    *Employment Type:* Full time *Shift:* *Description:* The Physician Assistant (PA) provides primary medical management in collaboration with PACE Organization (PO) physicians. This role includes performing participant assessments, developing and implementing care plans, evaluating physical complaints, and delivering appropriate treatment. The PA educates participants and caregivers, functions as a member of the Interdisciplinary Team (IDT), and oversees daily clinic operations. The PA ensures timely communication with the IDT regarding participant conditions and maintains accurate documentation in compliance with organizational policies. *Essential Functions* * Demonstrate and uphold the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and Trinity Health PACE in all interactions and decisions. * Provide primary medical coverage in collaboration with PO physicians, including initial, semi-annual, annual, and as-needed assessments. * Develop and integrate care plans into the overall IDT plan of care; participate in discharge planning for acute and long-term placement. * Evaluate and treat participants during acute illness; manage chronic conditions and preventive health maintenance (immunizations, screenings, monitoring). * Prescribe medications and therapies prudently; refer participants to specialists as indicated. * Collaborate with the Medical Director and Clinic Supervisor to develop clinical policies, procedures, and standards of care. * Serve as a resource for staff education and training; maintain current knowledge in geriatrics and related fields. * Participate in IDT meetings, staff meetings, peer reviews, and provider on-call rotation. * Ensure compliance with all applicable laws, regulations, and organizational policies, including disaster recovery and business continuity plans. *Minimum Qualifications* * Graduate of an accredited Physician Assistant program with an unrestricted PA license in the state of practice. * Minimum two (2) years of experience as a Physician Assistant in a geriatric setting preferred. * At least one (1) year experience working with frail and elderly populations, or willingness to complete PACE-specific training upon hire. * Demonstrated ability to assess, plan, and provide care for frail and elderly participants. * Strong decision-making, conflict management, and organizational agility; ability to work independently with minimal supervision. * Compliance with state-specific health requirements (e.g., Employee Medical Statement in North Carolina). * *Position Highlights and Benefits:* * Comprehensive benefits package, including First Day medical coverage, dental, vision, and paid time off. * Low-cost medical, dental, and vision insurance plans. Our Pediatric and Adult Congenital Cardiology program is the state leader in congenital heart disease - providing comprehensive surgical, interventional and ambulatory care to patients with all forms of congenital heart disease. We perform more than 250 congenital cardiac surgeries and interventional cardiac catheterizations each year and continue to grow. As a non-academic institution, Echocardiographers at Presbyterian are on the front lines of congenital heart disease care - frequently providing the initial diagnostic assessment of patients with undiagnosed congenital heart disease and working with physicians to ensure each patient receives a comprehensive echocardiogram. Sonographers also participate in all OR and cath lab imaging - in addition to providing inpatient echocardiograms and clinic-based echocardiograms. We're currently offering: • Sign-on bonus of $20,000 • Relocation assistance of up to $6,000 for qualifying candidates. This is a full-time (1.0 FTE), non-exempt position. Responsibilities: Echocardiography imaging: Perform comprehensive, diagnostic images using 2D, 3D, color imaging to evaluate the structure and function of the heart. Patient care: Ensure each echo is done in a patient-centered, comfortable and safe environment. Sonographers explain the process to patients and alert physicians of critical findings or changes in patient condition that require further evaluation. Reporting: Prepare preliminary echo reports including relevant measurements. Outreach: Willingness to participate in outreach clinics up to once a month, including potential overnight stays at clinics that serve rural locations in NM with no other access to congenital cardiac care. Call: Participate in weeknight and weekend call. Sonographers take "Home Call" with call pay. On average, sonographers are called in overnight approximately once a month. Weekend call is typically 2-6 echoes on Saturdays and Sundays, and triaged by the on call pediatric cardiologist. Collaboration: Work as a team with fellow sonographers and physicians to ensure smooth patient flow and optimal imaging for every patient. Equipment maintenance: Assist echo lab managers with maintaining equipment and performing quality control. Continuing Education: Participate in educational sessions to ensure we provide the most up-to-date care to our patients. Responsibilities: When collaborating with other health care providers, the APC shall be accountable for knowledge of the statutes and rules relating to advance practice nursing and function within the boundaries of the appropriate advanced practice category The APC shall practice within the advanced specialty and role appropriate to his/her advanced educational preparation The APC acts independently and/or in collaboration with the health team in the observation, assessment, diagnosis, intervention, evaluation, rehabilitation, care, counsel and health teachings of persons who are ill, injured or informed of experiencing changes in normal health processes, and in the promotion and maintenance of health or prevention of illness When providing medical aspects of care, APCs shall utilize mechanisms which provide authority for that care These mechanisms may include, but are not limited to, protocols or other written authorization Assess and manage patient health problems including actual and potential conditions within the hospital, clinic or outpatient setting Effectively identify, evaluate and address disease prevention/health promotion issues of the population in practice while administering quality patient care Responsible for the diagnosis and treatment of acute, chronic and long-term healthcare issues Plan, implement and evaluate health care treatments Obtain history and physical examinations on patients of all appropriate age groups per scope of practice Assess and interpret laboratory and radiological findings Utilize critical thinking skills to individualize patient care in order to manage illnesses, promote well-being, and foster rehabilitative/disease preventive lifestyle choices Prescribe medications and supportive therapies to help maintain health Perform advanced procedures as verified and approved per credentialing committee Educate patients and/or families about preventive care, medical issues and use of prescribed medical treatments and/or medications Work in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration and/or referral Synthesize new knowledge gained from conferences, workshops and professional literature Teach other healthcare professionals in the classroom, clinical area and/or community about practice, techniques and procedures which are unique to that specific clinical setting Participate in basic science and clinical research projects Prepare statistical reports and analyses to document progress, adverse trends and appropriate recommendations or conclusions Maintain legible, accurate and confidential medical records This includes documenting all medical evaluations diagnosis, procedures, treatments, outcomes, education, referrals and consultations consistent with governing boards such as NCQA (National Committee for Quality Assurance), JCAHO (Joint Commission on Accreditation of Healthcare Organizations), federal and state regulatory standards Requirements: Advanced Nursing Degree or Master's Degree required. Current Texas State Advanced Practice Nurse (APN) license or PA license. Certification in Basic Life Support (CPR), American Heart Association. Experience in the specialty area preferred. Experience in both clinical and management areas. Demonstrated ability to problem-solve and market services effectively. Excellent oral and written communication skills.
