    The primary purpose of the care manager position is to provide each assigned resident with routine daily nursing care and services in accordance with the residents' assessment and service plan and as may be directed by supervisor. Key Responsibilities: * Work cooperatively within the department as well as with other departments * Report pertinent resident information to the immediate supervisor and respond positively to resident inquiries or requests * Position requires daily social interaction with memory care residents, ability, to conduct specialized programming as well as providing care. * Active CNA/CHHA License * Experience in long-term care or assisted living preferred * Strong communication and interpersonal skills * Ability to multitask and remain calm under pressure Schedule Requirements: * Must be available for a minimum of 4 shifts per month * Must be available for at least one weekend shift and one holiday shift per month
