What you'll do as a Case Manager for Hep C:
Facilitate referral to care and system navigation for SUD, HCV, HIM, SMI, and social services to address Social Determinants of Health (SDOH)
Design, develop, implement, and supervise case management, after care, and follow-up policies and procedures for the program
Provide specialized case management and aftercare services to clients and conduct follow-ups as scheduled
May supervise clients in the community, carry a caseload of active clients, conduct group and individual counseling sessions, assist clients in achieving adjustments to specific problems or situations of a difficult or complex nature
Monitor emotional/psychological status of clients and arrange referrals to ancillary services.
Conduct workshops and seminars for clients and the community and maintain relations with referral sources and Hep C, health, addiction, social, educational, recreational, and vocational organizations for in-treatment and aftercare planning
Write reports, maintain necessary records, and assist other staff in discharge and aftercare planning.
Develop and implement procedures to complete clinical records and correspondence while maintaining compliance with confidentiality procedures and applicable state/federal regulations.
Occasional driving to conduct therapeutic community sessions.

Ability to establish and maintain effective working relationships with community agencies, organizations, and the public
Knowledge of Hepatitis C and intervention/treatment strategies.

What You'll Do as an Outreach Worker with Our PCCP Program:
The Outreach Worker is part of a team who engages in on-site outreach programming at BCCS Drop-in centers and statewide in the community with the medical mobile outreach team and the harm reduction team.
Distributes condoms, first aid kits, harm reduction supplies, and other materials as directed on mobile vehicle and while conducting outreach activities or in any BCCS drop-in centers.
Provides referrals to drug treatment, health care, and social services.
Maintains an accurate and current resource and referral source guide. Establishes and maintains working relationships with referral sources.
Promotes and advocates new programs and initiatives for new and existing participants and other community members.
Drives the BCCS RV or Van to daily mobile outreach sites, home deliveries, and/or events requested by the Supervisor.
Provides education materials on HIV/AIDS, HCV, Xylazine, Monkeypox, and any new trends (including but not limited to general information, testing sites, and referrals for care), safer sex, and risk reduction specific areas and specific populations designated in BCCS's contract and as directed by the Supervisor.
Staff's community outreach, education, and/or testing events during the day, in the evening, and/or on weekends, as directed by the Supervisor
Staff's the Drop-in Center, including but not limited to sitting at the front reception desk and signing in clients, directing clients to services, such as the laundry, shower, coffee, telephone, or computer, assisting clients with use of drop-in services, including but not limited to the laundry, shower, coffee, telephone, or computer, drop-in HIV and HCV education, testing, and counseling.
Enters data into data tracking software.
Provides wound care education and assists with wound care clinics both on site and in the community.
Collects drug supply samples and ships to drug checking center.
Provides case management to clients as needed.
Practices in an approach consistent with the philosophies of trauma informed care and recovery-oriented systems of care.
Works directly with Gibney Mobile Healthcare on outreach locations and within the community
Requirements:
Qualifications
REQUIRED: High school diploma or GED
PREFERRED
: Associate degree
REQUIRED: 6 - 12 months of experience in the behavioral health, substance use, or mental health field
PREFERRED
: Certified Peer Recovery Specialist or CPRS Eligible
REQUIRED: Valid driver's license with 2 points or less

Summary/objective
Under the supervision of the Crisis Integrated Services Coordinator, the Engagement Specialist plays a crucial role in coordinating care and providing support to children and youth experiencing crises. Summary/objective
Under the supervision of the Crisis Clinical Coordinator, the Youth Peer Support Specialist will provide peer mentoring services, include peer counseling, mentoring, support, and advocacy according to the needs, interests and perspectives of youth receiving services from the Mobile Response and Stabilization Services (MRSS) program due to an identified crisis. The ultimate goal of peer mentoring is to engage the youth in the support available, advocate for the needs of the youth, and assist in transitioning into other support services that may be needed in the community.
The focus of the Youth Peer Support Specialist is to support the youth during the risk assessment that occurs when a request is made to the MRSS program and to provide ongoing mentoring while the MRSS case is open. The Youth Peer Support Specialist will work closely with the MRSS crisis clinician to help the youth achieve their goals and find stability in the community.
In addition, the incumbent is responsible for documentation of mentoring services provided from intake through discharge. This position is available due to grant funding that will end on September 30, 2025.

Under the supervision of the Crisis Program Manager, the MRSS Crisis Supervisor plans, directs and coordinates activities of MRSS (Crisis Intervention/Prevention) to ensure that clinically appropriate goals and procedures are accomplished in accordance with risk factors, time limitations, funding requirements, and JCAHO and licensing regulations.

