Summary:

Essential Work Functions:
Evaluate patients' physical conditions using standardized tests, clinical examinations, and patient history to identify functional limitations and treatment need
Develop personalized rehabilitation programs based on assessment findings and patient goals
Implement physical therapy treatment plans using various techniques to improve patient mobility and reduce pain
Operate specialized physical therapy equipment and modalities safely and effectively, ensuring proper maintenance and calibration of devices
Assess patient progress, adjust treatment plans as necessary, and accurately document evaluations, interventions, and outcomes in the medical record
Work closely with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care and optimize treatment outcomes
Educate patients and caregivers on home exercise programs, proper body mechanics, injury prevention strategies, and self-management techniques to promote long-term functional recovery
Maintain a safe treatment environment by adhering to infection control protocols, safety guidelines, and regulatory standards while addressing patients' comfort needs during therapy sessions
Engage in quality assurance initiatives and professional development activities to remain current with best practices in physical therapy
Collaborate in discharge planning to ensure smooth transitions and continued patient progress in community or home-based settings

Overview

Oversee contractors and vendors to resolve repair and maintenance issues at Sage Dental facilities.
Respond promptly and follow up consistently to support overall success and workflow efficiency.
Vendor and supplier selection, relationship management, and assessment of cost, level of service availability, to efficiently serve the needs of the business.
Independently manage small capex remodels, minor remodel work, painting jobs and other miscellaneous improvements.
Qualify the scope, means, methods, schedule, etc. to ensure the needs of the business are met and with a proper plan to implement the facilities plan of Sage Dental locations.
Ownership of repairs or maintenance of the support center (corporate office).
Manage, or take ownership of support center corporate services for facilities and replenishment vendors.
Adherence to Sage Dentals standards for replacement design and branding image elements.
Subcontractor appropriation and the ability to pivot to other vendors as needed.
Evaluate and monitor contractors, vendors, and suppliers and ensure they are performing up to Sage's standards.
Work with the facilities management leadership team to ensure materials/services are obtained at competitive rates.
Approve and verify accuracy of facility invoices submitted by contractors, vendors, and suppliers.
Maintain and monitor any preventative maintenance programs relating to recurring services.
Invoice reconciliation, review, and approval of vendor pay requests.
Visit offices/sites when required to evaluate work under construction, punch lists, and/or issues negatively impacting practice operations.

Qualifications
3+ years' experience in a comparable role.
Bachelor's degree preferred (not required); High school diploma or equivalent required.
Multi-unit or multi-location facility management experience is preferred (not required).

Purpose & Impact:
Key responsibilities include the direct supervision of youth residents, providing quality care, ensuring a safe and secure environment and transporting and accompany/supervising residents on outside activities. This is a direct service delivery position at a primary location, but the position may work at any one of a number of regional group homes within a reasonable radius as shift needs arise.
These group homes house adolescent teens who have aggressive behaviors. The ideal candidate for this role is someone who would be comfortable working with at risk adolescent males and females. The ideal candidate would also not be intimidated by youth who have been impacted by abuse, neglect, and trauma. Take on a challenging role while having the ability to be a mentor and make an impact in the lives of youth.
Essential Functions:
Provide supportive environment for youth.
Assists children with preparing and eating meals, taking baths, maintaining hygiene, administering medication, and performing other daily routine tasks.
Transports individual and groups of children for appointments or field trips outside the facility
Provide daily security and up-keep of facility.
Develops, plans, coordinates, and participates in daily recreational and social activities, including outdoor programs and physical exercise.

Purpose and Impact:

Essential Functions:
Assists classroom staff in all classroom activities, field trips and outdoor activities included in lesson plans.
Assists in developing materials and providing activities related to the cultural background of the children and families served.
Helps supervise at mealtime, eating with the children to encourage good nutrition and social skills.
Helps keep the classroom and its contents clean and orderly.
Assists the teacher in any aspect of the program as needed (i.e. at family nights, copying newsletters, making phone calls, preparing classroom materials, etc.)
Serves as a temporary substitute in the periodic absence of the teacher assistant or teacher.
