13. Maintains all necessary patient and department records on the electronic medical record and medical administration record. Follows hospital policy/procedure regarding patient medication administration.
14. Participates in departmental quality assurance programs.
15. Maintains up-to-date knowledge of current techniques, developments, and issues in respiratory care through attendance at educational meetings, lectures, and conferences.
16. Provides verbal reports including diagnosis, significant past medical history, reason(s) for admission, treatment goals and plans, patient's response to treatment, and suggestions to optimize patient's treatment plan.
17. Sets up and operates devices such as a body plethysmograph in accordance with department policy and procedures.

2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.

2. Current Registered Respiratory Therapist (RRT) credential issued by the National Board for Respiratory Care (NBRC).
3. Current Massachusetts State Respiratory Care License.
4. Current Basic Life Support Certification.
5. Current Advanced Cardiac Life Support certification (required within 3 months of employment).
6. Current Neonatal Resuscitation Provider certification (required within 3 months of employment).
7. Current Pediatric Advanced Life Support certification (required within 3 months of employment).
8. Valid and unrestricted driver's license recognized by the Commonwealth of Massachusetts and access to a reliable, insured vehicle.
Preferred:
1. Bachelor of Science or higher.
2. Specialty credential issued by the NBRC (ACCS, SDS, NPS, or RPFT).
3. Current AE-C (Certified Asthma Educator) Certification.
Experience/Skills:
Required:
1. Ability to assess patients and make appropriate therapeutic recommendations along the continuum of care from admission to discharge including all aspects of respiratory care from floor therapies to mechanical ventilation and blood gas interpretation.
2. Ability to read, write and speak clearly in English.
Preferred:
1. Minimum two (2) years of acute care experience.

8. Inspects, tests, cleans, and maintains respiratory therapy equipment as per department policy.
9. Demonstrates and instructs respiratory care procedures to new staff, other healthcare personnel, patients, and patient's families.
10. Provides clinical and general guidance to new, agency, per-diem, and student therapy personnel.
11. Provides direct supervision for respiratory therapy students.
12. Sets up and operates devices such as basic oxygen therapy, mechanical ventilators, CPAP/Bi-level support, NIPPV, high flow oxygen therapy in accordance with department policy and procedures.
13. Maintains all necessary patient and department records on the electronic medical record and medical administration record. Follows hospital policy/procedure regarding patient medication administration.
14. Participates in departmental quality assurance programs.
15. Maintains up-to-date knowledge of current techniques, developments, and issues in respiratory care through attendance at educational meetings, lectures, and conferences.

I. Major Responsibilities:
1. Works as part of a team to ensure optimal patient care.
2. Reviews patients electronic medical record for appropriate history, reason for admission, pertinent laboratory/radiological data, current orders.
3. Administers aerosolized medications.
4. Draws and analyzes Arterial Blood Gases. Reports Critical Values per hospital policy.
5. Job Summary
Responsible for assessing, planning, organizing and administering Respiratory Care procedures in accordance with physician's treatment plan including mechanical ventilation and airway care; respiratory treatment; emergency respiratory maneuvers; transport and maintenance of critically ill patients for special tests and procedures. Performs and interprets test and procedures to assess and manage patient care in concurrence with physician. Oversees work of technical Respiratory Care personnel. Attends and participates in hospital educational programs and patient care rounds.
PRINCIPLE DUTIES AND RESPONSIBILITIES
1. Provides professional Respiratory Care including initiation and maintenance of mechanical ventilation. Utilizes neonatal conventional ventilators, High Frequency Oscillators, High Frequency Jet Ventilators, and INO ventilators for administration of Nitric Oxide.
2. Attends high-risk deliveries to assist physicians with intubation/NRP, surfactant administration, and transport of intubated patients. Responds to pediatric/neonatal emergencies (including CPR/NRP); establishes and maintains patient airway, providing appropriate oxygen therapy.
3. Performs intubations or assists in insertion, stabilization and maintenance of all artificial airways; and administration of Respiratory Care modalities. Applies use of prong and nasopharangeal CPAP devices, and oxygen delivery devices, nasal prongs and oxyhoods appropriately.
4. Attends daily bedside rounds and administers Respiratory modalities and medications including bronchodilator therapy, surfactant, oxygen, and nitric oxide. Assesses patients' conditions, progress and reaction to the therapy. Modifies treatment care plan accordingly.
5. Performs moderate complexity point of care (POC) testing for Blood Gas Analysis. Requires an Associate or Bachelors degree in chemical, physical, or biological science or medical laboratory technology or other qualifications as described in the Federal Register, 42 CFR, Subpart M 493.1403 - 1495.
6. Uses professional judgment regarding appropriateness of therapy; modifies treatment and/or informs physicians with recommendations for change. Administers appropriate tests and procedures as ordered including; oximetry, hyperoxia tests utilizing transcutaneous O2 monitoring, radial arterial blood gases.
7. Completes necessary documentation of all pertinent observations related to patient care and treatment administered with patients' responses to treatment. Reports to NICU Respiratory Therapy Clinical Coordinator and Clinical Supervisor and physicians. Maintains appropriate department records. Ensures proper and current physician orders with physician approval.
8. Participates in education of students, physicians, and staff. Explain and demonstrates respiratory techniques to house officers, medical students, nurses, and other hospital personnel. Provides guidance and constructive criticism to demonstrate appropriate methods as necessary to correct deviations from established standards.
9. Clinically supervises respiratory therapy students in patient areas. Assists and instructs students in performance of Respiratory Care procedures and maintenance of equipment.
10. Instructs patients' families in respiratory techniques and the use of equipment in the home necessary to continue patient care and rehabilitation.
11. Attends and participates in internal and external educational meetings to maintain and enhance clinical competencies including daily patient-care reports, staff meetings, and conferences as scheduled. The individual in the position must demonstrate appropriate knowledge of the principles of growth and development of the neonate. In addition, she/he must possess the ability to assess patient data relative to specific needs and provide care as described in the department's policy and procedures.
12. Assists in departmental research projects.
13. Maintains continuing education relating to cultural competency. The individual in this position must demonstrate appropriate knowledge and understanding of the diversity of any patient population to include but not limited to age, race, religion, ethnicity, ability, income, position, sexual orientation, gender or background.
Qualifications
1. Associate's Degree Respiratory Therapy required and Bachelor's Degree Respiratory Therapy preferred
2. Registered Respiratory Therapist (RRT) by the N.B.R.C. within three (3) months of hire
3. Currently licensed by the Commonwealth of Massachusetts as a Respiratory Therapist.
4. 1-2 year NICU training preferred or 2-3 years' minimum critical care training
5. NPS credential preferred
6. Work requires a minimum of ten weeks of orientation to acquire necessary familiarity with department policies and procedures.
7. Maintain NRP/BLS certification at all times.

Responsibilities
* Provides a variety of Respiratory Care Services to Dana Farber Cancer Institute main campus (Boston) and Chestnut Hill campus
* Applies scientific principles for the identification, prevention, and remediation of pulmonary and cardiopulmonary dysfunction

* Administers aerosolized medications
* Works with a multidisciplinary team to maintain natural and artificial airways