Work closely with the community mental health providers, law enforcement departments, State of Delaware hospitals and Prevention and Behavioral Health Services to facilitate appropriate services for children and their families.
Develop and maintain ongoing procedures to manage crisis situations ranging from decompensation of the mentally ill, drug/alcohol problems, suicidal/homicidal ideation, family conflict, physical and sexual abuse, etc.
Participate in one week a month on-call rotation for supervisory consultation with crisis clinicians who are assessing clients in the community
Uphold professional standards of treatment and the mission of Delaware Guidance Services in imparting services to children and their families.
Maintain a quality code of conduct, ethics and professionalism as demonstrated in the MRSS Program Manual.
Impart clinical knowledge and expertise in a manner congruent with providing goal-directed and team-oriented services towards children.
Communicate effectively and professionally with all customers, including children, parents, Department of Prevention and Behavioral Health Services and other state and county agencies.
Maintain crisis services to children and families in compliance with requirements published in the Department of Prevention and Behavioral Health Services Manual.
ADMINISTRATIVE RESPONSIBILITIES :
Responsible for coordinating and maintaining proper documentation of all incoming assessments from crisis evaluation. Verify that risk factors are addressed and all notations are precise and accurate.
Establish and maintain outcome measures.
Responsible for assigning and scheduling crisis counselors to specific shifts and maintain a sense of stability on those shifts.
Attend individual meetings with the MRSS Program Manager and management team meetings.
Responsible for weekly crisis program updates to the MRSSS Program Manager.
Participate in the interview process of potential candidates for the crisis program.
Responsible for evaluating crisis counselors' progress within the program and updating the MRSS Program Manager with staff concerns.
Facilitate training for newly hired crisis counselors.
Responsible for the coordination of cases and policies with the Department of Prevention and Behavioral Health Services.
Ensure staff knowledge of and compliance with appropriate licensing, contractual and other standards impacting daily operations.

Provides direct supervision of Crisis Bed Stabilization Coordinator/Therapist to ensure Crisis Bed Stabilization Program is operational at all times to ensure safety of clients, minimize risk factors and provide care integrity. Ensure program policies, procedures and expectations are met.
Engages in planning and program conception, including the development of other 24/7 and risk preventative services that keep children and families in the community safe
Implements, supports, and oversees the high-intensity services provided in subprograms that are established under the umbrella of crisis services. These programs consist of services that support youth at high to moderate risk for suicide, harm to others, significant instability following a traumatic event, or at risk of being removed from their home environment due to behavioral dynamics. All subprograms also require quick decision-making and turn-around time for services rendered.
Coordinates expedited community crisis responses as needed due to emergency traumatic/loss events that occur impacting youth in Delaware.
Maintains monthly data and complete monthly and quarterly reporting documentation about statewide crisis services and subprograms.
Oversees the case management and clinical documentation processes with Clinical Coordinators to establish procedures, ensuring rapid completion of crisis documentation, monitor chart completeness and case confidentiality.
Collaborates with community partners to address needs or concerns related to crisis intervention services. Community partners include schools, emergency departments, psychiatric inpatient facilities, pediatric offices, police departments, youth mental health service programs, etc.
Ensures program compliance with agency and accreditation standards.
Ensures that all contract goals and objectives pertinent to the program are met.
Establishes and maintains processes of effective recruitment of personnel.
Clinical Oversight
Provides direct supervision and clinical oversight of documentation of crisis clinicians, subprogram clinicians, and call center clinicians as needed when direct supervisors are unavailable or for licensure purposes.
Provide direct supervision of Crisis Clinical Coordinators and Crisis Bed Stabilization Coordinator/Therapist and assist them in development of leadership and clinical supervisory skills.

Summary/objective
Under the supervision of the Lead Crisis Bed Shift Supervisor, the Mental Health Technician is responsible for providing a variety of direct client care duties and unit support functions to children and adolescents with a wide variety of psychological and emotional problems (ie. aggressive and defiant behavior due to problems with emotion regulation, suicidal/homicidal ideation, plans, or attempts, family conflicts, etc.) to ensure the safety and wellbeing of clients in a Crisis Bed setting. The Behavioral Health Office manager has primary responsibility for clerical duties involved with coordinating client care at the site. These duties include greeting clients/visitors, answering phones, managing collections and insurance verifications, directing visitors/clients to the appropriate areas and performing routine clerical duties. It is also the responsibility of this position to model and maintain high professional conduct and standards.
As a Behavioral Health Office Manager with us, you will:
Answer telephones in a timely manner and direct all calls or messages to the appropriate department or person.
Complete reminder calls or correspondences for clients.
File and maintain all administrative documentation needed to stay in compliance with DE state and federal regulations.
Obtain client demographics, identification, authorizations and verifying insurance information at each visit.
Obtain necessary client information to meet current Federal guidelines and insurance use guidelines for electronic medical records.
Enter and scan pertinent client information into EMR including client authorizations, recent labs delivered etc.
Schedule follow-up appointments and inputting recall notices into EMR as needed
Identify and resolve minor client billing issues
Request medical charts from hospitals and other medical centers.
Obtain information sheet on new clients and prepares charts at intake, including initiating new charts or retrieving existing charts and signing intake documents.
Collections of payments, maintaining a list of self-pay clients and managing payments
Insurance verification on a daily basis
Collaborate with medical staff and clinical staff on day-to-day operations related to client care.
Have client sign preadmission forms (Financial Notification Agreement, Service Representative, Sliding Discount).
Is responsible for ordering office supplies and any other supplies as needed for clinical and building supplies.
Reviews and follows up to ensure ability to pay information and client documentation provided and entered the EMR; alerting the billing department as needed.
Lead insurance verification efforts in real time to ensure that co-pays, co-insurance, and deductibles are met according to the health insurance guidelines.
Check and address client alerts as clients come in for services.
Review 28/14 days/7 day/OBOT client's chart for financial forms compliance - Complete monthly reports for ModivCare. Verify attendance, contact information.
Perform other related duties as assigned by Program Manager

