Responsible for informing the community and creating awareness about Borinquen Medical Centers (BMC) services, including: substance use, mental health, HIV prevention, and prevention education on the risks of substance misuse, HIV/AIDS, and STDs. The Behavioral Health Outreach Specialist also conducts outreach in high-risk areas to recruit participants for testing and BMC services, provides needed linkages to service provision for individuals with mental health, substance use and/or co-occurring disorders, and participates in the coordination and facilitation of presentations to educate the community about our services. Description and Responsibilities Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky's talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served. The Awake Overnight Counselor provides guidance during the overnight hours and assists individuals with all morning routines. Through implementing procedures that ensure the health, safety, and well-being of individuals and general cleanliness and safety of the physical site, counselors ensure individuals are living a healthy, productive, and independent life. Counselors are strong advocates for individuals served, believe in the power of community, and acknowledge the importance of community integration! Other Key Responsibilities: Utilize crisis intervention or other means to intervene during a crisis situation, following established program and agency procedures. Implement individuals' medical and medication protocols in accordance with agency and MAP policies. Assist individuals to access and/or provide all needed transportation. Our mission is to provide homes that are safe, nurturing, and address the individual's personal needs and preferences. Every individual in an Open Sky residential program is encouraged to play an active role in community life, whether it be by volunteering, taking classes, participating in group outings, or joining a health club. Our dedicated staff ensures that individuals have the necessary supports and skills to build a successful and satisfying life in the community while enabling individuals to lead meaningful and active lives. The Asleep Overnight Counselor works with the Awake Overnight Counselor to support people with developmental disabilities during the overnight and morning hours. The Asleep Overnight counselor may sleep during the overnight hours, but is available in the event of crisis and assits in the morning hours.
