DUTIES/RESPONSIBILITIES:
Coordinate the health care and medical related case management for clients. Responsible for basic health assessments, checking vitals, blood pressure and weight. Provide ongoing health monitoring to manage and support clients' health needs. Make appropriate referrals and follow-up for medical, substance use, and/or psychiatric treatment. Observe, monitor and document client self-management of medication. Process physicians' orders, and obtain prescriptions. Collaborate with medical and psychiatric providers and hospitals. Develop and facilitate health-related educational groups and one-on-one client education. Participate in interdisciplinary team meetings and case conferences. Respond to medical emergencies and provide emergency first aid/CPR, and Narcan opioid overdose reversal when needed. Respond to other program crises as needed. Ensure compliance with regulatory and agency guidelines. Related duties as assigned.
HOURS:
Full-time, 37.5 hours per week
* Saturday 8am-6pm; Wednesday-Friday 12pm-10:30pm
QUALIFICATIONS:
NYS LPN license required. Bachelor's Degree preferred. Experience working with individuals experiencing homelessness preferred. Knowledge of mental illness and substance use disorder helpful. Proficiency in MS Office, including Word and Excel. Bilingual skills a plus. CPR training certification or willingness to complete certification training in CPR and opioid overdose prevention.

ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature.
ACT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week.
Overview
The LicensedPracticalNurse (LPN) functions as a primary practitioner who assesses and monitors ACT participants' clinical health and response to treatment. The nursing approach of assessment, planning, intervention, implementation and evaluation is used to assist individuals in their achievement of mental health, medical, rehabilitation and recovery goals.

Position Summary
The MMTP LPN dispenses medication to patients with opioid use disorder and is an active participant of the interdisciplinary treatment team in the provision of medical and clinical services.
Primary Responsibilities:
Dispenses and administers prescribed medication; maintains accountability, adequate record keeping, and documentation.
Assessment and verification of last methadone dose for patients returning from the hospital, detox, etc.
Carries out MD/PA orders, and assist during crisis, emergencies, and other procedures.
Works collaboratively with Physician, Director, and other staff as part of the treatment team.
Participates in clinic meetings, and interdisciplinary case conferences with clinical staff.
Assist in coordination of medication deliveries for appropriate patients, as indicated.
Assess patients and provide patient care within scope of practice.
Promotes a positive, constructive clinic atmosphere.
Knowledgeable of all state and federal regulations and accreditation standards as well as program policies and procedures and the role of a nurse in its operations, including confidentiality and HIPAA requirements.
All other duties as required by the supervisor.

Interacts with all levels of personnel within the organization and maintains external business relationships within the social services community.

Qualifications:
License to practice as a LicensedPracticalNurse in New York State or LicensedPracticalNurse meeting above criteria within 3 years.
2 years' experience in a substance abuse services environment preferred.
Demonstrates clinical competence and leadership traits (e.g., decision making ability, skills in problem solving and communication, prioritizing and team building)