Summary/objective
Under the supervision of the Crisis Clinical Coordinator, the Crisis Clinician will provide rapid response screening and assessment and counseling intervention services to youth and families experiencing urgent psychological and emotional problems (ie. aggressive and defiant behavior due to problems with emotion regulation, suicidal/homicidal ideation, plans, or attempts, family conflicts, etc.) while maintaining safety and implementing therapeutic treatment plans in a community-based setting. Summary/objective
The Intensive Community Services Clinician is responsible for providing clinical behavioral health services in the community to DGS Clients. Therapists in this program will provide mental and behavioral health support to clients and their families, assisting them in being able to step-down to a less intense level of care.
Essential Functions:

Attends and actively engages in weekly individual supervision and monthly group supervision
Provide evaluative, diagnostic, and psychotherapeutic services to children, adolescents, and their families; and collaborate with DPBHS and other area providers as needed

Summary/objective
Under the supervision of the Crisis Clinical Coordinator, the Youth Peer Support Specialist will provide peer mentoring services, include peer counseling, mentoring, support, and advocacy according to the needs, interests and perspectives of youth receiving services from the Mobile Response and Stabilization Services (MRSS) program due to an identified crisis. The ultimate goal of peer mentoring is to engage the youth in the support available, advocate for the needs of the youth, and assist in transitioning into other support services that may be needed in the community.
The focus of the Youth Peer Support Specialist is to support the youth during the risk assessment that occurs when a request is made to the MRSS program and to provide ongoing mentoring while the MRSS case is open. The Youth Peer Support Specialist will work closely with the MRSS crisis clinician to help the youth achieve their goals and find stability in the community.
In addition, the incumbent is responsible for documentation of mentoring services provided from intake through discharge. This position is available due to grant funding that will end on September 30, 2025.

The Advanced Clinical Training Program (ACTP) Clinician is a Graduate level (Master's level) clinician responsible for providing clinical behavioral health services to children and families. Clinicians in this program further develop therapeutic skills, build core clinical competencies, and have the opportunity to practice these skills and competencies in a family-based child guidance setting. Clinicians provide therapeutic services under the direct supervision of a licensed clinician while earning their required hours toward licensure.
Essential Functions :
Clinical
Attends and actively engages in core ACTP training sessions in accordance with ACTP onset date
Provides evaluative, diagnostic, and psychotherapeutic services to children, adolescents, adults, and their families
Conceptualizes course of treatment inclusive of appropriate modalities, interventions, frequency and duration of clinical services
Assesses clients' needs and risks by utilizing crisis intervention skills and/or services
Applies knowledge related to developmental factors, family and community systems in order to develop and implement treatment planning
Maintains an assigned caseload in compliance with agency standards related to direct clinical care
Demonstrates comprehensive skills in providing evaluation, diagnosis, and therapeutic services to build resilience in children, adolescents, adults, and families
Provides feedback and suggestions for program improvement
Administrative & Clinical Documentation
Engages and collaborates with community resources, makes appropriate referrals, and consults with other agencies on behalf of clients
Collaborates with school staff regarding behavioral health services for students
Completes required documentation for clinical records within established timeframes.

Required Qualifications:

Two years relevant full-time post graduate experience working with children and families

What You'll Do as a Medical Assistant with Our HEP-C Program:
Support our medical providers and nurses while adhering to high professional conduct and ethical standards
Interview clients, assist with medical exams, collect vital signs, document medications and allergies, perform EKGs, obtain blood draws, collect and process specimens, order supplies
Coordinate physical and behavioral health needs for individuals with HCV and other health conditions.
Serve as the primary point of contact for patients enrolled in the Rapid Hepatitis C Treatment Program
Coordinate all aspects of the treatment plan, including scheduling appointments, labs, referrals and/or follow-ups. Track treatment milestones on a spreadsheet
Communicate effectively with pharmacies and insurance companies to ensure timely medication delivery.
Create and submit prior authorizations for medications when needed.
Perform Rapid Fingerstick tests
Collect drug screens and accurately record results
Maintain accurate documentation and data entry in electronic health records.
