SUMMARY OVERVIEW
Under the general direction of the Program Manager, the Case Manager is an integral part of the case management function of the agency. The Case manager provides ongoing assessments of the client's needs including eligibility for Boston Senior Home Care (BSHC) and other community resources. Furthermore, the Case Manager will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment.
This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation. The Case Manager is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, vendors, potential referral sources and other community agencies. Case Managers actively participate in BSHC organizational life.
ESSENTIAL FUNCTIONS

Through home visits conduct multi-dimensional assessment of elders' functional and social needs incorporating eligibility for State Home Care services, special programs and other community resources.
Provide ongoing assessment of client needs/status through contact with collaterals, informal supports and contact with the client via phone and regularly scheduled home visits. Adjust service planning and follow-up as indicated.
Through a needs assessment, knowledge of resources, and problem solving skills develop a service plan that incorporates the areas identified within the assessment. Seek supervision and/or interdisciplinary consultation for ongoing case management direction, high-risk client issues, vendor needs/issues, etc.
Implement the plan utilizing all available resources; BSHC vendors/programs and other community resources
Monitor the efficacy and quality of services provided by BSHC through contacts with client, their family, collateral contacts and vendor monitoring.
Maintain the client record through timely documentation that is complete and accurate of all related communications and required forms.
Provide ongoing advocacy for client, as follows:
Exhibit working knowledge of BSHC
Exhibit working knowledge of other community programs
Continually assist in the development of community resources knowledge within BSHC
Seek peer or supervisory consultation to explore unfamiliar resource areas.
Contribute to organizational life through active participation in team activities, committees, and work groups. Promote a positive working environment through cooperative and respectful interaction with others.
Through all external contacts with vendors, collaterals, and community at large continually provide outreach efforts as a BSHC representative in order to ensure access to services for all client groups.
Continually pursue knowledge of special populations and client issues through in-services, seeking outside conferences/trainings, with utilizing this new knowledge throughout the assessment and service planning function of the position.
COMPETENCIES
Ability to effectively communicate in person, in writing, and by telephone.
Ability to effectively utilize BSHC's computer and information systems.
Bilingual (English/Mandarin/Cantonese) is preferred.
SUPERVISORY RESPONSIBILITY
This position has no supervisory responsibilities.

REQUIRED/PREFERRED EDUCATION AND EXPERIENCE
A Bachelor's degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years' recent experience working with elderly or disabled adults; or
A Bachelor's degree and two years' clinical experience in the care of elderly or disabled persons.

We are the neonatal-perinatal care specialists for Tufts Medicine, providing care at Tufts Medical Center in a level III, 40-bed neonatal intensive care unit, and in the special care nurseries at Lowell General Hospital and Melrose Wakefield Hospital.
We supervise and provide services for our other community-based partners including Signature Healthcare Brockton Hospital, Lawrence General Hospital, and Holy Family Hospital.
In our network we oversee the neonatal-perinatal care of a system that covers approximately 9,000 deliveries per year, 1,200 intensive or critical care admissions per year, approximately 250 neonatal transports, and 200 high-risk neonates in our neonatal follow-up program.
Our Maternal-Fetal-Medicine (MFM) program is outstanding with numerous interactions at Tufts and within the Fetal Care Center at Boston Children's Hospital. We perform thousands of prenatal (inpatient/outpatient) consults each year.
Our division currently includes 23 neonatologists, 3 neonatal hospitalists, and 14 physician assistants. We work seamlessly with Tufts' Division of Pediatric Hospital Medicine to ensure that the neonatal and pediatric care across our network is exceptional.

SUMMARY OVERVIEW
Under the general direction of the Program Manager, the Case Manager is an integral part of the case management function of the agency. The Case manager provides ongoing assessments of the client's needs including eligibility for Boston Senior Home Care (BSHC) and other community resources. Furthermore, the Case Manager will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment.
This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation. The Case Manager is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, vendors, potential referral sources and other community agencies. Case Managers actively participate in BSHC organizational life.
ESSENTIAL FUNCTIONS

Through home visits conduct multi-dimensional assessment of elders' functional and social needs incorporating eligibility for State Home Care services, special programs and other community resources.
Provide ongoing assessment of client needs/status through contact with collaterals, informal supports and contact with the client via phone and regularly scheduled home visits. Adjust service planning and follow-up as indicated.
Through a needs assessment, knowledge of resources, and problem solving skills develop a service plan that incorporates the areas identified within the assessment. Seek supervision and/or interdisciplinary consultation for ongoing case management direction, high-risk client issues, vendor needs/issues, etc.
Implement the plan utilizing all available resources; BSHC vendors/programs and other community resources
Monitor the efficacy and quality of services provided by BSHC through contacts with client, their family, collateral contacts and vendor monitoring.
Maintain the client record through timely documentation that is complete and accurate of all related communications and required forms.
Provide ongoing advocacy for client, as follows:
Exhibit working knowledge of BSHC
Exhibit working knowledge of other community programs
Continually assist in the development of community resources knowledge within BSHC
Seek peer or supervisory consultation to explore unfamiliar resource areas.
Contribute to organizational life through active participation in team activities, committees, and work groups. Promote a positive working environment through cooperative and respectful interaction with others.
Through all external contacts with vendors, collaterals, and community at large continually provide outreach efforts as a BSHC representative in order to ensure access to services for all client groups.
Continually pursue knowledge of special populations and client issues through in-services, seeking outside conferences/trainings, with utilizing this new knowledge throughout the assessment and service planning function of the position.
COMPETENCIES
Ability to effectively communicate in person, in writing, and by telephone.
Ability to effectively utilize BSHC's computer and information systems.
Bilingual (English/Mandarin/Cantonese) is preferred.
SUPERVISORY RESPONSIBILITY
This position has no supervisory responsibilities.

A Bachelor's degree and two years' clinical experience in the care of elderly or disabled persons.
